SCHOLARSHIP AWARD This Inaugural Scholarship of the BSF is for a one-time award of $2,500.00 for the use any college related student needs. HOW TO GET THE APPLICATION You may download the application materials at www.losbravos.org/scholarship as either a Microsoft Word file that you can complete on your computer or as a PDF file that may be printed and used the same as the hard copy document. complete application packets May 2006 - July 2006 Application processing August 2006 Award notifications sent ELIGIBILITY REQUIREMENTS To be eligible for this scholarship, you must:

• Be of Hispanic heritage - persons descended from Central American, Cuban, Dominican, Mexican, Puerto Rican, South American, or other Spanish cultures.

• Be a high school senior attending Layton High School and graduating in 2006.

• Be a U.S. citizen or legal permanent resident of the United States with a valid Social Security Number. All legal permanent residents must submit a copy of their valid permanent resident card or passport stamped I-551.

• Be enrolled full-time in a degree-seeking program at a U.S. accredited college or university in the U.S., Puerto Rico, or U.S. Virgin Islands for the 2006-07 academic year (enrollment is verified in the fall of 2005).

• Have a minimum cumulative grade point average (GPA) of 3.00 on a 4.00 scale.

• Apply for federal financial aid by completing the FAFSA form (Free Application for Federal Student Aid) for the 2005-06 college academic year. The FAFSA will be available at www.fafsa.gov after January 1, 2006. Student applicants selected to receive a BSF High School Scholarship Award will be asked to submit a

copy of their Student Aid Report in the fall of 2006 in order to receive a scholarship award check SELECTION CRITERIA Applicants are evaluated on the following criteria: • Academic record • Financial need (for educational purposes) • Community service • Personal qualities APPLICATION PREPARATION

PLEASE SUBMIT TWO (2) COPIES OF THE FOLLOWING DOCUMENTS (INCLUDING ESSAY RESPONSE):

1. Application (6 pages) – Use a computer or typewriter to complete your form. The minimum character size for all responses is 11-point. We strongly encourage you to type your application as it makes it easier for our selection committee to read; however, if you do not have access to a computer, you may neatly print the application in blue or black ink. Applications that are illegible will be disqualified. Check your essays for correct grammar, spelling, and punctuation. Use the official application form or a photocopy of the form. Answer all questions. Confine your responses to the space provided.

Community Service/Volunteer Activities section: Only list one activity per line. If the information you wish to list exceeds the allotted space, select those activities of longest duration or that are most meaningful to you. If you have not been able to participate in any community service or volunteer activities, please use the space to explain why.

2. Transcript(s) – Provide an official transcript from your current high school (unofficial transcripts will not be accepted). Transcripts should include cumulative GPA.

3. Recommender Form – This form must be completed by an instructor or advisor. It may not be completed by a relative. Provide the Recommender Form to the person who will complete it. Allow your recommender at least two weeks to complete the form. The Recommender Form must be confidential. Ask your recommender to return the Recommendation Form, and a photocopy, to you in a sealed envelope with his or her signature written across the seal. Submit the sealed recommendation with your application. You should pick a recommender who can address the following items: • Academic record, plans, and goals • Personal strengths, including motivation, leadership, and commitment • Community service and extracurricular activities

PLEASE SUBMIT ONE (1) COPY OF THE

FOLLOWING DOCUMENTS:

4. GPA Certification Form – This form must be signed by your counselor or other school official, verifying your weighted GPA on a 4.00 scale. If your school does not use a 4.00 scale, please ask your counselor to covert your GPA to a 4.00 scale. Submit the signed form with your application materials.

5. Financial Information Documentation – The

following document is essential to the processing of your application. You must include:

• A copy of your parent’s (or guardian’s) 2005 federal income tax return (Form 1040, 1040A or 1040EZ. Please send pages 1 and 2 only), AND a copy of all W-2 forms.

• If you [the student] filed a 2005 federal tax return (Form 1040, 1040A, or 1040 EZ), you must include a copy of your tax return (first 2 pages only) AND a copy of all W-2 forms Section B • Parents’ Household Size – include your parents and

yourself, your parents’ other children if your parents

will provide more than half of their support from July 1, 2006 through June 30, 2007.

• Number in College in 2006-2007 – include yourself as a college student but do not include your parents; include other household members only if they will attend, at least half time in 2005-2006, a program that leads to a college degree or certificate.

• Important: If your parents did not file a 2005 federal income tax return, you must provide documentation of their income, such as copies of social security statements, disability payments, paycheck stubs, or the like. Please call HSF if you are unsure what to provide.

Section C • For other real estate, be sure to include other rental

property, land, second or summer homes. For investments, include trust funds, stock options, savings bonds, mutual funds, money market accounts, Certificates of Deposit, college savings plans and prepaid tuition plans, commodities, precious and strategic metals, and any other investments.

• If the student's parents own a business, report the calculated equity. To determine the value, include the value of land, buildings, machinery, equipment, inventories, etc. Then, subtract the business debt.

All selection decisions are final and are not subject to appeal. Applications and support documents become the sole property of BSF and cannot be returned.
