Thank you for completing your required forms on Rank One!
ALL new students must have their Physical Exam Form & Medical History Form uploaded before the 2020/21 school year begins, or before any summer athletic on-campus camps/training, or fall sports practice.
ALL returning fall athletes must have their required forms completed before they can participate in fall sports practice, or any summer athletic on-campus camps/training.
ALL returning winter & spring athletes must have their required forms completed before they can participate in winter & spring sports practice, or any summer athletic on-campus camps/training.
Press the FCHS Parent Portal Link, which is located on the FCHS Rank One webpage,

Select “Proceed to Online Forms” or “Click Here” at the top of the page.
2. To create a new parent account, select “Create New Account”
If you already have a Parent Account, you may use your credentials
to login.
Not sure if you have an account? Select “Forgot your account?
Search for your account.” to see if you have already registered.
If you forgot your password, select “Click here” next to “Forgot your
password” and you will be emailed a re-set password link.
If you would like to use your Facebook or Google Plus
credentials to create your parent account, select the
appropriate icon to register.
If you do not want to use Facebook or Google Plus, you
may register for your account by entering your
First Name, Last Name, and Email Address
Create a Password and confirm the password (Passwords must be at least 6 characters long)
Select Register
After entering your information and selecting Register you will
be sent a confirmation email.
Click on the link in the email to continue.
The link will redirect your web browser. Click the link to sign into
your parent account.
If you do not receive the confirmation email, or have login issues with your user ID or password, please
contact Rank One: [email protected]
If you created your account with email, enter your Email Address and Password and select Sign In
If you used Facebook or Google Plus to create your account, select the appropriate icon to Sign In
Log In
You will link your student to your parent portal account. If you have multiple children you will have the
opportunity to link multiple students to your account. The FCHS Parent Portal will use your student’s
first and last name & date of birth as the search categories. Your student’s name will need to be
spelled exactly the way that it is input in Rank One. If you are receiving an error message, please
email [email protected] to find out how your student’s name is programmed in Rank One.
You will see “Success! Your student has been linked to your parent account” after successfully claiming your
student. To ensure you receive the correct forms to fill out, select any sports/activities that you expect your
student to participate in this year. After all Sports/Activities are selected click “Start Forms.”
If your student is not participating in athletics, please check “Physicals” under
“Combined Sports/Activities.” Please do not check “Art” or “Robotics” – we
are working to have these options removed.
After you choose your student’s sports/activities you will be directed to the forms list. Click “View” to see the forms.
If you need to add an additional sport/activity for your student, select “Add Sports/Activities” and select the additional sport/activity.
To add another student to your parent account, select “Click here to add a new student”
For Student Athletes
If your student is participating in athletics, you will see the righthand screen.
You will upload your student’s Physical Exam Form and Medical History
Form separately, in the designated areas. The medical forms will NOT be
automatically approved. A FCHS staff member will review the medical forms
to make sure that all signatures are completed (both forms need a medical
professional’s signature), and verify that a hearing screening was performed
for all new or transferred students.
You will e-sign the Athletics Handbook Acknowledgement Form. The
FCHS 2020/21 Athletics Handbook is embedded in the Athletics Handbook
Acknowledgement Form (please see the location of the handbook in the
bottom right screen shot). You can also find it on our Athletics Homepage,
or request a hard copy from our Athletics Department.
You will e-sign the Transportation Release Form
You will complete or update (for returning athletes) the TAPPS Student
Profile. Depending on your answers, some students will be prompted to fill
out additional forms (transfer students & international students). Please
contact [email protected], for help with completing these
additional forms.
You will complete the TAPPS Signature Page.
For Students NOT participating in Athletics
If your student is not participating in athletics, you will see the screen
to the right.
You will upload your student’s Physical Exam Form and Medical
History Form, separately, in the designated areas. The medical forms
will NOT be automatically approved. A FCHS staff member will review
the medical forms to make sure that all signatures are completed (both
forms need a medical professional’s signature), and verify that a
hearing screening was performed for all new or transferred students.
You will complete the TAPPS Student Profile. Depending on your
answers, some students will be prompted to fill out additional forms
(transfer students & international students). These additional forms
only apply to athletes, so you can disregard them.
The TAPPS Signature Page, the Athletics Handbook
Acknowledgment Form and the Transportation Release Form, only
apply to students who are participating in athletics. You do not need to
fill these forms out.
Additional Features inside your Parent Account include:
Inbox – Where you can receive Rank One messages from FCHS (if you do not download the Rank One app – see next slide)
Download and Print – Access documents that you can print such as the Physical form
Manage Account – Change your Password or Delete your account
Tutorials – Currently, these tutorials are standard Rank One instructions, which differ from FCHS specific instructions. Please use this tutorial
instead, and if you have any questions please email [email protected]

Logout – Click Logout to Sign Out of your account
For Falcon Athletes – please download the Public Rank One Mobile App to receive schedule alerts
and messages from coaches and the Athletic Department. Your login ID & password will be the same
ID & password that you created for the Rank One Parent Portal. If you have an older phone that is not
compatible with the R1 Public app, you will receive emails from Rank One, notifying you to log into the
parent portal to view messages.
