This document is designed to help you prepare for your presentation at the end of your safety improvement project

What is expected of me?

▪ Some of you will be asked to do a brief (under 10 minutes) presentation regarding your project

▪ Others will be asked to display a poster presentation

▪ We recommend that you limit your presentation to the format provided in this document

▪ On the day you may also be asked some questions about your work

Safety improvement project aim

How many, by when, measured by?

The aim of this project was to improve the reliability of prescriptionof low molecular weight heparin (LMWH) by introducing a ward round check list

Before the test less than 80% of patients had LMWH prescribed when indicated (Review of 10 charts)

The target was 100% of items on the checklist would be completed on one ward by the end of the project (within 8 weeks). The list of things to check includes the prescription of LMWH where indicated

Measured by checking off list during ward rounds to ensure it was done

▪ S – Summarise what was learnt. Revisions needed to checklist as certain points were not clear

▪ A – Check list was revised

▪ S – Completion of checklist increased to 80% – but in some cases there was confusion about who should be completing it
