Statements of “Comply” must be supported by evidence in a Bidders Bid and cross-referenced to that evidence. Evidence shall be in the form of manufacturer’s un-amended sales literature, unconditional statements of specification and compliance issued by the manufacturer, samples, independent test data etc., as appropriate. A statement that is not supported by evidence or is subsequently found to be contradicted by the evidence presented will render the Bid under evaluation liable for rejection. A statement either in the Bidders statement of compliance or the supporting evidence that is found to be false either during Bid evaluation, post-qualification or the execution of the Contract may be regarded as fraudulent and render the Bidder or supplier liable for prosecution subject to the provisions of ITB Clause 3.1(a)(ii) and/or GCC Clause 2.1(a)(ii). Bidder may submit offer which provides for superior specifications and /or better terms and conditions to the government at no extra cost. General Description

The supplier shall supply, install and commission one (1) Digital X-ray Machine into the Dental Bus of the hospital

The X-ray machine can be operated using the

power supply of the Dental Bus provided by its generator.

The x-ray system must be provided with vertical

cassette holder with stationary grid of high ratio and not showing any grid lines.

The digital radiography system must be Picture Archiving Communication System- (PACS) ready and able to retrieve and send imaging studies to the SPMC PACS through wireless and wired LAN. It can also be integrated with the RIS of the hospital. The machine can store images in Dicom format It should have a system and monitor screen that

The brand of the equipment being offered by the supplier must be in existence in the Philippine market carrying the x-ray machine for at least ten years.

The supplier shall provide a certificate of Dicom Conformance for the x-ray machines. The supplier or its principal can provide trained

service personnel/s for preventive and corrective maintenance of the x-ray machines.

principal by providing Service Engineer or Technical personnel to the supplier of its equipment

The factory trained Service Engineers employed by

the supplier or its Principal should be based in Davao City for preventive and corrective maintenance of the x-ray machines during the warranty period.

The service personnel referred in its bid must be the employee of the supplier or its principal for the past three (3) years has experience in maintaining a digital radiography x-ray system for the same length of time. It can be proven by submission of its training certificates, record of employment and SSS remittances for three years. It must be submitted together with its proposed technical specification as part of its technical bid.

e. It can respond within 24-48hrs should the existing equipment breaks down. In case of machine downtime, the supplier is given 3 days to do remedial action (repair). On the 4th day, if the unit is not operational, a penalty shall be imposed in accordance with the General Condition of the Contract of the bidding document. The equipment / machine must be able to comply with its installation and operation qualification which would be conducted in the presence of SPMC Biomedical Technician, End-user and the Company Engineer.
