ABOUT THE MIDWINTER CONFERENCE The Midwinter Conference is the largest educational event held annually in Southern California targeting the educational needs of the private practice Radiologist, Radiation Oncologist, Radiologic Technologist and Therapist.  Other medical professionals with a need or interest in radiology or radiation oncology education including business managers and leaders are also welcome to participate. There’s   is  no better way to reach the Cal i fornia community.     Based on LARS historical attendance and the addition of the CRS Annual, we expect 450 participants to attend this multi‐program conference February 25‐26, 2017. 2017 PROGRAM The Conference is organized into two concurrent clinical programs, each featuring multiple focus sessions or tracks. New in 2017 is the integration of the California Radiological Society’s (CRS) Annual Meeting into the Midwinter weekend. RADIATION ONCOLOGY FOCUS SESSIONS  (1/2 day each)  Breast Cancer     •    CNS Cancers  Head & Neck Cancer     •   Lung Cancer

CALIFORNIA RADIOLOGY SOCIETY ANNUAL MEETING  Features nationally prominent faculty providing 

presentations to educate radiologists and radiation oncologists about management, policy, and advocacy issues affecting the practice of medicine today. 

Both the Radiation Oncology and Radiology programs have been planned to provide practice‐oriented, scientifically–based educational sessions to help participants maintain and advance skills and knowledge as well as to promote lifelong learning and continual self‐assessment. Educational sessions are planned to be free of commercial bias. 

Participants will be able to integrate the scientific developments and clinical advances presented into practice and implement improvements and changes such as utilizing new and evolving modalities, applying new diagnostic and treatment techniques, revising diagnostic strategies and treatment guidelines or changing patient management strategies.    ABOUT LARS   LARS is a professional association with over  750 member radiologists, radiation oncologists and allied technologists who practice, have practiced or trained in the greater LA area. LARS mission is to provide clinically oriented, evidence‐based CME designed to maintain and advance knowledge and competence and promote lifelong learning and self‐assessment by addressing gaps between current and best practices.  We are a component organization of the California Radiological Society and of the American College of Radiology.  ABOUT CRS  CRS is a state chapter of the ACR with 1,200 diagnostic radiologist, radiation oncologist, resident and physicist members.  The CRS assesses the impact of regulatory, legislative and socioeconomic health issues and advocates on behalf of radiologists and quality patient care with both the California Legislature and regulatory agencies. The Society holds an annual meeting to disseminate this information and tools to empower radiology and radiation oncology practices to remain competitive in today’s challenging practice environment. 

WHO SHOULD PARTICIPATE The Midwinter Technical Exhibit is a forum for professional exchanges intended to enhance the practice of medicine and maximize patient care by providing participants with the latest, most accurate information available regarding the benefits, risks, and appropriate use of the products, technologies, services and therapies available. 

Companies that share this objective are encouraged to exhibit.   

Support opportunities have also been developed for those that wish to demonstrate a commitment to providing Continuing Medical Education for Radiologists and Radiation Oncologists.

EXHIBIT INFORMATION APPLICATION To apply for exhibit space, complete the Exhibit Application Form on page 7 and send the form including exhibit fee and a product description to Los Angeles Radiological Society (LARS). E, Sierra Madre, CA 91024 LARS Exhibit Contact: Beth Filip, Executive Director [email protected] / 626-836-5800 COST OF EXHIBIT SPACE The Basic Exhibit Package includes a 8’x8’ exhibit space, one 2’x6’ draped table, two chairs, one wastebasket, and a single line 7”x44” id sign for $1,300. Exhibit space rental does not include any additional materials such as, but not limited to, electrical connections, carpets, additional tables, chairs, etc. These items must be arranged for separately with the service contractor. *Note: Exhibit ballroom is carpeted. SERVICE CONTRACTOR Global Experience Specialists (GES) has been appointed the official drayage and decorating contractor. An Exhibit Service Manual containing information and rates for all labor, shipping, services, electrical connections, furniture, and miscellaneous equipment will be sent to your authorized representative by GES a short time after the assignment of space has been confirmed. An Exhibit Service Center will be maintained on the exhibit floor during installation hours and each morning from 7am- 8am to facilitate service requests. EXHIBITOR APPOINTED CONTRACTORS Requests to use Exhibitor Appointed Contractors (EAC) must be made on the “Notice of Intent to Use Exhibitor Appointed Contractor Form” included

in GES’ Service Manual and an Original Certificate of Insurance covering each proposed contractor must be received no later than Monday, January 23, 2017. Applications received after January 23 may not use an EAC. PAYMENT & CANCELLATION Applications must be accompanied by payment. LARS accepts the following forms of payment, VISA, MasterCard, or Company Checks/Money Orders in U.S. currency payable to Los Angeles Radiological Society (LARS), tax id. 95-3661585. If written notice of space cancellation is received by the LARS office prior to January 10, 2017 a full refund less a $300 administrative fee will be made. No refund will be made for cancellations received after January 10. SPACE ASSIGNMENT Exhibit space will be assigned first on priority points and second on the date that LARS receives the application. LARS will make every effort to accommodate your space request; however, space selection is not guaranteed. If all your choices are unavailable, LARS will assign the next best space available. Applications received with full payment on or before January 9, 2017 will be assigned a space on January 10. Notification of space assignments will be emailed to the authorized representative on January 11, 2017. Applications received after January 11 will be assigned space as available. LARS reserves the right to reconfigure the floor plan and/or relocate any exhibit if deemed advisable and in the best interest of the Conference. SUBLETTING OF SPACE Exhibitors may not assign, sublet or apportion to others the whole or any part of the space allocated and may not display goods or services other than those manufactured or regularly distributed by them.

CONFERENCE & EXHIBIT LOCATION The conference will be held at the Pasadena Convention Center located at 300 E. Green Street, Pasadena, CA 91101 – www.pasadenacenter.com. The conference will be utilizing the entire Grand Ballroom, with the exhibit hall located in section D. Refer to the exhibit floor plan - page 5. Educational sessions will be held in the adjacent sections of the Grand Ballroom, a floor plan showing the locations of the meeting rooms can be found on the Conference page of LARS website at www.larad.org. HOTEL ACCOMMODATIONS LARS is not holding a block of rooms for the conference; however, four of Pasadena’s major hotels are located within walking distance of the center. -Hilton Pasadena www.hilton.com/pasadena -Courtyard by Marriot www.courtyardpasadena.com -The Westin Pasadena www.westin.com/pasadena -Sheraton Pasadena www.sheraton.com/pasadena REGISTRATION & NAME BADGES Exhibit personnel must be bona fide employees of the exhibiting company. Each company, whose space is paid in full, may have a maximum of four (4) exhibit booth personnel (per 8x8 space) participate in the conference at any given time. Exhibit badges must be requested on or before February 15, 2017. Staffing a booth in shifts is acceptable as long as the number of personnel at the conference does not exceed the maximum of 4. An exhibit booth personnel badge entitles the holder to admittance to the Exhibit Hall only. Exhibitor personnel may attend educational sessions on a space available basis, however, no CME credit or registration materials will be provided unless the representative registers as an attendee.

RULES & REGULATIONS SPECIFIC TO COMMERCIAL SUPPORT 1. LARS encourages supporting companies to also display in the Technical Exhibit Hall. Our goal is to provide an exhibit hall

where attendees can learn about new products that will enable them to do their jobs more efficiently and effectively, to review and update technical knowledge of equipment and services they may already use, and to learn what’s available to help them provide the best possible care to their patients.

2. Support of this meeting requires a written agreement of Commercial Support be signed by both parties and returned to LARS prior to the Conference. LARS will forward its standard Letter of Agreement (LOA) to you upon receipt of your application for Support. In the event your company’s compliance department requires its own LOA be signed, LARS is able to substitute your LOA as long as it includes all the required elements of a written agreement as outlined by the ACCME in its Standards of Commercial Support.

3. These items will only be included in the Midwinter Conference if Corporate Support is confirmed in writing prior to January 9, 2017.

RULES & REGULATIONS All matters and questions not covered in these Rules and Regulations are subject to the decision of the Los Angeles Radiological Society (herein referred to as LARS), or its Executive Director. These Rules, along with the information provided in the exhibit prospectus are an integral and binding part of your contract for exhibit space or commercial support with LARS. ELIGIBILITY Qualified exhibitors or commercial supporters are those firms, organizations, and agencies whose products, services, technologies, and therapies are in harmony with and supportive of the objectives of the Conference. LARS retains sole authority to determine the eligibility of any company to exhibit or provide commercial support. LARS may refuse exhibit space to a potential exhibitor for any reason. MEETING PURPOSE & CONTENT The Midwinter Conference has been planned to provide practice-oriented, scientifically based educational sessions to help LARS members and conference participants maintain and advance skills and knowledge as well as to promote improvements or quality in healthcare, physician life-long learning and continual self-assessment. Educational sessions are intended to be free of commercial bias and have been planned without influence from commercial interests and prior to the solicitation for technical exhibits or commercial support. The concurrent exhibit hall is offered to provide a forum for professional exchange of non-CME information regarding new and updated equipment, devices, services and therapies directly related to radiology and radiation oncology clinical and/or business practice. By signing an application and participating in the conference as an exhibitor or commercial supporter, company acknowledges:

1) LARS controls, independent of commercial influence, the planning and implementation of the conference including: needs identification, determination of objectives, selection of presentation methods, content, faculty, planners, moderators, and activity evaluation. 2) The educational sessions are non-promotional and focused on educational content. To maintain independence from commercial influence or bias, exhibit or commercial support companies shall not engage in scripting, targeting points for emphasis, or any other actions designed to influence the content of the educational sessions. 3) It will not distribute product-promotion material or product-specific advertisement of any type except in the designated exhibit hall and company representatives will not engage in sales activities while in any room where CME activity takes place. 4) LARS has control over all decisions relating to the disposition and disbursement of exhibit fees and support funds. Fees/funds may be used to offset conference expenses including: (a) support of meals and refreshments of modest value to conference attendees as long as they do not take precedence over the educational sessions, (b) reasonable honoraria, travel, lodging, and meal expenses for event faculty or presenters. Fees/funds shall not be used: (a) to pay for travel, lodging, meal expenses for spouses, attendee guests or non-faculty attendees, (b) for compensation to the conference chairman, planners, joint sponsors or any others involved with supporting the event other than as an incidental benefit made available on the same terms to all attendees. All exhibit fees and support funds associated with the conference must be given with the full knowledge and approval of LARS and payment must be made directly to LARS.

Exhibit fees and support funds are not conditioned on LARS’ acceptance of advice or services concerning, faculty, participants or other educational matter, including educational content of the activity. By assigning exhibit space or accepting commercial support, LARS acknowledges: 1) All exhibitors are equal regardless of size and should be given an equal opportunity, within reason, to present their products in the most effective manner to the audience. 2) All commercial supporters are equal regardless of size and should be given equal opportunity, within reason, to participate and be recognized for support. 3) Disclosure shall be made to participants, prior to the beginning of the conference, regarding company’s exhibit participation and/or support without reference to specific products, 4) Disclosure shall be made of any known commercial relationships between LARS, conference faculty, moderators, and planners and that such disclosure shall never include use of a trade name, product-group message or corporate logo. SECURITY & LIABILITY LARS or its officers, directors, agents and employees cannot guarantee against loss, theft or damage and will assume no liability for damages or theft or guarantee the exhibitor against loss of any kind. The exhibitor agrees with the Society to assume the entire responsibility and liability for losses, damages and claims, including but not limited to lost earnings and punitive damages deriving out of this contract or injury, theft or damage to exhibitors displays, equipment and other property brought upon the premises of the Pasadena Convention Center and shall indemnify and hold harmless LARS, the Pasadena Convention Center and their respective agents, servants, and

employees from any and all such losses, damages, and claims, including attorney’s fees and costs. LARS, its officers, directors, agents, and employees also shall not be liable for, and are hereby released from, any claims, liabilities, losses, damages or expenses relating to or arising out of this contract or any injury to any personnel or exhibitor or to any other person or any loss of or damage to any property of exhibitor or to any other property where such injury, loss, or damage is incident to, arises out of, or is in any way connected with this contract or exhibitor’s participation in the exhibition, and the exhibitor shall be fully responsible for any such injury, loss, or damage, including attorney’s fees and costs and the exhibitor shall protect, indemnify, hold harmless, and defend LARS, its officers, directors, agents, and employees provided that the foregoing shall not apply to injury, loss or damage caused by or resulting from the negligence of LARS, its officers, directors, agents, or employees. In the event the exhibit hall is destroyed or damaged so as to prevent an exhibit from occupying assigned space during any part of the exhibition period; or in the event occupation of assigned space during any part of the exhibition period is prevented by strikes, Acts of God, national emergency or other cause beyond the control of LARS, then the exhibitor will be charged for space during the period it was or could have been occupied by the exhibitor. The exhibitor hereby waives any claim against LARS, its directors, officers, agents, or employees for losses or damages which may arise in consequence of such inability to occupy assigned space, its sole claim against LARS being for a refund of rent paid for the period it was prevented from using the space. INSURANCE Exhibitors must maintain general public liability

insurance against claims for personal injury, death or property damage in any way connected with the exhibitor’s participation in the exhibition, in an amount of not less than two million dollars ($2,000,000) for any one occurrence and must name LARS as an additional insured under such insurance. Such insurance must contain a waiver of any right of subrogation as to any claims against LARS, its officers, directors, agents or employees. All agents or representatives performing services at the Pasadena Convention Center directly for an exhibitor other than the exhibitor’s employees must provide LARS with Original Certificates of Insurance no later than January 23, 2017. Photocopies of Certificates of Insurance are not acceptable. Badge checkers provided by the Society will be on duty at all times when the Exhibit Hall is open to prevent those not registered for the conference from entering the exhibit hall. Security guards will not be provided by LARS. It is the sole responsibility of each exhibitor to secure their exhibit, its contents, and all personal belongings. Equipment cannot be left in the exhibit area unattended. ARBITRATION Any controversy or claim arising out of or relating to this contract shall be settled by arbitration, in Los Angeles, California, in accordance with the rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrator may be entered in any court having jurisdiction thereof. EXHIBIT LABOR All work involved in the erection, touch-up painting, dismantling, and repair of all exhibits may fall under union jurisdiction. This work is to include wall coverings, floor coverings, pipe and drape, painting, hanging of signs and/or decorative materials from the ceiling, placement

of all signs, and the erection of platforms used for exhibit purposes. This does not apply to the unpacking and placement of merchandise. Full-time employees of exhibiting companies may set their own exhibits provided that one person can accomplish the task in less than one (1) hour without the use of tools. If the exhibit preparation, installation, or dismantling requires more than one (1) hour, exhibitors must use personnel supplied by the Official Services Contractor. However, please note that when union labor is required, exhibitors may provide company personnel to work along with union installer in Southern California on a one-to-one basis. Any full-time company personnel involved should be prepared to produce some type of company identification when engaged in these activities. FREIGHT HANDLING All work involved in the loading and unloading of all trucks, trailers and common and contract carriers, as well as the handling of empty crates and the operation of material handling equipment, is under union jurisdiction. The union also has the jurisdiction of the unloading, uncrating, unskidding, leveling, as well as the reverse process. Full-time employees of exhibiting companies may ‘hand carry’ material provided that it is limited to only what one person in one trip can carry and they do not use material handling equipment. When exhibitors do choose to ‘hand carry’ material, they may not be permitted access to the loading dock/freight door areas. GES Exposition Services will not be responsible for any material we do not handle. ELECTRICAL All hardwiring ordered outlets to the line side of the exhibitors’ equipment is under union jurisdiction. All plugs over 120 volts will be plugged in by electrical union

personnel. Exhibitors may plug in their own plugs of 120 volts to their ordered outlets. All exhibitors are expected to comply with any union requirements in effect and as outlined in the “SHOW SITE WORK RULES” section of the GES Exhibitor Service Manual. GRATUITIES GES work rules prohibit the SOLICITATION OR ACCEPTANCE of tips in cash, product or gifts in kind by any employee (union or non-union). Our employees are paid appropriate wages denoting professional status, therefore tipping of any kind is not allowed. STANDARDS FOR EXHIBITING LARS is committed to presenting CME activities that promote improvements or quality in healthcare and are independent of the control of commercial interests. As part of this commitment, LARS has outlined here its standards for commercial participation in the Midwinter Conference. 1) It is expressly understood that in leasing and using space in the exhibit hall, the exhibitor agrees to abide by LARS’ Exhibit Rules and Regulations, as well as the ACCME Standards for Commercial Support of Continuing Medical Education. Refer to the ACCME Website - www.accme.org for a copy of the ACCME Standards for Commercial Support. 2) Drugs, chemicals or similar preparations used in the diagnosis and/or treatment of disease, or medical publications that contain advertisement of such drugs, which do not conform to the rules of the Council on Clinical Pharmacology and Therapeutics of the American Medical Association, cannot be exhibited.

3) Distribution of literature, samples, etc., by firms which are NOT participating in the Commercial Exhibit is prohibited. Evidence of violation of this rule should be reported immediately to a Continuing Education Committee member or the Executive Director of LARS. 4) No entertainment or social functions may be scheduled that conflict with conference program or exhibit hours—7:30am to 5:00pm Saturday and Sunday—or that conflict with scheduled LARS social events—6:30pm—9:30pm February 25, 2017. 5) Exhibit companies may not include as part of their display the use of LARS members, invited speakers, or other individuals giving lectures or short presentations in person in the exhibit hall during the scheduled educational activity. Brief videotape messages are permissible providing that they are not presented in a theatre style setting. 6) Unethical conduct or infraction of rules on the part of the exhibitor or his representative, or both, will subject the exhibitor or his representative to dismissal from the Exhibit Hall, in which event, it is agreed that no refund will be made by LARS. 7) Exhibitor’s displays must not obstruct the view of neighboring exhibitors. Structures over three (3) feet in height may not exceed three (3) feet from back wall. 8) Sound devices above conversation level will not be permitted in any booth. 9) Company representatives staffing the exhibit are limited to (4) four, and must remain inside the exhibit space area. At no time can solicitation be made in the aisles or from any area outside that of the booth space. Additionally, representatives of companies may not engage in sales activities while in any room where an educational activity takes place. 10) In the interest of providing meeting attendees with information about the variety of products and

services available to radiology and associated healthcare professionals, LARS permits companies to set up exhibits at LARS meetings. The attendance of exhibitors at LARS meetings does not constitute the Society’s endorsement of any product or service offered. LARS makes no representations, express or implied, regarding the nature or quality of the products or services offered by meeting exhibitors. Attendees are urged to consult with their own legal advisors before signing any contract with an exhibiting company. Any points not covered in these Rules and Regulations or elsewhere in the Technical Exhibit & Commercial Support Prospectus are subject to settlement by LARS or its Executive Director. LARS reserves the right to change or modify any rule or regulation, or any specification therein, when deemed advisable and to the best interest of the Society.
