The quotation form can be had from the Panchayat Office by showing dealer certificate, PAN Card and the experience certificate of having supplied of abovesaid items. The cost of the quotation form Rs. 100/- (Rupees one hundred only). 11. The application shall be received on 10.10.2016 upto 3.00 PM at Panchayat Office, Bambooflat-II and after scrutiny, the quotation form shall be issued on 10.10.2016 upto 4.00 PM. The application should accompanied with Dealer Certificate, PAN Card and the requisite documents such CST etc. 12.
