    This Loss Prevention Program has been prepared as required byOPMSANGs Construction

    Safety requirements asserted in the Statement of work and FAR documents. OPMSANGs

    GeneralInstructionsandotherrelevantrecordstobeimplementedintheProject.

    5 PROGRAMOUTLINE

    5.1. INTRODUCTION

    It is the policy of Tawreek Dimensions Est. herein after known as the TDE, that no

    employee of OPMSANG or subcontractor or public will be put at risk under any

    circumstances. TDE will obey the spirit and all Saudi Arabia Government (SAG)

    Regulations,OPMSANGsSafetyRegulationsnowpublishedandthatmaybepublished

    inthefutureduringthedurationoftheContract.

    Thisprogramestablishesandsetsoutmanagementand supervisionguidelines for the

    LossPrevention

    activities

    of

    the

    company.

    Itoutlinestechniquesandproceduresforsafeguardingthecompany'sgreatestasset its

    WORKERS.

    It isemphasizedthat thecontentofthismanual isabasicpartofcompanypolicy,and

    the application of its provisions mandatory. Supervision at all levels will be held

    responsible for the proper implementation and observance of the procedures and

    standardshereinprescribed.

    5.2.STATEMENT

    OF

    POLICY

    The safety,health andwellbeingof itsemployeesareofparamountconsideration to

    TDEinallitsundertakings. Experienceprovesthatthesafeandefficientperformanceof

    Work thoughcostlyor timeconsuming,TDEManagementsetsahighstandard for the

    protectionofitsemployeesinSafety,HealthandWelfare.Inrecognitionofthisprecept,

    TDEwillconstantlyworktowards:

    4.3.2.5. Instituteapropersystem forreporting, investigatingandestimating thecostof

    injury, damage and fire. Promote action to prevent recurrence and initiate

    analysistodiscoveraccidentcausesandtrends.

    4.3.2.6. Make sure that in tendering, at planning stages, and throughout the contract,

    allowanceis

    made

    for

    suitable

    and

    sufficient

    equipment

    to

    enable

    the

    job

    to

    be

    donewithminimumrisk.

    4.3.3. CONSTRUCTIONSUPERINTENDENTS

    4.3.3.1. Understand thecompany'spolicyandappreciate the responsibilityallocated to

    eachgradeofsupervision.

    4.3.3.2. Know the requirements of EM 385-1-1 and relevantpartsofSAGWorkmen's

    Regulationandensuretheyareobserved.

    4.3.3.3. Ensure that tenders are adequate to cover sound working methods and

    reasonablewelfarefacilities.

    4.3.3.4. Determineattheplanningstage:

    a. Themostappropriateorderandmethodsofworking.

    b. Storageareas,access,etc.

    c. Hazardswhichmightarisefromoverheadorundergroundservices.

    d. Facilitiesforwelfare,firstaid,andsanitation.

    4.3.4.3. Keep in touch with new developments in the field of accident prevention,

    4.3.4.5. Ensure that any equipment on site is suitable and has the necessary valid

    certificate; and to draw site management's attention to any defects or

    deficiencies.

    4.3.4.6. Investigate any injury to personnel, loss ordamage toequipment,nearmisses

    andaccidents,

    the

    corrective

    action

    taken

    on

    the

    recommendations

    made

    in

    thehazardreport.

    Certificate.

    Check thatperiodic tests, inspections,andmaintenancearecarriedoutwhendue,

    DocumentedpreuseInspections&workpermits(PeriodicInspectionSpotters,Tires,

    Lights,Brakes,Mirrors,Rolloverprotection,Backupalarmseffective,warningsigns,

    12. WELDINGANDCUTTING

    Onsite welding and cutting will be required during various phases of construction.

    Hazardswill arise frommolten slag,ultraviolet rays,open flames,hotelectrodes,and

    electric shocks. These hazards are increased if equipment is defective or improperly

    used. The followingactionswillbe implemented tominimizeanyweldingandcutting

    hazards:

    11.1 Restrictgreaseandoilnearoxygen.

    11.2 Store, handle, and use oxygen and acetylene cylinders chained in a vertical position,

    whetherfullorempty.

    11.3 Ensure OPMSANGs colorcoding of cylinders is recognized by welders and other

    workers.

    11.4 Protectequipmentandpersonnelfromtemperatureextremes.

    11.5 Storeoxidizersawayfromflammablecylinders.

    11.6 Providewarningsigns.

    11.7 Examine all welding equipment immediately before use and ensure regular

    maintenance.

    cordset),therelationshipoftheplugandreceptaclecontactsshallfirstbe

    checkedto

    ensure

    that

    they

    are

    of

    proper

    mating

    configurations.

    e) Attachmentplugsandreceptaclesmaynotbeconnectedoralteredinamanner

    whichwouldpreventpropercontinuityoftheequipmentgroundingconductor

    atthepointwhereplugsareattachedtoreceptacles.Additionally,thesedevices

    maynotbealteredtoallowthegroundingpoleofaplugtobeinsertedinto

    slotsintendedforconnectiontothecurrentcarryingconductors.

    f) Adapterswhichinterruptthecontinuityoftheequipmentgroundingconnection

    maynotbeused.

    g) Energizedplugandreceptacleconnectionsmaybehandledonlywithinsulating

    protectiveequipmentiftheconditionoftheconnectioncouldprovidea

    conductingpathtotheemployee'shand(if,forexample,acordconnectoris

    wetfrombeingimmersedinwater).

    h) Overcurrentprotectionofcircuitsandconductorsmaynotbemodified,even

    INTRODUCTION

    TDEwillprovideandensurethatonlymetalortimberladdersofOPMSANGapprovedtypesare

    providedtoensureasafemeansofaccessandegresstotheworkplace.Laddersshallconform

    tosectionII9.2oftheSACSMandSAScaffoldingSafetyHandbook(SASSH)requirements.

    15.1.SELECTION

    A laddermustbeof theproper length for thejob tobedone. If it is tobeused for

    to

    aheight

    of

    at

    least

    0.9

    meter

    feet)

    above

    thelandingplace.

    Metal ladders,ladderswithmetalreinforcedsiderails,andladderswhicharewet,shall

    notbeusednearelectricalequipmentwithexposedliveconductors. Suchladdersshall

    haveawarningnoticeattachedtoguardagainstusenearelectricalequipment.

    Aluminumladdersshallnotbeusedwherethereisalikelihoodofcontactwithmaterials

    harmfultoaluminum,suchascausticliquids,damplime,wetcement,andseawater.

    15.2.CONDITION

    Each laddershallbeexaminedbeforeusebytheuser. Thosewithsplitorbrokenside

    rails,missing,broken, loose,decayedordamaged rungsor cleatsorwithother faulty

    equipmentshallbetaggedDONOTUSEandremovedfromservice.

    Rungsshallbeproperlymortisedintosiderails. Cleatsshallbeinsetbyonehalfinch,or

    fillerblocksusedonthesiderailsbetweenthecleats. Cleatsshallbeuniformlyspaced

    30.5centimeters(1foot)fromtoptobottom.

    15.3.POSITION

    Thesiderailsofaladdershallbeequallysupportedonafirmlevelsurface. Boxes,blocks

    barrelsetc.shallnotbeusedasameansofsupport. Theareaatthebaseoftheladder

    must be kept clear. Ladders shall not be used in a horizontal position as platforms,

    runways,orscaffolds.

    Laddersshallnotbesupportedontheirrungsorcleats. Rungsorcleatsshallnotbeused

    tosupportscaffoldplanks.

    Whereeverpossible, ladders shallbe setatanapproximateangleof75degrees (one

    footoutwardforeveryfourfeetupward).

    Aladdershallalwaysbepositionedsothatthereisspacebehindeachrungorcleatfora

    properfoothold. Thereshallbenoobstructioninthewayofamansfoot,particularlyat

    thelandingplatform.

    15.4.USE

    Whereanextension ladder is fullyextended, theminimumoverlapofsectionshallbe

    fourrungs. Splicingorlashingladderstogetherisnotpermitted.

    Menascendingordescending laddersshallnotcarrytoolsandmaterials intheirhands.

    Toolsmaybecarried inpocketsoronspecialbelts,provided there isnoriskof injury,

    andmovementisnotimpaired. Materialshallbeloweredsecurelytiedorinabasket.

    Amanworkingon,orfromaladder,musthaveasecurehandholdand,bothfeet,onthe

    same rungor cleat. If thework tobedone requires theuseofbothhands, a safety

    hangerisrequired. Onlyonepersonshallbeonaladderatonetime.

    Jobmade laddersshallbeconstructed for their intendeduseand to thestandardsset

    forth

    by

    the

    OPM

    SANG.

    15.5. GENERAL

    Ladders shall bemaintained in good condition at all times. Joints shall be tight, all

    hardwareandfittingsshallbesecurelyattached,andmovablepartsshalloperatefreely

    withoutbindingorundueplay.

    Laddersmustnotbepainted.

    All electrical works, installation and wire capacities shall be in accordance with the

    INTRODUCTION

    All scaffolding work shall meet the requirements of OHSAs related guidelines and

    recommendations.

    8. Donotclimbverticalposts,horizontalmembers,orbracing.Accessmustbeprovidedto

    allworkplatforms.Accessladdersmustextendatleast3feetaboveplatform.

    9. Horizontal and/or vertical diagonal bracing may be required to prevent rack of the

    scaffoldstructure.

    10. Do not use of truss bearers without thorough consideration for the loads to be

    supported.Donotcantilevertrussbearersorotherhorizontalmembers.

    11. Guardrailsmustbeusedonallopensidesandendsofscaffoldplatform.Bothtopand

    midrailsarerequired.Guardrailsshallbeprovidedwhenminimumheightisincreasing.

    12. Toe boards are requiredwhenever people are required towork around the scaffold

    platform.

    for threemonthsandwillbe issuedafterthecranehasbeen Inspectedand

    foundserviceablebytheOPMSANGscraneinspector.

    17.2.2. Allcranesusedforthesiteshallbeequippedwithfireextinguisher,seatbelts,

    backup alarmand spark arrestor.Allmobile cranesworking forOPMSANG

    shallbeequippedwithloadmomentindicatorsandshallbeequippedwithan

    antitwoblockdeviceonatwoblockdamagepreventionfeatureforallpoints

    oftwoblockingi.e.Jibs,extensions,etc.

    17.2.3. Cranesshallnotenterarestrictedarea,oroperatewithinoneandaquarter

    boom lengths of an oil line, gas line,oroverheadpower line, until awork

    permitforthatparticularcranehasbeenissued.

    17.2.4. Work Permitwill notbe issued if the operatordoesnothavewithhimhis

    OPMSANGsCertificate.

    17.2.8. Noliftingdevices,suchasAFrames,Chainhoists,Spreaderbars,etc.,shallbe

    useduntiltheyhavebeeninspectedandstickeredbytheOPMSANGsCrane

    InspectionGroup(CIG).

    17.2.9. Man baskets shall be inspected and stickered by the OPMSANGs Crane

    InspectionGroup(CIG),beforebeingused.

    17.2.10. All slings shallbe inspected at leastmonthly andmarkedwith thatmonths

    safety color tape. All slings shall be documented on a sling log book as

    specified by theOPMSANG. All slings,whennot in use, shall be properly

    storedtopreventdamaged.

    18.3.OPM

    SANGS

    OPERATOR

    /EQUIPMENT

    CERTIFICATION

    Only qualified personnel in possession of the required SAG License andOPMSANGs

    certificationshallbepermittedtooperateequipment.Craneoperatorsshallbequalified

    inawrittenandapractical test inDhahran.Personnelmustbeable toreadandwrite

    EnglishorArabictotakethewrittentest(Reminder:TheOPMSANGcranecertification

    states the types of cranes that the crane operator is certified to operate. The crane

    operator shall not operate any cranes other than those stated on the certification.

    OperatorsshallnotliftmanbasketsunlessOPMSANGcertified.)

    CranesmusthaveacurrentOPMSANGscraneinspectionsticker.Inorderforthecrane

    tobe inspected,acertifiedoperatororcertifiedcranemechanicforthatcraneshallbe

    available.

    Shallacranefailexaminationduetomajorfaults,andthosefaultscannotberectifiedat

    thejobsite,itshallberemoved.Itshallnotbeallowedtoremainonsite.

    Failure to complywith these requirements could result inworkbeing stopped in that

    part of the operation until those deficiencies have been corrected, as stated in the

    19.1.SAFEPRACTICES/PROCEDURES

    The following outlines precautionarymeasures need to be observedwhen operating

    oil, water, hydraulic levels, signals and indicators system, guards, limits

    switches andother safety equipment to ensure smoothoperations. Check

    stickers,passes/inspectiontagsandfirefightingequipment.

    TDEstorageyardsshallbe laidout inaccordancewithOPMSANGsStandardswithrespectto

    combustiblematerialsshallbeinterspersedwithnoncombustiblematerials. Storageareasand

    aislesshallbedelineatedbymarkers.

    TDE shall submit to OPMSANGs Representative for review prior to implementation plan,

    layouts and storage areas for storingmaterials and equipment and additionally, site offices.

    Thesestorageareasshallbedesignedandlaidoutasfollows:

    Adequate access road to the lay down yard and shall be kept clear and unobstructed at all

    times.

    Adequatelightingtoilluminatethestorageyards.

    A systemof securitymenofeight (8)hours shift shallbeplanned tomaintainsecurity in the

    absenceofworkersinthesite.

    TDE storage yardswillbe laidout inaccordancewithOPMSANGsStandardsandwillbe

    indicatedinTDETemporaryLaydownYardDwg.

    The perimeter fence of sound construction and design appropriate for the intended

    protectionof

    the

    area

    will

    be

    provided

    for

    materials

    storage

    yards.

    ThefenceshallbefrequentlyinspectedbyTDESafetySupervisorandSecurityGuard.

    Twoaccessgateswillbeprovidedforthestorageareadirectlyoppositetoeachother.

    Materials shallbe arrangedwithOPMSANGs Standardswith respect to spacingof rows,

    concentrationwithinrows,andconcentrationofmaterials.

    Stockpilingofcombustiblematerialsshallbeinterspersedwithnoncombustiblematerials.
