This Loss Prevention Program has been prepared as required byOPMSANGs Construction
Safety requirements asserted in the Statement of work and FAR documents. OPMSANGs
GeneralInstructionsandotherrelevantrecordstobeimplementedintheProject.

5 PROGRAMOUTLINE
5.1. INTRODUCTION
It is the policy of Tawreek Dimensions Est. herein after known as the TDE, that no
employee of OPMSANG or subcontractor or public will be put at risk under any
circumstances. TDE will obey the spirit and all Saudi Arabia Government (SAG)
Regulations,OPMSANGsSafetyRegulationsnowpublishedandthatmaybepublished
inthefutureduringthedurationoftheContract.
Thisprogramestablishesandsetsoutmanagementand supervisionguidelines for the
LossPrevention
activities
of
the
company.
Itoutlinestechniquesandproceduresforsafeguardingthecompany'sgreatestasset its
WORKERS.
It isemphasizedthat thecontentofthismanual isabasicpartofcompanypolicy,and
the application of its provisions mandatory. Supervision at all levels will be held
responsible for the proper implementation and observance of the procedures and
standardshereinprescribed.
5.2.STATEMENT
OF
POLICY
The safety,health andwellbeingof itsemployeesareofparamountconsideration to
TDEinallitsundertakings. Experienceprovesthatthesafeandefficientperformanceof
Work thoughcostlyor timeconsuming,TDEManagementsetsahighstandard for the
protectionofitsemployeesinSafety,HealthandWelfare.Inrecognitionofthisprecept,
TDEwillconstantlyworktowards:
a. Themaintenanceofsafeandhealthyworkingconditions.
b.
Consistentadherence
to
safe
operating
practices
and
procedures
to
minimize
accidentsandIllness.
c. Conscientious observance of all OPMSANGs requirements as stipulated in

4.3.2.4 Reprimandanymemberofsupervisorystaffforfailingtodischargesatisfactorily
theresponsibilitiesallocatedtohim.
4.3.2.5. Instituteapropersystem forreporting, investigatingandestimating thecostof
injury, damage and fire. Promote action to prevent recurrence and initiate
analysistodiscoveraccidentcausesandtrends.
4.3.2.6. Make sure that in tendering, at planning stages, and throughout the contract,
allowanceis
made
for
suitable
and
sufficient
equipment
to
enable
the
job
to
be
donewithminimumrisk.
4.3.3. CONSTRUCTIONSUPERINTENDENTS
4.3.3.1. Understand thecompany'spolicyandappreciate the responsibilityallocated to
eachgradeofsupervision.
4.3.3.2. Know the requirements of EM 385-1-1 and relevantpartsofSAGWorkmen's
Regulationandensuretheyareobserved.
4.3.3.3. Ensure that tenders are adequate to cover sound working methods and
reasonablewelfarefacilities.
4.3.3.4. Determineattheplanningstage:
a. Themostappropriateorderandmethodsofworking.
b. Storageareas,access,etc.
c. Hazardswhichmightarisefromoverheadorundergroundservices.
d. Facilitiesforwelfare,firstaid,andsanitation.

deficiencies
have
been
corrected,
authority

safetysupplies&equipmentasneeded.,including:

4.3.4.3. Keep in touch with new developments in the field of accident prevention,
personalprotective
equipment
and
first
aid
equipment,
procedures.
4.3.4.4. Coordinate efforts ofmanagement and labor, and direct them towards totally
safeoperations.
4.3.4.5. Ensure that any equipment on site is suitable and has the necessary valid
certificate; and to draw site management's attention to any defects or
deficiencies.
4.3.4.6. Investigate any injury to personnel, loss ordamage toequipment,nearmisses
andaccidents,
and
the
corrective
action
taken
on
the
recommendations
made
in
thehazardreport.
4.3.4.7. Carryoutfrequentsitechecks,inadditiontotheofficialweeklyinspections,ofall
sitesforwhichheisresponsible.
4.3.4.8. Encourage all grades to Safety consciousness during safety meeting and to
suggestwaysofImprovingSafetyandpreventinglossanddamagetoequipment
andmaterials.

is
not
only
essential
forsafetybutisalsoindicativeofefficientsupervision.
4.3.6.4. Eachsupervisorisresponsibleforthepropertrainingoftheemployeesreporting
to him. Job hazards and safe procedures shall be fully explained to each
employeebeforecommencingwork.
4.3.6.5. Itisalsothesupervisor'sresponsibilitytoseethatallneededPersonalProtective
Equipment (PPE)meetsOPMSANGsStandardsand isused inaccordancewith
SafetyRules&Practices.
4.3.6.6. Encourageemployeessuggestionsandgivesthemimmediateconsiderations.
4.3.6.7. Be familiarwithWorkPermitProceduresofOPMSANGsOperating Instruction
ManualonSafety.

8.1. REQUIREMENT
TDEshallensurethatadailyandweekly inspectionoftheworksiteshallbeconducted
andlistofviolations,hazards,unsafeconditionsoractions,andimproperhousekeeping
notedandcorrected. Thusupgradethecompanysjobsafetyevaluation.Thecompany
shallconductweekly inspections inadditiontoweekly inspectionswiththeOPMSANG
Site Representative or Safety Officer when required. Safety reports and corrective
actionsshallbeatopicattheweeklyjobsitemeeting.Theinspectionchecksheetwillbe
submittedtoOPMSANGaspertheweeklyjobsiteprogressmeetingsandallinspection
reports
shall
be
kept
on
file
&
made
available
to
OPM
SANG
upon

A breach in the conditions of issue, or a change in circumstances, may lead to

12. WELDINGANDCUTTING
Onsite welding and cutting will be required during various phases of construction.
Hazardswill arise frommolten slag,ultraviolet rays,open flames,hotelectrodes,and
electric shocks. These hazards are increased if equipment is defective or improperly
used. The followingactionswillbe implemented tominimizeanyweldingandcutting
hazards:
11.1 Restrictgreaseandoilnearoxygen.
11.2 Store, handle, and use oxygen and acetylene cylinders chained in a vertical position,
whetherfullorempty.

workers.

INTRODUCTION
TDEwillprovideandensurethatonlymetalortimberladdersofOPMSANGapprovedtypesare
providedtoensureasafemeansofaccessandegresstotheworkplace.Laddersshallconform
tosectionII9.2oftheSACSMandSAScaffoldingSafetyHandbook(SASSH)requirements.
15.1.SELECTION
A laddermustbeof theproper length for thejob tobedone. If it is tobeused for
accessor
as
aworking
place,
it
shall
rise
to
aheight
of
at
least
0.9
meter
(3
feet)
above
thelandingplace.
Metal ladders,ladderswithmetalreinforcedsiderails,andladderswhicharewet,shall
notbeusednearelectricalequipmentwithexposedliveconductors. Suchladdersshall
haveawarningnoticeattachedtoguardagainstusenearelectricalequipment.
Aluminumladdersshallnotbeusedwherethereisalikelihoodofcontactwithmaterials
harmfultoaluminum,suchascausticliquids,damplime,wetcement,andseawater.
15.2.CONDITION
Each laddershallbeexaminedbeforeusebytheuser. Thosewithsplitorbrokenside
rails,missing,broken, loose,decayedordamaged rungsor cleatsorwithother faulty
equipmentshallbetaggedDONOTUSEandremovedfromservice.
Rungsshallbeproperlymortisedintosiderails. Cleatsshallbeinsetbyonehalfinch,or
fillerblocksusedonthesiderailsbetweenthecleats. Cleatsshallbeuniformlyspaced
30.5centimeters(1foot)fromtoptobottom.
15.3.POSITION
Thesiderailsofaladdershallbeequallysupportedonafirmlevelsurface. Boxes,blocks
barrelsetc.shallnotbeusedasameansofsupport. Theareaatthebaseoftheladder
must be kept clear. Ladders shall not be used in a horizontal position as platforms,
runways,orscaffolds.
Laddersshallnotbesupportedontheirrungsorcleats. Rungsorcleatsshallnotbeused
tosupportscaffoldplanks.
Whereeverpossible, ladders shallbe setatanapproximateangleof75degrees (one
footoutwardforeveryfourfeetupward).

basewillbeunabletocontrolaladdermorethan6meters(20feet)inlength.
Aladdershallalwaysbepositionedsothatthereisspacebehindeachrungorcleatfora
properfoothold. Thereshallbenoobstructioninthewayofamansfoot,particularlyat
thelandingplatform.
Whereladders
have
to
be
suspended,
both
side
rails
shall
be
lashed
top
and
bottom
to
provideequal support. Where long laddersareused, they shallalsobe lashedat the
centertopreventlateralmovement.
15.4.USE
Whereanextension ladder is fullyextended, theminimumoverlapofsectionshallbe
fourrungs. Splicingorlashingladderstogetherisnotpermitted.
Menascendingordescending laddersshallnotcarrytoolsandmaterials intheirhands.
Toolsmaybecarried inpocketsoronspecialbelts,provided there isnoriskof injury,
andmovementisnotimpaired. Materialshallbeloweredsecurelytiedorinabasket.
Amanworkingon,orfromaladder,musthaveasecurehandholdand,bothfeet,onthe
same rungor cleat. If thework tobedone requires theuseofbothhands, a safety
hangerisrequired. Onlyonepersonshallbeonaladderatonetime.
Jobmade laddersshallbeconstructed for their intendeduseand to thestandardsset
forth
by
the
OPM
SANG.
15.5. GENERAL
Ladders shall bemaintained in good condition at all times. Joints shall be tight, all
hardwareandfittingsshallbesecurelyattached,andmovablepartsshalloperatefreely
withoutbindingorundueplay.
Laddersmustnotbepainted.
Wherea ladder iscarriedbyoneman,thefrontendshallbekepthighenoughtoclear
mensheads,andspecialcareshallbetakenatcornersandblindspots.

17. SCAFFOLDING
INTRODUCTION
All scaffolding work shall meet the requirements of OHSAs related guidelines and
recommendations.
Management
TDE identifies that everyones safety depends upon the proper erection and safe use of
scaffolding and stress upon the need to inspect scaffolding before each use to see that the
assemblyhas
not
been
altered
and
is
safe
for
use
as
per
the
scaffolding
Safety
handbook
and
applicableG.I.
A. PriortoErection
1. Jobsiteconditions,suchasproximitytoelectricpowerlines,weatherprotectionscaffold
coverings, highwinds and falling object protection shallbeevaluated and adequately
providedfor.
2.
Inspectall
equipment
before
use.
Keep
all
equipment
in
good
repair.
Do
not
use
equipment that isdamagedordeteriorated.Scaffoldingshallrestonsoundbase.Sand
ormadeupgroundshallbefullycompacted.
3. Scaffolddesign shall includeanalysisof loadcarryingmembersbyqualifiedpersonnel.
Scaffold shall be erected, moved and modified only under the direction of trained
(competent)Scaffoldbuilders.
3.a Scaffoldbuildersshallusefallprotection(fullbodyharness)andfallarrestsystemwhen
workingatheightsof1.82mtr.(6feet)orgreateronallscaffolds. Personnelshallbetied
off
at
all
times
while
on
scaffolds.
4. Allequipment shallbe inspected tosee that it is ingoodconditionand isserviceable.
Damagedordeterioratedequipmentshallnotbeused.
5. Woodplankare inspected to see that it isgraded for scaffolduse, is soundandgood
condition,straightgrained,freefromsawcutsandholes.
6. Scaffoldassemblyshallbedesignedtocomplywithsafetyrequirement.
7. AllScaffoldSupervisorsandInspectorsshallbeOPMSANGCertified.
8. OPMSANGsScaffoldTagSystemshallbeused.

9. Employeesshallbedisciplinedforfailuretouseproperfallprotectionwhileonscaffolds
asperOPMSANGProgressiveDisciplinaryPolicy.
B. ErectionoffixedScaffold
1. Providesillsofadequatesize,properlyspaced.Baseplatesmustbeinfirmcontactwith
bothsillsandverticalposts.Careshallbetakenwhenscaffoldsaretobeerectedonfill
or otherwise soft ground. Any part of a building or structure used to support the
scaffoldingmustbecapableofsupportingtheloadtobeapplied.
2. Compensate forunevengroundbyusingscrewjackswithbaseplatesonly:Donotuse
unstableobjects
such
as
blocks,
loose
bricks
and
so
forth.
3. Wearsafetyglassesandhardhatswhenerectinganddismantlingsystemsscaffold.
4. Besure,allscaffoldingbeingerectedplumbandlevelatalltimes.
5. Donotforcememberstofit,plumbandlevelscaffolduntilproperfitcanbemadewith
ease.
6. Whenassemblingposttogetherbesurethatalldoublesnapbuttonsarefullyengaged.
7. Install horizontalmembers on both rings of each ring set from below and firmly set
wedges immediatelybeforeplacingnextmember.Donotstand, lean,orputweighton
horizontalmembersuntilthewedgesarefullyset.
8. Donotclimbverticalposts,horizontalmembers,orbracing.Accessmustbeprovidedto
allworkplatforms.Accessladdersmustextendatleast3feetaboveplatform.
9. Horizontal and/or vertical diagonal bracing may be required to prevent rack of the
scaffoldstructure.
10. Do not use of truss bearers without thorough consideration for the loads to be
supported.Donotcantilevertrussbearersorotherhorizontalmembers.
11. Guardrailsmustbeusedonallopensidesandendsofscaffoldplatform.Bothtopand
midrailsarerequired.Guardrailsshallbeprovidedwhenminimumheightisincreasing.
12. Toe boards are requiredwhenever people are required towork around the scaffold
platform.
13. Nottoextendplatformheightwithladders,boxesetc.
14. Allworkplatformsshallbefullyplanked.

15. Not to use scaffolding formaterial hoist towers or formounting deckswithout first
evaluatingtheloadsandstressesinvolvedandadequatelyprovidingforthem.
16. Workplatformsmustbefullyplankedeitherwithscaffoldgradedorlaminatedplank,in
goodsoundconditionorwithfabricatedplatformsingoodcondition.
a) Eachplankmustoverlapitssupportbyaminimumof6inchesorbecleated,i.e.8
footplankson7footspansmustbecleated.
b) Planknotextendedbeyond itssupportbymorethan18 inches.Suchoverhangs
shallbeseparatedfromtheworkplatformbyguardrailingsothattheycannotbe
walkedon.
On
irregular
scaffolding
configurations
it
may
be
necessary
to
have
a
plankoverhang itssupportbymorethan18 inches.Inthesecasestheoverhang
nottoexceed1/3thelengthoftheplank.Overhangmustbeoutsidetheguarded
work platform andmust never be loadedwithmaterials nor shall any person
standorwalkontheseoverhangs.
c) Plankoncontinuousrunsmustextendoverthesupportsandoverlapeachother
byatleast12inches.
d) Spansof2 inchx10inchscaffoldgradeplankshallneverexceed10feet.Loads
onplank
shall
be
evenly
distributed
and
not
exceed
the
allowable
loads
for
type
ofplankbeingused.Nomorethanonepersonshallstandonanindividualplank
atonetime.

18. CRANEANDRIGGINGEQUIPMENTOPMSANGOPERATOR/EQUIPMENT
CERTIFICATION
TDEwillcomplywithallcodesforusingmobilecraneonOPMSANGsFacilities.
18.1.GENERALRULEFOROPERATORSANDRIGGERS
a. TheoperatormustbetheholderofacurrentSAGoperators licenseandbe
OPMSANGCertifiedforthecraneheoperates.
b. Certificateshallalwaysbeinhispossession.
c. TheRigger
must
be
the
holder
of
OPM
SANGs
Rigger
Certificate
to
at
least
a
LevelIII.
d. Equipment Supervisormustensure thathisoperatorsarephysically fitand
mentally alert. If operators show any sign of illness,hemustbe removed
from thecrane.
e. Theriggerisresponsibleforproperattachingtheloadtothecraneandgiving
thecorrectsignalstothecraneoperator.
f. Therigger
is
the
only
person
authorized
to
give
signals
to
the
crane
operator.
g. The Crane Operators shall inspect cranes daily using the Preuse Daily
InspectionCheckListsandmaintainacopyintheCraneCab.
18.2.GENERALRULEFORCRANE
17.2.1. NocranemaybeoperatedwithoutOPMSANGsSticker. Thesticker isvalid
for threemonthsandwillbe issuedafterthecranehasbeen Inspectedand
foundserviceablebytheOPMSANGscraneinspector.
17.2.2. Allcranesusedforthesiteshallbeequippedwithfireextinguisher,seatbelts,
backup alarmand spark arrestor.Allmobile cranesworking forOPMSANG
shallbeequippedwithloadmomentindicatorsandshallbeequippedwithan
antitwoblockdeviceonatwoblockdamagepreventionfeatureforallpoints
oftwoblockingi.e.Jibs,extensions,etc.
17.2.3. Cranesshallnotenterarestrictedarea,oroperatewithinoneandaquarter
boom lengths of an oil line, gas line,oroverheadpower line, until awork
permitforthatparticularcranehasbeenissued.
17.2.4. Work Permitwill notbe issued if the operatordoesnothavewithhimhis
OPMSANGsCertificate.

17.2.5. A distanceof at least20 feet shallbemaintainedbetweenanypartof the
craneandanyoverheadpowerlineupto250KVand25Feetforover250KV.
17.2.6. Craneandliftinggearmustbeingoodmechanicalconditionandassembledin
accordancewiththemanufacturer'sspecifications.
17.2.7. Thedrivingandoperatingcabsshallbekeptfreeofrubbishandunnecessary
materials. Allbrokenglassshallbereplaced.
17.2.8. Noliftingdevices,suchasAFrames,Chainhoists,Spreaderbars,etc.,shallbe
useduntiltheyhavebeeninspectedandstickeredbytheOPMSANGsCrane
InspectionGroup(CIG).
17.2.9. Man baskets shall be inspected and stickered by the OPMSANGs Crane
InspectionGroup(CIG),beforebeingused.
17.2.10. All slings shallbe inspected at leastmonthly andmarkedwith thatmonths
safety color tape. All slings shall be documented on a sling log book as
specified by theOPMSANG. All slings,whennot in use, shall be properly
storedtopreventdamaged.
18.3.OPM
SANGS
OPERATOR
/EQUIPMENT
CERTIFICATION
Only qualified personnel in possession of the required SAG License andOPMSANGs
certificationshallbepermittedtooperateequipment.Craneoperatorsshallbequalified
inawrittenandapractical test inDhahran.Personnelmustbeable toreadandwrite
EnglishorArabictotakethewrittentest(Reminder:TheOPMSANGcranecertification
states the types of cranes that the crane operator is certified to operate. The crane
operator shall not operate any cranes other than those stated on the certification.
OperatorsshallnotliftmanbasketsunlessOPMSANGcertified.)
CranesmusthaveacurrentOPMSANGscraneinspectionsticker.Inorderforthecrane
tobe inspected,acertifiedoperatororcertifiedcranemechanicforthatcraneshallbe
available.
Shallacranefailexaminationduetomajorfaults,andthosefaultscannotberectifiedat
thejobsite,itshallberemoved.Itshallnotbeallowedtoremainonsite.
Failure to complywith these requirements could result inworkbeing stopped in that
part of the operation until those deficiencies have been corrected, as stated in the
contract.

18.4.OPMSANG IDs/VEHICLES
AllTDEemployeesshallhaveaOPMSANGIDbeforebeginningwork,intheproject.
TDEmanagement ensure that no one shall use another persons ID and shall notify
employeesrequiringaccesstoSecurityComputerGatesthatanyattempttogainaccess
bytwo(2)ormorepeopleusingthesame IDattheturnstilegatewillfailasawarning
signalsounds immediately inthegatehouse.The IDshallbeconfiscatedandadditional
penaltiesmaybeimposed. AlostIDshallbereportedimmediately.
TDE employees shall be trained to be courteous and civil to personnel at Plant and
Facilitysecurity
gates.
All TDE vehicles requiring access to a Restricted Area Plant or Facility shall have a
RestrictedAreaAccessStickervalidforamaximumofsix(6)months.

19. MECHANICALEQUIPMENT
19.1.SAFEPRACTICES/PROCEDURES
The following outlines precautionarymeasures need to be observedwhen operating
mechanicalequipmentinordertopreventcostlyaccident:
18.1.1. TheuseofequipmentshallbeinaccordancewithEM 385-1-1ofOPMSANG
SafetyRegulations.
18.1.2. Operatorsofmotorizedequipment shallalwayscarrywith them theirSaudi
ArabianLicense
when
operating
motorized
equipment.
18.1.3. Only authorized, experienced and trained operators shall be allowed to
operate/drivemechanicalequipment.
18.1.4. Provide guard to all exposedmoving parts ofmachinery, i.e., gears, belts,
pulleys,fans,hotpartsandrevolvingparts.
18.1.5. Allmechanical equipment shall be inspected regularly before using and at
regular Intervals thereafter. Servicing of preventivemaintenance schedule
shallbe
strictly
observed.
18.1.6. Norepairs,adjustment,orreplacementofpartsshallbepermittedonmoving
machinery. Alwaysstopmachinewhendoingrepairworkstoavoidaccidental
operation.
18.1.7. Daily,beforethestartof thework,operatorshallcheckthefollowing items:
oil, water, hydraulic levels, signals and indicators system, guards, limits
switches andother safety equipment to ensure smoothoperations. Check
stickers,passes/inspectiontagsandfirefightingequipment.
18.1.8. Mechanicalequipment shallnotbe leftunattendedwhen running. Parking
brakesshallbeapplied,wheelschockedandenginestopped. Hydraulicparts
suchasblades,scrapers,etc.,mustbeloweredtotheground.
18.1.9. Maintaincleanlinessofallmechanicalequipment
18.1.10. While traveling or working near the highway, all signal lights shall be
operated. Reflectors,barricades,signs,etc.shallbeconspicuouslyinstalled.
18.1.11. Beforemovingequipment fromanyposition,checkaround theunit forany
obstacles,hazardsetc.

Designated Drivers who posses valid Saudi Government Drivers License and

19.2. FITNESSTEST:

TDEstorageyardsshallbe laidout inaccordancewithOPMSANGsStandardswithrespectto
spacing of rows, concentrationwithin rows, and concentration ofmaterials. Stockpiling of
combustiblematerialsshallbeinterspersedwithnoncombustiblematerials. Storageareasand
aislesshallbedelineatedbymarkers.
TDE shall submit to OPMSANGs Representative for review prior to implementation plan,
layouts and storage areas for storingmaterials and equipment and additionally, site offices.
Thesestorageareasshallbedesignedandlaidoutasfollows:
Adequate access road to the lay down yard and shall be kept clear and unobstructed at all
times.
Adequatelightingtoilluminatethestorageyards.
A systemof securitymenofeight (8)hours shift shallbeplanned tomaintainsecurity in the
absenceofworkersinthesite.
TDE storage yardswillbe laidout inaccordancewithOPMSANGsStandardsandwillbe
indicatedinTDETemporaryLaydownYardDwg.
The perimeter fence of sound construction and design appropriate for the intended
protectionof
the
area
will
be
provided
for
materials
storage
yards.
ThefenceshallbefrequentlyinspectedbyTDESafetySupervisorandSecurityGuard.
Twoaccessgateswillbeprovidedforthestorageareadirectlyoppositetoeachother.
Materials shallbe arrangedwithOPMSANGs Standardswith respect to spacingof rows,
concentrationwithinrows,andconcentrationofmaterials.
Stockpilingofcombustiblematerialsshallbeinterspersedwithnoncombustiblematerials.
Storageareasandaislesshallbedelineatedbymarkers.
