SCENARIO 11. IOCL LPG bottling plant is storing liquid LPG in three horizontal bullets and two

Horton spheres under atmosphere temperature. At about 4.15 hrs on 20 December 2008, a leak of LPG was noticed due to failure of one of the flanges (an engineering fault) of Horton Sphere by the operational staff on duty.

2. Due to leaking of the LPG outside the boundary of IOC LPG plant, the gas came out of the plant boundary and caught fire due to spark from one of the truck passing outside the plant boundary and after a while this fire led to BLEVE in the one of the Horton spheres:

(a) Inside Plant –3 killed and 12 injured.

(b) Outside Plant– 5 killed and 37 injured.

SITUATION 3

BPCL terminal stores, handles and processes a number of hazardous inflammable petroleum products at atmosphere temperature and pressure.

At about 6.00 hrs on 20 December 2008, two persons were challenged by the security staff in the piping and pumps area in the BPCL terminal. They were wearing the BPCL staff dress. One of them removed his coat and suddenly there was a loud blast, killing the suicide bomber to pieces. While people rushed towards the incident site, the second person fired from his AK 47 rifle, killing 3 security personnel & injuring 8 staff members of BPCL. The blast affected the pipes rupturing a number of them causing leakage . The firing by the second terrorist caused pool fire which spread to the MS and Diesel tanks.
