How to Prevent Duplicate Medication Dispenses A recent ISMP Medication Safety Alert newsletter item noted a duplicate medication error that occurred after a patient transferred to another unit.1

Before transferring the patient, a nurse removed all the patient’s morning medications from the automated dispensing cabinet (ADC) and administered them to the patient. Because it was still early in the morning, the nurse on the receiving unit gave the patient the same medications, without checking the patient’s medication administration record (MAR).

To prevent these kinds of errors Omnicell® ADCs can be activated with a Last Issue Alert,2 which is recommended by ISMP.

Last Issue Alert This hospital-wide alert warns nurses when removing a medication previously issued for a patient within a set time period, regardless of the cabinet being used. The last issue date and time will be displayed in red, drawing the nurse’s eye to this critical information.

If the nurse attempts to remove the medication, an alert message will be displayed (as shown below), requiring the nurse to actively decide whether to proceed with removal or cancel to further investigate.

Other Safety Mechanisms Additional Omnicell safety features that help prevent duplicate dispense errors are described below.

Scheduled Meds The Scheduled Meds tab displays the list of medications orders that are within the administration due time, including the early/late window. Once an ordered medication is issued from the cabinet within this time period, it is no longer displayed on the Scheduled Meds list.

By following the practice of dispensing scheduled medications from the Scheduled Meds tab, nurses are prevented from inadvertently issuing a scheduled dose more than once.

Last Issue Status To reduce the potential for error, the following is displayed during the medication selection process:

• Last date and time the medication was issued

• The cabinet it was issued from

• The user who issued it

This hospital-wide information reflects all issues for that medication order or medication within the facility, giving the nurse up-to-date status.

Reducing Risk Greater opportunity for error occurs when care is transferred between nurses, when a patient transfers to a different level of care, or during nurse shift changes. The risk can increase when the patient profile is not available.

Using the features described here, including activating the Last Issue Alert, helps safeguard against duplicate medication dispense errors. Accessing the patient’s MAR adds another layer of safety.

Medication Adherence – The Future Is Upon Us Dennis Newsom, RPh, has been a pharmacist and entrepreneur for 44 years. During his career, he has founded or acquired three healthcare companies. The Pharmacy Counter, his latest acquisition, was sold in 2010 to Ohio-based ProMedica, a healthcare provider that includes 11 hospitals.

Dennis has always been a strong advocate for the profession of pharmacy—not the counting and pouring part, but the part spent offering face-to-face counseling to patients and assuring proper use of medication. He believes pharmacists are an integral part of the healthcare equation.

“Medication adherence is part of our future,” said Dennis,who cites the following statistics:

• About 50% of prescriptions filled each year are not taken correctly.3

• 10% of all general hospital admissions and 23% of nursing home admissions are due to medication non-adherence.4

• 25,000 avoidable deaths are caused each year by non-adherence.5

The Pharmacy Counter began to address non-adherence eight years ago with a medication therapy management program—an intensive counseling, motivational, and clinical goal setting relationship between selected patients and pharmacists. The program was successful and won national awards.

But even with these interventions, The Pharmacy Counter could not solve the patient dilemma of forgetting to take medications or doubling doses—especially when multiple medications are prescribed. In addition to counseling, some sort of tool was necessary to ensure patients were medication-adherent.

The Pharmacy Counter looked at various options, but everything had its limitations. They envisioned a simple seven-day, multi-dose medication pack with breakfast, lunch, dinner, and bedtime blisters—a visual display where people could see whether or not they had taken their medication for a specific time slot. The capability to produce this type of packaging on a large scale did not seem to exist.

Then about four years ago at a national pharmacy conference, Dennis and his team came across an OnDemand® 400 machine for RxMap® from MTS Medication Technologies, an Omnicell company. The equipment allows pharmacies to quickly and accurately organize medications into RxMap® multiple medication adherence packaging—blister cards arranged by time-of-day and day-of-the-week—exactly what The Pharmacy Counter wanted.

The Pharmacy Counter worked with MTS to integrate RxMap with their ongoing adherence counseling. This program has helped to keep patients out of the emergency department, out of the hospital, and out of the nursing home. “Patients are at home where they want to be and are able to manage their medications themselves,” said Dennis.

As the future unfolds, Dennis believes that pharmacies specializing in medication adherence will be an integral part of local “integrated care management teams” tasked with keeping people healthy and independent. He believes this model can assist in making healthcare costs affordable and sustainable.

Happy Nurses, Happy Patients With healthcare shifting to a pay for performance model, nearly $1 billion in payments to hospitals over the next year will be tied in part to patient satisfaction.6 CMS will base 70% of payments on how well clinical guidelines for care are followed and 30% on patient experience, via patient surveys.7

The functions nurses perform are intrinsically tied to many of the survey factors. But how does a nurse’s own job satisfaction correlate with patient satisfaction? Several studies have looked at this question, and not surprisingly, proven that the correlation exists. A few of the studies are highlighted below:

In examining the relationship between nursing and patient satisfaction across 430 hospitals, the authors found that nurse perception of their work environment was associated with all 10 measures of patient satisfaction based on HCAHPS surveys, and was strongly related to whether patients would definitely recommend the hospital.

This study, involving four ICUs in a 750-bed hospital, also found a relationship between nurses’ perception of their work environment and patient satisfaction. In particular, the study showed higher patient satisfaction when nurses have a favorable perception of the nurse manager.

This study found that the percentage of patients who would definitely recommend the hospital decreased by about 2% for every 10% of nurses reporting dissatisfaction with their job. The authors concluded that “improving nurses’ working conditions may improve both nurses’ and patients’ satisfaction as well as the quality of care.”

More than good feelings are at stake. Research from the University of Pennsylvania School of Nursing found that Medicare patients treated in hospitals with good working environments for nurses had up to 10% lower readmission than hospitals with poor environments.8

Are You Aware of HIPAA Risks Due to XP End of Support? On April 8, 2014, Microsoft ends support for Windows XP. HIPAA Security Rule section 164.308(a)(5)(ii)(B) mandates that systems must be guarded against malicious software. After April 8, 2014, Microsoft will not release any Windows XP security patches or updates, which would prevent malicious software infections and possible data breaches.
