We are increasing the company match for the 2019 Health Savings Account (HSA) and are bringing on new tools to help you save money and increase the quality of care you receive.

We recognize that choosing the best level of benefit coverage for you and your family can be difficult, so this year we are introducing Custom Benefit Consultants (CBC), a new administration company who will be able to assist you with your decisions and enrollment, replacing Accolade. Beginning in January 2019, you will have access to CBC’s assistance year round to answer questions about claims, benefits, help with finding providers or services, and so much more.

Our partnership with CBC brings with it a new program called HealthJoy. HealthJoy provides a cellphone app that stores all your benefits cards, plan details, HSA balance, and even the link to our telemedicine services in one place, along with many additional services.

During our first year with AdvanceMedical, our Team Members found this resource to be invaluable in the diagnosis and treatment of their medical conditions. Based on the positive feedback, we are expanding coverage under AdvanceMedical to include extended family (adult children, parents, grandparents, and siblings). This resource provides expert second opinion support for medical conditions by evaluating diagnosis and treatment options. Using services like this has been shown to reduce your costs and improve medical outcomes.

Lastly, based on your requests, we have added identity theft coverage to our benefit lineup, which will be offered through InfoArmor. Inside the guide you’ll find more information regarding this new option. First and foremost, we continue to offer plans that embody our flexibility and allow you to customize your benefits plan to best fit the needs of you and your family. This guide includes details about each of the plans. Nationwide, benefit costs continue to increase, but we’ve worked hard to modify our offerings to minimize your cost increases to the largest extent possible. You’ll see premium adjustments by less than 2% across all plan designs. One of the more noticeable ways we’ve accomplished this is a switch from the Aetna to the Anthem insurance network, which should offer you more choices for providers and a better overall network, and adding Costco Pharmacy as our primary, most discounted prescription cost option.

ENROLL IN YOUR BENEFITS IN THREE EASY STEPS!

1. Know your options REVIEW ALL PLAN OPTIONS AND COSTS. Finding the right coverage for you and your family is a balance between the amount deducted from your paycheck and the amount you pay at time of service.

Choose carefully: The benefits you select will generally begin the first of the month following completion of your eligibility waiting period and remain in effect through December 31, 2019. If you have elected a benefit that requires Evidence of Insurability, that benefit will be effective the first of the month following approval from the carrier. During the year, you can only change benefits, including adding or removing a dependent, if you have a Qualified Life Event such as birth, adoption, marriage, or divorce. These events are defined by the IRS or the plan. If you have a Qualified Life Event during the 2019 Benefits Open Enrollment period, you must report the event separately within 31 days of the event.

2. Decide who to coverYOU CAN ENROLL YOURSELF, AS WELL AS:yy Your spouse yy Your children under age 26, if they are your natural children, stepchildren, legally adopted children, foster children, or children for whom you have legal custody or court-appointed Guardianship. yy Your children of any age, if they are physically or mentally unable to care for themselves as long as they were disabled before age 26.

y� Your Login ID — Is the first initial of your first name and first initial of your last name followed by your date of birth (mmddyyyy) with no spaces in between. (For example: If “Linda Test” was born on September 17, 1971, her ID would be “LT09171971”.)

y� Your Password — If this is your first time logging into the Allegiant enrollment website, you will need to enter your temporary password. Your temporary password is the last six digits of your Social Security #. (For example: If Linda Test’s full SS number is 123-45-6789, the password is “456789”.)

When you Enroll:yy You’ll need to designate which dependents receive medical, dental or vision coverage by checking their names on the page that appears after you have selected your medical, dental and/or vision plan. yy Once you’ve made your choices, take a close look to be sure you’ve selected the benefits you want before you submit your elections. yy Print a copy of your elections as your confirmation statement.

Please check your Confirmation of Election Statement carefully before exiting the online benefits center. If you see a problem or want to make a change, simply go back to the “Enroll Now” benefits section and modify your election. When you have completed your elections, print your Confirmation of Election Statement.

Regardless of any statement made in this guide, benefits will be provided in accordance with the plan documents, contracts and trust agreements between Allegiant and the various provider organizations.

The Allegiant Benefits Plan: A Self-Funded Model

Medical insurance is complicated and risky – and its overall cost is often very difficult to predict. Employers can choose a more costly, but lower-risk design, called “fully-funded,” or a less expensive, higher-risk one, called “self-funded.” It is important you understand our model and how your choices can impact the plan costs, so we’ve outlined here a basic primer on insurance plan design.

In a “fully-funded” plan design, an insurance carrier, like Anthem, collects monthly premiums to pay for medical bills. Those premiums are partially paid by you from your paychecks and partially paid from Allegiant. That’s all the money Anthem collects to pay all the bills, so they need to charge enough to cover what they expect all the bills to be, plus a fee for doing all the bill payment work, plus a cushion for taking the risk to offset the years they don’t collect enough money to pay all the bills.

In those years, Anthem is stuck paying out more than they collected, so the following year, they raise the cost for “fully funded” insurance by 5, 10 or 15% as we’ve seen happen multiple times to Allegiant over the past decade.

In a “self-funded” plan design, the insurance carrier, like Anthem, is simply allowing employees to use its network of providers at the discounted rates it has negotiated, and processing the bills, but not paying them. There is a dramatically smaller monthly fee Anthem collects for these services, but it passes all the bills to the Company to pay. The Company puts the premiums we collect from employees plus the premiums Allegiant pays, just like in the fully-funded model, into a bank account used to pay for all the medical bills received. We save the amount of money Anthem would have taken as a fee and the cushion amount they would tack on to the premiums to ensure they collected enough to cover the bills, if the plan does not run as expected. If the bills are really, really high, though, this is where the riskiness comes in, because Allegiant has to pay them all, whereas if we were fully funded, Anthem would have to pay them.

Here’s what this means for each of us: Since we’ve opted for the self-funded model, each medical decision we make is charged directly to Allegiant. Choose Urgent Care instead of Emergency Room = plan savings. Choose your doctor to draw blood instead of asking to be sent to Quest labs = extra plan costs. Call telemedicine for your sore throat instead of going to your primary care doctor = plan savings. At the end of the year, next year’s paycheck premiums are based on this year’s actual spending – so every one of us, and every medical decision we make, can influence our future costs.

Hopefully this explains the type of plan we offer, and how all of us on Allegiant’s benefit plan have ownership in it. This is also why we offer so many resources like AdvanceMedical and HealthJoy to keep your costs and Allegiant’s costs as low as possible.

HDHP (High Deductible Health Plan)

Both the HDHP 3500 and HDHP 1350 plans are High Deductible Health Plans. Preventive care costs are covered 100%. Team Members can pay for qualified medical expenses with pre-tax dollars from their Health Savings Account (HSA). With an HSA, dollars contributed roll over from year to year. If you leave the Company, the account, along with all the funds in it, goes with you. Both Team Members and Allegiant can make contributions to an HSA. Allegiant contributes to your HSA by matching any contributions you make, dollar for dollar, up to the following amounts:

ANNUAL MAXIMUM MATCH* yy Team Member Only $700yy Team Member Plus $1,400 If annual salary <= $40,000, matches $2 to $1; If annual salary > $40,000, match is $1 to $1.

*The IRS annual limit maximum contribution for the 2019 plan year is $3,500 for those with Team Member Only coverage and $7,000 for Team Member plus Dependent(s). The annual limits include both the Team Member and employer contributions. Team Members who contrib-ute to an HSA, are 55 or older and who are not enrolled in Medicare are allowed to contribute an additional $1,000.

PPO (a Preferred Provider Organization) - FSA Eligible Plan

With the PPO plan you have access to both in-network and out-of-network health care providers. Your portion of the costs are less when you use in-network providers. There is no Health Savings Account (HSA) with this plan. With this plan you may enroll in a Flexible Spending Account (FSA). However, there is no company match and funds do not roll over to the next year.

All contributions need to be used within the year. Any ‘left-over’ funds are forfeited. This is often referred to as a ‘use-it-or-lose-it’ account.

HMO (a Health Maintenance Organization) - FSA Eligible Plan With the HMO plan you have access to certain health care providers within a limited network. Unlike other insurance plan types, care is covered only if you see a provider within the HMO network. You will pay a set co-pay when you seek health care.

There is no Health Savings Account (HSA) with this plan. With this plan you may enroll in a Flexible Spending Account (FSA). However, there is no company match and funds do not roll over to the next year. All contributions need to be used within the year. Costco Mail Order Pharmacy Preferred maintenance medications may be obtained through the Costco mail order pharmacy. If you are on a medical plan, you will obtain a 90 day supply for less than at a retail pharmacy.

Allegiant offers optional short-term disability. This plan provides a source of income for up to 11 weeks, should you become disabled due to a non-work related illness or injury. A 14-day waiting period applies. You are paid 60% of your weekly earnings, up to a maximum of $2,500 per week. Because you pay the premium after tax, the benefits you receive may not be taxable, and premiums are paid via payroll deductions. Rates are based on your age and salary. Team Members who elect this benefit during the initial offering will be guaranteed issuance without completing an Evidence of insurability (EOI). Future elections after your initial enrollment period may be subject to an EOI.

You can use the chart to the right and the information below to determine your short-term disability benefit and premium cost.

Divide your annual salary by 52 (weeks), then multiply it by 60%, this will be your weekly benefit amount. If this amount exceeds $2,500 your benefit amount will be capped at $2,500. You can change, add, or drop coverage outside of Open Enrollment if you experience a qualifying ‘life event’ WHAT IS A ‘LIFE EVENT’? Generally, a life event is a situation in which your family size or your availability for other insurance coverage changes. A change in your life can make you eligible for a Special Enrollment Period to enroll in health care coverage through Allegiant outside of the Open Enrollment period.
