Health Insurance for Students• Even Healthy Students may need healthcare services while at
College.• Student Health Centers provide many primary care services at
no charge, but they are not equipped to provide imaging, or diagnostic laboratory services.• Students needing x-ray, an ER visit, blood work, or a weekend
visit to an urgent care center, will not have access to these services without insurance or incurring a significant bill.• A single hospitalization can cost thousands of dollars.
Health Insurance for Students• Having Insurance means having access to the medical care
needed in a way that minimized disruption to your child’s studies, life and finances.• While Mercyhurst University currently requires insurance for
Athletes and International Students, it strongly encourages ALL students to carry adequate health insurance.• Mercyhurst students not currently required to show proof of
insurance, are still welcome to purchase the Highmark Community Blue PPO.
Rules for EnrollmentMercyhurst University requires all student Athletes and all International Undergraduates to have certain minimum health care requirements in order to attend classes.
If students are currently covered by either their parents’ plan or a spouse’s employer-sponsored group plan, they may waive Mercyhurst’s sponsored plan if it meets the requirements set forth by the University. Individual Policies & Travel Polices are not accepted.
Rules for Enrollment (con’t)Mercyhurst University strongly recommends all students obtain and maintain health insurance while they are earning their degree. The Highmark Community Blue PPO is available for all students, undergraduate and graduate alike, regardless of the number of course credits they are enrolled in.
Students who are not Athletes or International Students, do not need to complete the online waiver.
• To enroll in one of the University’s Student Health Plans, go to: www.hubbardbert.net
• Click on “Student Login” to access the Student Insurance Page, the link to enroll is at the top of the page
• You will need your student ID (7 digits beginning with “0”) and your date of birth
• Click “Continue” to proceed to the Enrollment or Waiver process
Online Enrollment Procedures
• Offers at least 80% coverage for in-patient and outpatient medical services
• Includes mental health benefits• Has a deductible not to exceed $500• Does not contain any language limiting coverage on pre-existing
conditions• Must cover injuries that occur during participation in
intercollegiate athletics• Has limits of at least $90,000 that includes coverage for sports-
related injuries
Waiver Requirements for Athletes & International Students
If coverage student currently has meets or exceeds these requirements, a waiver form must be
completed to opt out of the Mercyhurst University Student Health Plan

• Play pays – 80% after deductible• Out-of-pocket maximum - $3,000/$6,000
• Retail clinic visit – 100% after $40 copayment• Primary Care Provider office visit – 100% after
$25 copayment• Specialist office visit – 100% after $50 copayment• Urgent Care Center visit – 100% after $40
copayment

United Healthcare Plan Benefits Summary(this plan is only available for Non-Athlete, Non-US Citizens and
their dependants)
• Deductible - $100 in-network, $300 out-of-network per insured person
• Play pays – 80% after deductible in-network, 70% after deductible out-of network
• Out-of-pocket maximum – – $6,300 single/$12,700 family in-network; – $8,000 single/$16,000 family out-of-network
Preventative care services include:• Annual Physicals & routine OB/GYN exams• Immunizations
Are covered at 100% with no copay or deductible when performed by a Preferred
Provider.

Spring Semester– No refunds given if a student withdraws from classes– No plastic ID cards; students must register on
www.PGHStudent.com to print a copy of their ID card.
