However, the application forms do not vary a great deal and completing the visa application form is generally straightforward. Secti on Heading Guidance 1 Surname Enter your surname as recorded in your passport. 2 Surname at birth Enter your maiden name if different from the one used in Heading 1 3 Given name(s) Enter your given names as indicated in your passport (e.g. Paul John Smith, fill in Paul John) 4 Date of birth Format is DD/MM/YY 5 Place of birth Enter place of birth as indicated in your passport 6 Country of birth Enter country of birth as indicated in your passport 7 Current nationality Enter the nationality as indicated in your passport, as well as any other nationalities that you hold 8 Sex Tick appropriate box 9 Marital status Please tick the box in accordance with your legal status. If you are living in a marriage-like relationship with your partner without being legally married, tick "other" and enter "unmarried partner". If you are in a civil partnership or PACS with a French citizen, tick "other" and write “Civil partnership” or“PACS” 10 In the case of minors Enter details regarding the child's legal guardian 11 National insurance (NI) number Leave it blank unless you have resided in the UK and have been issued a NI number

Step by step guide to the Schengen application formEach Schengen country may produce its own variant of the standard Schengen visa application form.  However, the application forms do not vary a great deal and completing the visa application form is generally straightforward. 

Below is a section-by-section guide to filling out a standard Schengen visa application form.  An example of a correctly filled-out application form is also available in Appendix C.  

Section Heading Guidance1 Surname Enter your surname as recorded in your passport.

3 Given name(s) Enter your given names as indicated in your passport (e.g. Paul John Smith, fill in Paul John)

18 Residence in a country If you are residing in the UK, tick "Yes" and enter your UK visa number.  This is located in the UK visa in your passport, on the top right hand corner.  It is either a 9 digit number (e.g. 123456789), or you may have a residence permit stamp with a black 7-digit number preceded by UK (e.g. UK1234567).  Enter also your Expiry Date if applicable

19 Current occupation Enter your current occupation (e.g. doctor, student, engineer)

20 Employer and employer's address

For workers, enter the name, address and telephone number of your employer.  

For students, enter the name, address and telephone number of your education institution

21 Main purpose(s) of the journey

Tick as appropriate

22 Member State(s) of destination

Enter the Schengen countries that you will be travelling to (e.g. France and Italy)

23 Member State of first country

Enter the Schengen country where you will enter first

24 Number of entries Tick as appropriate.  Note if you are traveling to more than one Schengen country, you must tick "Multiple"

25 Duration of intended stay

State the number of days you wish the visa to be valid for.  This cannot be longer than 6 months, nor longer than your UK visa validity

26 Schengen visas during the past 3 yeas

Enter details of previous Schengen visa(s) granted (e.g. Germany 2009)

27 Fingerprints collected previously…

Tick as appropriate and enter details if your fingerprints were taken in order to grant you a Schengen visa

28 Entry permit for the final country of destination

Only complete if applying for a transit visa.

29 Intended date of travel Enter your intended date of arrival in the Schengen area

30 Intended date of departure

Enter you intended date of departure from the Schengen area

31 Surname and first name of the inviting person(s) in the Member State(s).  If not application, name of hotel(s) and fax

Enter the name, address and telephone number of your host or hotel that you have booked to stay with.  If you are staying at several hotels, enter details of the first hotel and then "various"

32 Name and address of inviting company/organisation/ telephone and fax of company/organisation

Complete only if applying for a business Schengen visa.  Enter details (name, address, telephone number) of the company that has invited you

33 Cost of travelling and living during the applicant’s stay is covered

Enter yourself, unless you are being sponsored.  Tick "Cash" and "Credit Card"

34 Personal data of the family member who is an EU, EEA or CH citizen.

Only complete if you are the spouse, dependent child or parent of an EU, EEA or CH national

35 Family relationship with an EU, EEA or CH citizen

Tick as appropriate

36 Place and date Enter the place and date according to the time and place where you complete this application even if different from where you will submit your visa

37 Signature Sign the application form.  For minors, the application form must be signed by his/her legal guardian
