STEATOHEPATITIS (NASH) Hepatic cell injury
+ Inflammation
STEATOSIS (NAFL)
NAFLD
Different clinical outcome
Alone or non-specific inflammation
Caractérisation histologique
Stéatose sans autres signes histologiques (parfois inflammation minime aspécifique)
Stéatose isolée
Stéatose
Stéatohépatite
Signes de souffrance hépatocytaire Ballonisation,
inflammation/nécrose
lobulaire,
Mallory, PNN
ballonisation
fibrose portale
fibrose
en pont
cirrhose
fibrose peri
sinusoidale
Usual diagnostic circumstances for NAFLD
Liver tests + Ultrasound
Liver tests + Metabolic Risk Factors
Increased ferritin
Cirrhosis X
Metabolic Risk Factors + Normal ALT
NASH
Key points : presence of steatosis (ultrasound, markers) presence of metabolic risk factors
Prevalence of NAFLD/NASH/Fibrosis in asymptomatic outpatients
Williams, Gastroenterology 2011
US Army personnel N=328
NAFLD 46%
NASH 13%
Advanced fibrosis 3%
(23% of NASH)
Personnes d’âge moyen
Définition échographique
Définition histologique
Population Population générale
Chirurgie bariatrique
Diabète 2 Dyslipémie
NAFLD 46% 90% 69% 50%
NASH 13%
Cirrhose 3% 5%
• Amiodarone
• Méthotrexate
• Tamoxifène
• Corticoïdes
• …
NAFLD and Alcohol can coexist
and HCV
and HBV
and Hemochromatosis (ferritin; transferrin saturation) And autoimmune hepatitis: NB 21% of NAFLD have ANA or ASMA
NAFLD SHOULD NO LONGER BE A DIAGNOSIS OF EXCLUSION
Consommation significative d’alcool= > 21 verres /sem chez homme > 14 verres /sem chez femme

NAFLD
Secondary
PRIMARY Metabolic Risk Factors
• Alcohol • Drugs (amiodarone, metothrexate, tamoxifen, corticosteroids) • A/hypo betalipoprotéinémia • Chronic HCV (genotype 3) • Maladie de Wilson •Parenteral nutrition • Severe malnutrition • Toxiques industriels • Lipodystrophies • Cholesteryl ester storage disease • Microvesicular steatosis (Reye’s syndrome, medications (valproate, antiretroviral medicines), acute fatty liver of pregnancy, HELPP syndrome)
