INSTRUCTIONS TO THE CANDIDATES 1. a) Candidates who have been allotted through centralized counseling by the
MCC of DGHS, New Delhi for admission to M.D./M.S Degree courses in Sri
Lakshmi Narayana Institute of Medical Sciences for the academic year 2019-20,
are advised to report and complete the admission process within the date and
time specified in the “Allotment Letter‟ issued by the DGHS.
Venue for reporting by the candidates Admission Office (Hospital Campus)
Osudu, Agaram Village, Villianur Commune, Kudapakkam Post,
Puducherry – 605 502.
b) The candidates should report along with original certificates and Demand Draft
for tuition fees (refer www.bharathuniv.ac.in and www.slims.ac.in ) and other fees or
UTR No. for Electronic Fund Transfer through RTGS as specified herein below.
2. The candidates should bring the following certificates/documents in Original
(along with 3 sets of photocopies of the same):- 1. Allotment letter issued by the MCC of DGHS, New Delhi. 2. Admit Card issued by NBE. 3. Result/Rank Letter issued by NBE. 4. Aadhaar Card 5. MBBS Degree Certificate/Provisional Certificate. 6. MBBS Internship Certificate.
7. Permanent Medical Council Registration Certificate issued by MCI or any
State Medical Council. 8. MBBS Mark Statements (First Year to Final Year). 9. MBBS Transfer Certificate. 10. Migration Certificate.
11. SSLC/10th Mark Sheet.
12. HSC/ 12th Mark sheet. 13. Community Certificate. 14. Conduct Certificate issued by the Head of Institution Last Studied.
15. Twelve Passport size & Two Stamp size colour photographs (same as in
NEET admit card) and one Passport size photograph of the candidate’s
Father and Mother and also of the local guardian, if applicable.

17. Any one of the identification proofs (ID Proof as mentioned in the
information Bulletin published by the National Board of
Examinations. i.e., (Voter ID/ Passport).
3. i. Candidates without original certificates/documents shall not be allowed
to take admission in Sri Lakshmi Narayana Institute of Medical Sciences.
ii. Candidates who have deposited their original documents with any other
institute/college/University and come for admission with a Bonafide
certificate stating that “their original certificates are deposited with
institute/college/University” shall not be allowed to take admission in
the allotted Medical College.
4. Allotment letter given by DGHS for admission, is subject to fulfillment of all
eligibility criteria prescribed by the MCI and medical fitness of the candidates.
Those who do not fulfill the prescribed eligibility criteria and not found medically
fit will not be admitted to the course of study to which they are allotted by the
DGHS. 5. Other Fees Details:

Please indicate total number of PG seats (Recognized+permitted) available in 2nd column. Out of total number of seatsavailable, 50% All lndia Quota to be indicated in 3rd column and remaining 50% State Quota seats to be indicated in 4thcolumn.
