Sports Physicals & Baseline

Concussion Testing

CORA Physical Therapy and Southeast Orthopedics will be holding pre-

participation physicals and baseline concussion testing for the St. Johns

Middle School Athletic Association on April 29, 2017.

Location: Body Control Gym, 120 Everest Lane, St. Johns, FL 32259

Time: Check-in starts at 1:00 pm and closes at 2:30 pm (please note physical

and baseline testing are approximately 1 hour)

Cost: $20 per student athlete

All proceeds will go directly back to St. Johns Middle School Athletic Association for

this season’s athletics.
