LectureTitle:Whycracks?WhatyouneedtoknowaboutitThemaingoalsofendodontictreatmentaretoremovebacteriafromtherootcanalsystemtopromotehealingandrepairoftheperiapicaltissuesandtopreventmostbacteriafromre-enteringthecanals.Therearemanywaysforbacteriatopenetrateintotherootcanalsystem–suchascaries,fractures,cracksandbreakdownofrestorations.Inordertohaveasuccessfulendodontictreatmentoutcomes,itisimportanttoidentifythecausesofthepulpandperiapicaldiseases–thatis,thepathwaysofbacterialpenetrationintotherootcanalsystem.Thisisnecessarysothatthecausescanberemovedaspartofthetreatment.Cracksisapathwayforbacterialpenetrationintotherootcanalsystem.Cracksinteethareoneofthemajorcausesofpulpandperiapicaldiseases.Cracksmaybedetectedduringtheclinicalexaminationunderthenormaldentaloperatinglightbylightdeflection,changeintoothcolour,shadow,stainingorwhenthecrackislargeenoughtobefeltusingasharpdentalprobe.Anothercommonlyusedinstrumentisaselectiveocclusalforceapplicator.However,itisoftendifficulttofindcrackswhentheyaresmallandthereisnoassociatedpulpresponse.Therearenotypicalsymptomswithcrackedteeth.Theteethwithcrackshavevarioussymptomsdependingonthestatusofthepulp;fromclinicallynormal,reversibleorirreversiblepulpitistonecroticorpulplessrootcanalsystem.Therefore,thetreatmentsforcrackedteethalsoshouldhavevariousapproachesdependingonthestatusofthepulp.Thislecturewilldiscusstheadvantagesanddisadvantagesofdifferentmethodsofdetectingcracksduringdentalexaminationaswellasmanagementstrategiesofcrackedteeth.
