PURPOSE Tampa Electric Company - Solar Operations (Solar) - Hazardous Energy Control (HEC) Program has been established, in accordance with OSHA Standards to prevent the unexpected release of potentially hazardous energy (e.g. mechanical , electrical, hydraulic, thermal, chemical, pneumatic, potential, etc.) during the maintenance and servicing of equipment. This HEC Program consists of a comprehensive set of equipment specific HEC Procedures, personnel training requirements, and guidelines for the periodic inspection of the HEC procedures and program. This procedure defines the minimum requirements for the control of hazardous energy

to prevent personal injury and equipment damage by ensuring all known sources of

energy are isolated and any potential energy released or contained in a safe manner

during work activities.

INTRODUCTION Tampa Electric Company (TEC) is dedicated to providing a safe and healthy workplace for its employees by communicating information concerning HEC. This program applies to Tampa Electric Company Solar employees and contractors to prevent injuries, loss of life and/or property damage from exposure to hazardous energy. The Solar HEC Program applies to the servicing and maintenance of equipment at all TEC facilities under the jurisdiction of Solar. The HEC Supervisor has lockout authority and control over the equipment in all Solar plants (except the medium voltage equipment in the substation). The division of responsibility between Solar and the Energy Delivery Department will be the substation Medium Voltage Breaker unless otherwise indicated in specific HEC procedures or switching orders.

SCOPE

This procedure applies to all TEC, contract, and subcontract employees performing

work on Solar Generating Sites.

This procedure provides the following information:

• Identifying energy isolations and developing “energy isolation/switching”

and Lock Out Tag Out (LOTO) Permit forms.

• Executing a LOTO Permit (energy isolation/release and locking and

tagging energy isolations)

• Changing LOTO

• Shift Change

• LOTO Committing

RESPONSIBILITY 1.0 General It is the responsibility of Solar Management to approve, implement, monitor, and enforce the Solar HEC Program. Joint responsibility for continuous improvement of the Program is shared between workers and management through a partnership dedicated to protection of workers and compliance with regulations. It is the responsibility of the Department Head or designee to select competent and qualified employees to act as HEC Supervisors. The HEC Supervisor is the person under whose orders the HEC Procedures are performed. The Department Head is responsible for the review, revision, implementation and maintenance of the Solar HEC Program. Duties supporting this objective may be assigned to the Coordinator of Safety & Industrial Health or others as designated. Solar management is responsible for coordinating work of outside contractors and will work jointly with the HEC Supervisor in the implementation of the Solar HEC Program for outside contractors. 1.1 Department Head

Acts as the Control Authority at each site under their control.

1.2 Site Supervisors

Supports Control Authority in managing the LOTO Program at the site, and often,

serves the role of HEC Supervisor.

1.3 Safety Department

Conducts periodic LOTO observations, audits LOTO activities and monitors adherence

of this policy. They will also act as a resource for compliance with applicable regulations

and Industry Standards.

1.4 Control Authority

Ensures O&M personnel are properly trained and qualified to perform the activities

stated in this procedure. Monitors that site personnel maintain these standards at all

times and are in in full compliance. Authorizes personnel to perform duties in

accordance with this LOTO program. Monitors and enforces all site policies, processes,

procedures, and guidelines. Authorized individual that may provide consent to remove

the locks and tags of another person as per the “committing” process. Responsible to

maintain the Authorized Employee List by validating personnel LOTO training is current.

1.5 HEC Supervisor

Administers the LOTO program for authorized employees. Issues LOTO devices,

assigns tasks to isolate equipment, and verifies the equipment isolation of authorized

employees. Verifies that the specific system condition requirements are reviewed during

the LOTO pre-job briefing and the plan is adequate for personnel and equipment

protection. Ensures specific LOTO conditions can be accomplished before the LOTO is

in place. Maintains content and records of the LOTO activity for the site. All released

LOTO Permits are retained for one year. Authorization is continuous across all shifts

and this procedure allows the transfer of an active LOTO to another HEC Supervisor.

1.6 HEC Operator

Isolates equipment in preparation for establishing a LOTO and applies the locking

devices. In many cases, the HEC Operator and HEC Supervisor may be the same

person.

1.7 Job Lead

Communicates with the HEC Supervisor to ensure the scope of work and scope of

hazardous energy control match. The Job Lead provides the work order and task

numbers or scope of work description for recording on the HEC Procedure by the HEC

Supervisor. Provides guidance to personnel requiring protection from hazardous

energy while performing work on equipment. Applies a personal lock to the primary lock

box and places the key to this lock in a group (or crew) lock box to which authorized

employees shall affix their personal lock. Applies a personal lock to the group (or crew)

lock box and maintains possession of the key.

1.8 Confirmation Employee

Determines adequate isolation for each energy source type. Confirmation employee

can be a Competent Contractor Employee. Visually inspects (confirms) all energy

isolation devices are listed on the written HEC procedure. Certifies the confirmation of

the HEC procedure isolation by applying an isolation confirmation lock to the primary

lock box. The key to this lock is maintained by the HEC Supervisor and is transferred to

the oncoming HEC Supervisor, each shift. The Confirmation Employee cannot be the

HEC Operator or HEC Supervisor.

1.9 Authorized Employee

Validates the established LOTO provides enough protection for the job scope prior to

signing onto a LOTO and starting maintenance or servicing. Participates in post task

review to report safety process information to site management for process

improvement. The Authorized Employee cannot be the HEC Operator or HEC

Supervisor.

1.10 Affected Employee

Responsible for being aware of this program. Recognizes a lock and locking device

and understand that the locking devices may not be defeated in any way. Understands

that they are not afforded protection from hazardous energy and they are responsible

for following Safe Work Practices. The Affected Employee cannot be the HEC Operator

or HEC Supervisor.

GENERAL REQUIREMENTS

2.1 Understanding and Compliance

Tampa Electric Company’s Solar sites are designed and constructed to ensure

employees avoid incidents which may result in death, injury, and equipment damage.

Within the safety rules, the LOTO Procedure is the cornerstone of the Safe Work

Process due to the potential seriousness of an incident if this procedure is not followed.

It is vital that each employee learns, understands, and complies with this procedure.

The use of good judgement, along with this procedure, makes the sites a safer place for

employees to work. Strict adherence is required. Failure to comply with this procedure

may result in disciplinary action, up to and including termination of employment or

contract.

2.2 Energy Isolating Devices

Locked and tagged energy isolating devices cannot be manipulated, dismantled, or

removed and cannot be used for any other purpose, other than the requirements listed

in this procedure.

2.3 Identification of Isolation Points

All LOTO isolation points must be labeled.

2.4 Locks and Keys

Authorized Employees signing onto a LOTO Permit Form, must have their own personal

lock(s) and key(s). The following color locks are required:

• Orange Locks – Equipment isolation / personal locks

Equipment locks may be keyed alike while personal locks shall only have one key. Keys

for the installed padlocks always remain with the employee during lockout.

Only the Authorized Employee may remove the lock(s), unless that individual is

unavailable, (or their key is lost) then only the Control Authority and HEC Supervisor

can authorize the removal by utilizing the LOTO HEC Committee Form.

When a piece of equipment is locked/tagged out, the Authorized Employee must log the

activity on the LOTO Permit Form.

Each employee must keep, in his/her control, the key to the personal lock he/she

always placed on the designated Lockbox .

The Job Lead will not remove his/her lock until all employees have removed their

personal lock from the group/crew Lockbox and signed off the LOTO Permit Form.

2.5 Tags

Personal tags must be attached to all personal LOTO locks. Tags will have name and

phone number on the tag.

Equipment isolation locks must have stickers or tags that show the lock number and the

HEC Operators name & phone number.

Energy isolating devices, which are isolated only with a tag out device, cannot be operated under any circumstances.

When possible, locks shall be applied to all isolation points with tags. If unable to use a lock, additional hardware (lockout devices) will be utilized to eliminate the likelihood of inadvertent energizations such as clamshells, chains, and/or switch covers. Lockout Devices will be utilized along with a tag should be considered such as:

• Removing an isolating circuit element

• Blocking a controlling switch

• Opening an extra disconnecting device

• Double blocking and bleeding

• Removing a valve handle to reduce inadvertent energization

When tag out devices are used in lieu of locked out devices, the following must be met:

• Must be durable enough to withstand wet, damp, or corrosive environments

• Legibility must be retained

• Attachments (i.e. zip ties) are required to have an unlocking strength of at least 50 pounds.

Tag out devices are placed on the operating handle of the energy-isolating device with an approved zip-tie. If installing a tag to an operating handle is not possible, affix the tag as close and safe as possible to the device. The position should be immediately obvious to anyone attempting to operate the device.

INITIATING LOTO

3.1 Energy Control Preparation

The HEC Supervisor reviews the applicable documentation to achieve energy isolation and adds to the LOTO Permit Form. The following are examples of this documentation:

• Work Order/Job Scope

• Schematics (electrical/mechanical/hydraulic)

• Procedures (MOP/NOP)

• Energy Isolation Points

• Stored Energy

• Grounding

A Job Safety Analysis (JSA) and Pre-Job Briefing (PJB) must be conducted with all parties involved to identify hazards and control measures prior to initiating a LOTO. During the JSA/PJB, the HEC Supervisor communicates the methods that will be used to verify all energy sources have been released, removed, or proven dead.

The HEC Supervisor notifies all Affected Employees and ensures that they are clear of the energy control activities before lockout or tagout is initiated.

3.2 Equipment Shutdown

Equipment or systems that are running must be shutdown. The HEC Supervisor authorizes the shutdown of equipment by using the site operating procedures (MOP/NOP).

3.3 Energy Release

HEC Operator executes the specific procedural steps exactly as required by the HEC Supervisor, using a copy of the LOTO Permit Form and LOTO devices. HEC Operator must verify energy has been released. Energy sources include:

• Electrical

• Vacuum

• Mechanical

• Hydraulic

• Pneumatic

• Thermal

• Other sources in machines/equipment

If the energy source cannot be confirmed as released or dead, the LOTO cannot be issued. Control Authority approval is required to proceed. Any hesitation or doubt requires the HEC Operator to report to the HEC Supervisor for further instructions.

HEC Operator verifies equipment is fully isolated and de-energized by completing the following steps (included, but not limited to):

a. Starting and observing the affected equipment. b. Inspecting local indication. c. Electrical testing (Live-Dead-Live checks) by a qualified electrical worker.

3.4 Install LOTO Device(s) HEC Operator

An equipment lock (and if needed a lockout device) must be affixed to each energy-isolating device by the HEC Operator as described in the HEC Procedure in the following manner:

a. Attach each LOTO device and complete the equipment specific LOTO energy control steps.

b. Complete the Installed section of the LOTO Permit Form after LOTO devices are attached.

c. Report to the HEC Supervisor verbally, that the LOTO devices and tags have been attached.

d. Print name, company, and contact information on the LOTO Permit Signature Form to confirm the energy isolation is complete.

e. Place keys to the energy isolating equipment LOTO devices in the designated Lockbox.

f. Equipment locks and lockout devices shall not be by-passed, ignored or otherwise defeated.

g. In addition to the equipment lock and lockout device, a Danger Identification Tag shall be attached to the lock onto the shackle. The Danger Identification Tag shall contain the following mandatory information:

a. The corresponding primary lockbox number b. The assigned Danger Identification Tag Number c. A brief description of the energy isolation device to which the equipment’s

lock, and lockout device are being attached. d. The clearly printed name of the HEC Operator applying the equipment

lock to the energy isolation device. h. The additional information may be included on the Danger Identification Tag as

optional: a. HEC Procedure number, which an identifier to the database of

procedures. b. The location of the energy Isolation device c. The isolation method of the energy isolation device.

HEC Supervisor

a. Complete the LOTO Issued section of the LOTO Permit Form and place a lock on the latch of the Lockbox.

b. Add the LOTO Permit information to the LOTO Log Form.

AUTHORIZED EMPLOYEES WORKING UNDER A LOTO

4.1 Working Under an Active LOTO

Authorized Employees meet with the HEC Supervisor to sign the LOTO Permit Signature Form. They ensure the LOTO boundaries are appropriate for the work scope, prior to working on equipment under the control of the LOTO. A detailed discussion establishes the LOTO includes adequate isolation points and appropriate positioning of components for the requested work scope.

a. Review the energy controls that are listed on the LOTO Permit Form prior to signing the LOTO Permit Signature Form.

b. Walk down the work area and boundary points to verify energy isolation for the specific work scope before starting work.

c. Print name, company, contact information, date, time, and initials on the LOTO Permit Signature Form.

1. Place a personal lock on the LOTO Lockbox in the presence of the HEC Supervisor.

2. Additional Authorized Employees may sign onto an active LOTO Permit Form and place a personal lock on the designated Lockbox without completing a separate form, but only if the job does not require modification of isolation with regards to the equipment.

3. If the work scope differs, or changes at any time, a new LOTO Permit form must be submitted.

4. Each personal lock identifies the person that placed the lock by an approved means, such as an ID tag or stamped lock number.

d. After each Authorized Employee completes their work, they must meet with the HEC Supervisor prior to removing their personal lock from the designated Lockbox and signing off the LOTO Permit Signature Form.

4.2 Committee a HEC Device

If an employee or contractor leaves the site without removing their personal lock from the lockbox and work must proceed, the following actions shall be implemented.

The HEC Supervisor must first verify that the employee or contractor whose personal lock remains on the lock box is not at the site.

All reasonable efforts to contact the employee or contractor shall be made for that person to remove their personal lock. This shall include notification of his/her supervisor. If the person can be contacted, they should report back to the site to remove their personal lockout device.

Prior to removal of the personal lock, the HEC Supervisor shall initiate and ensure completion of the Committee Form Appendix B, by performing the following:

1. Obtain written consent from the Department Head, Solar Operations (or designee) and

2. Obtain written consent from the employee’s supervisor or Job Lead.

If a personal lock belongs to a contractor, a competent representative of the that organization must be contacted for coordination and approval.

The immediate supervisor of the employee will inform and review the incident with the employee when that employee returns to work.

After completion, the Committee Form must be routed to the Sr. Manager, Solar Operations and the Renewables Safety Coordinator.

In case an employee or contractor loses his/her key, the Committee process would be conducted before the lock is replaced.

4.3 Coordination between Solar and Energy Delivery

An Energy Delivery (ED) clearance is the authorization by the Energy System Operator (ESO) to perform work within a designated zone. The authorization is granted only after all sources of hazardous energy have been isolated from all known feed points and the isolation devices have been switched and tagged. When the Solar(ES) Department requests clearance on a circuit or piece of equipment that is under the jurisdiction of the Energy Delivery Department, the switching and tagging shall be done under the orders of the ESO and shall comply with OSHA Standard 1910.269 paragraphs (l), (m) and others that may be applicable. In most cases, to safely isolate the equipment, both ED clearance and ES hazardous energy control are required from two separate isolation supervisors. The ESO is the isolation supervisor for the ED equipment and the HEC Supervisor is the isolation supervisor for the ES Equipment.

4.4 Contractor Compliance Procedures

Application

This section applies to contractors that are working under the jurisdiction of TECO that is controlled by Plant Operations.

General

Contractors are required to abide by all applicable regulatory guidelines and best business practices.

Tampa Electric Company Solar shall inform the contractor of the applicable hazardous energy sources, the type and magnitude of energy available, and the means and methods necessary for energy isolation and control. This information is provided in writing on every TECO Procedure. Additionally, the facility HEC Supervisor can provide additional information upon request.

In instances where Tampa Electric Company and contract personnel may encounter each other’s lockout/tagout devices, Tampa Electric Company and contractors shall exchange information regarding the HEC program to be used by each employer’s workers. Each employer shall ensure that their personnel understand and comply with restrictions and prohibitions of the energy control program being used.

CHANGING LOTO

5.1 LOTO Change Requirements

The status of a LOTO can be changed to allow removal of locks to allow a component or a portion of a system under the LOTO to return to service only if Control Authority or designee agree that the work can continue safely.

A change of LOTO status is made only after:

• The HEC Supervisor has reviewed the proposed change and determined the change does not compromise the initial LOTO.

• All work associated with the LOTO is stopped.

• A description of the change and the reason for it will be recorded on the LOTO Permit Form.

• The HEC Supervisor changes the LOTO Permit Form as needed (locks removed, switching performed and additional locks/tags/grounds added).

• The HEC Supervisor instructs the Authorized Employee to perform the actions identified on the changed LOTO Permit Form.

• Once changes have been performed, the HEC Supervisor reviews the LOTO Permit Form and re-signs approving the changed LOTO.

• The Job Safety Risk Briefing (JSRB) is re-performed to discuss changes and change impact with all Authorized and Affected Employees prior to work resuming.

SHIFT CHANGE

6.1 Authorized Employee Transfer

A separate JSRB must be conducted during shift or personnel changes for continuity of LOTO protection. This includes the orderly transfer of LOTO device protection between shift changes and off-going and on-coming employees.

Equipment that is locked out during shift changes, the Authorized Employee on the next shift must apply their LOTO devices before the employee that is leaving can remove their LOTO device from the Lockbox.

6.2 HEC Supervisor Transfer

The HEC Supervisor Transfer Form will be completed only when a physical briefing cannot occur between two HEC Supervisors. The form is used as a tool for knowledge/document transfer from one HEC Supervisor to another.

REPOSITIONING AND TESTING LOTO

7.1 Removal of LOTO for Repositioning or Testing

LOTO devices may be temporarily removed from energy isolating device(s), and the equipment energized for testing, repositioning or the temporary introduction of energy only. No maintenance or repair work will be performed on equipment that is in test operation.

a. The employee requesting the test will contact the HEC Supervisor to request the test operation. The HEC Supervisor will either approve or deny the request.

b. If the request is approved, the HEC Supervisor will complete LOTO Test Form. c. The HEC Supervisor will then designate an employee to verify the following:

1. Removal of all tools and materials from the affected system(s) and/or equipment to be tested;

2. Equipment components are operationally intact; Machine guards or other devices that were removed are installed. NOTE: Where it is impractical to reinstall machine guards, red barrier tape and danger warning tags may be used.

d. Each Authorized Employee shall be contacted to remove his/her lock from the designated Lockbox

7.2 Sign off the LOTO Permit Form

a. When signing off the LOTO for the test, employees will indicate whether they have completed their work task in the “Removed” column next to their name on the LOTO Permit Form.

b. When all Authorized Employees have removed their locks from the designated Lockbox, the HEC Supervisor will list the energy isolating devices that need to be removed for the test, and their position for the test on the LOTO Test Form and remove their lock from the designated Lockbox, removing only the key(s) necessary to do the test.

c. The HEC Supervisor then designates an employee to remove only the LOTO devices necessary to do the test.

d. The LOTO devices will then be returned to the HEC Supervisor for verification of correct LOTO device and tag removal. The Authorized Employee then prints their name on the “LOTO Devices Removed By” line on the LOTO Test Form.

a. The HEC Supervisor will maintain control of all removed LOTO devices during the test operation.

e. The HEC Supervisor, or designee, will verify that the work has been checked to ensure that all employees have been safely positioned or removed.

f. The removed devices will be energized and testing and positioning may proceed. g. Following the test, the Authorized Employee that completed the test, will contact

the HEC Supervisor to sign the “Test Completed By” line on the LOTO Test Form.

h. The LOTO may then be reinstated or released as determined by the HEC Supervisor.

7.3 Reinstating LOTO after Test

a. The HEC Supervisor will circle “Reinstate” on the LOTO Test Form indicating that the LOTO is to be reinstated.

b. The HEC Supervisor will then designate an HEC Operator to de-energize the system and return isolation points to the isolation position indicated on the LOTO Test Form. The HEC Operator will then reapply all isolation devices that were removed for the test.

c. The keys, to the energy isolating equipment lockout devices, will then be placed back in the designated Lockbox.

d. Following the isolation and attachment of the LOTO devices, the HEC Supervisor, or designee, will walk down the isolated equipment to verify the LOTO, and ensure, to the extent possible and location/condition of the equipment, there is no stored or residual energy.

e. Once the walk down is complete, the HEC Supervisor, or designee, prints his/her name on the LOTO Test Form.

f. The HEC Supervisor will then sign the LOTO reinstated line on the LOTO Test Form and reattach their lock onto the designated Lockbox.

g. The HEC Supervisor will then contact all employees with incomplete work, from the LOTO Permit Form, to sign onto the LOTO Permit Form, and reapply their locks to the designated Lockbox before beginning work.

RELEASING LOTO

8.1 LOTO Release Preparation

a. HEC Supervisor verifies that all employees have completed assigned work, signed off the LOTO Permit Signature Form, and removed their personal locks from the designated Lockbox.

b. HEC Supervisor assigns a TEC Solar Technician (or equivalent competency) to verify the equipment for the following:

• Cleanliness (All tools, parts and debris are removed and disposed of properly)

• Doors closed and properly secured

• Equipment is ready for restoration and operation

8.2 Equipment Restoration and Operation

HEC Supervisor

a. Notify all Affected Employees that the restoration of equipment is about to commence and confirm unnecessary personnel are removed from the work area before it is released from LOTO.

b. Communicate the LOTO specific restoration steps to designated Authorized Employee for equipment restoration.

c. Remove lock from the designated Lockbox latch and release the energy isolating device keys to the designated TEC Solar Technician (or equivalent competency) for equipment restoration and operation.

Authorized Employee (TEC Solar Technician or equivalent competency)

a. Use the copy of the LOTO Permit Form and energy isolating device keys, to execute the equipment specific restoration steps exactly as required by the HEC Supervisor.

b. If there is any hesitation or doubt, report to the HEC Supervisor immediately for further instruction.

c. Remove appropriate LOTO devices. Operating procedure may be performed simultaneously with the removal of the lock. Equipment should not be placed in service until entire process has been restored to normal operating state.

d. Report to the HEC Supervisor that steps are complete and return LOTO devices. e. Complete the Removed section of the LOTO Permit Form.

HEC Supervisor a. Verify all required released LOTO devices and keys have been returned. b. Complete the LOTO Released and Permit sections of the LOTO Permit Form.

GROUNDING LOTO

9.1 Qualifications

Grounding may only be performed by qualified electrical workers that are specifically trained to perform the attachment of grounds and they must be on the Authorized Employee List.

9.2 Attaching Grounds

Grounds are attached per TEC’s Energy Supply Electrical Safety Program. All qualified electrical workers involved in attaching grounds must perform the following steps:

a. Perform Live-Dead-Live checks prior to installing grounds.

b. Attach grounds and apply tag to the ground sets.

c. Apply grounding (green) hasp to appropriate Lockbox in the control house.

d. Sign onto Issued LOTO Permit Form.

9.3 Discovery of Incomplete Isolation

During verification, if the HEC Operator discovers that the system is not able to be isolated completely, the HEC Operator must notify the HEC Supervisor to implement the following steps:

1. The HEC Supervisor will determine new isolation points in order to effectively isolate the system. The HEC Supervisor shall contact the Job Lead(s) regarding the new isolation points.

2. If isolation cannot be effectively completed the HEC Supervisor shall assemble a team of personnel to include at least the following: the HEC Supervisor, HEC Operator(s), the Job Lead(s), and the Job Lead’s supervisor(s) whether contractor or TEC to determine how the work will be made safe by employing safety-related work practices that are consistent with the nature and extent of the associated hazards such as: Line Breaking, Electrical Safe Work Practices, etc.

3. If the work cannot be made safe, repairs may only be made when the system in question is completely shut down and grounded, if possible.

4. Any corrections in the HEC Procedure that need to be made permanently shall be identified and submitted to the appropriate HEC Supervisor for the procedure to be updated in the master HEC Procedure file.

TRAINING REQUIREMENTS

10.1 Affected Employee

Affected Employees will receive an overview of Tampa Electric Company’s LOTO Procedure along with the Site Safety Orientation. The Site Safety Orientation is good for one year and is NOT transferable from site to site.

10.2 Authorized Employee

LOTO Authorized Employee training is required annually and anytime a lack of knowledge, near miss, significant incident or change has occurred to the procedure. Authorized Employee training is transferable from site to site.

Upon successful completion of the initial training and 6-month experience working at the site, Authorized Employees are added to the Authorized Employee List and complete the LOTO Training Record for Authorized Employees and considered Competent Employees for the site, likewise Contractors are considered Competent Contractor Employees.

10.3 Removal from the Authorized Employee List

Any person that deviates or demonstrates inadequacies in knowledge, use of the LOTO Procedure, or fails to remain current on LOTO Procedure training, will be removed from the Authorized Employee List by the Control Authority. The Control Authority notifies the supervisor or contractor company representative of the employee.

An employee cannot be reinstated until retraining, requalification, and Control Authority approval has been obtained.

AUDITS

11.1 Monthly Inspections

Monthly inspections, of site LOTO activity, are completed by the Control Authority (or designee) to identify potential deficiencies with the execution of the LOTO Procedure and any area for improvement. Findings are assigned a corrective action and sent to the HEC Committee for review. Corrective Actions will be archived on the Solar Operations SharePoint Documents site (Appendix F).

These inspections may include:

• Review of active LOTO documents for accuracy and completeness.

• Field inspection of LOTO devices from a random selection of active LOTOs to identify improvements.

• Field inspection of Authorized Employees working under an active LOTO.

• Review of all released LOTO documents for discrepancies.

For HEC Supervisors, a monthly administrative inspection of LOTO will be conducted while a field inspection of an active LOTO will not exceed quarterly.

11.2 Annual Audit

A comprehensive audit of the overall Solar LOTO Procedure will be performed, at a minimum, at one site on an annual basis. The audit will be conducted by a team designated by the Control Authority (or designee) that could include a HEC Supervisor, HEC Operator, Job Lead, and/or Job Lead’s supervisor.

This audit should include a review of all LOTO documentation for all solar sites for that calendar year as well as any incidents and investigation, as a result of the LOTO throughout the year. The audit also includes interviews with employees responsible for various functions under the LOTO Procedure. Completed annual audit assessment report forms are provided to the Control Authority. The completed reports will be forwarded to the Senior Management Team and HEC Committee as well as any lessons learned shared with the organization.

RECORDS

12.1 LOTO Book

A LOTO Book is maintained at each Tampa Electric Solar site for each organization controlling equipment. It is available in the Control House for review and contains the following:

• Active and released LOTO Permits

• Authorized Employee List (electronically recorded)

• Current copy of the LOTO Procedure

• Current LOTO Log

12.2 Record Retention

LOTO Books are always maintained and current. LOTO Permit retention is five years.

APPENDIX A - GLOSSARY

Affected Employee: Person whose job requires them to operate or use equipment that service or maintenance is being done and is isolated using LOTO. This includes employees that work in areas where this type of servicing or maintenance is being done. An Affected Employee may also be an Authorized Employee. This occurs when the Affected Employee performs maintenance or service on equipment that must be isolated using LOTO.

Authorized Employee List: List of employees trained on Tampa Electric Company’s LOTO Procedures and perform functions related to the procedure. May include Tampa Electric Company’s employees and Competent Contractor Employee. This list will be maintained by the Control Authority.

Capable of Being Locked Out: Energy isolating devices are considered capable of being locked out in the following situations:

• If it has a hasp or other attachment that a lock can be attached to prevent accidental operation

• If it has a locking mechanism built into it to prevent accidental operation

• Energy isolation device that can be locked into position using commercially available lockout devices and industry recognized isolation methods

Control Authority: Individual that has ultimate responsibility for the equipment and operation of the site. This may be the Department Head (or designee).

Competent Contractor Employee: Contractors trained on Tampa Electric Company’s LOTO Procedures and perform functions related to the procedure. Contractor is also capable of identifying existing and predictable hazards in the surroundings or working conditions which are hazardous or dangerous to personnel and who has authorization to take prompt corrective measures to eliminate them.

Danger Identification Tag: An identification tag applied with a lockout device to provide the following information:

• The corresponding primary lock box number.

• The assigned Danger Identification Tag Number on the written HEC Procedure.

• A brief description of the energy isolation device to which the lockout device is being attached.

• The HEC Operator will clearly print their name and the date that they hung the lockout device.

• The HEC Operator will clearly print their name and the date that they removed the lockout device.

Disconnect: Device that isolates the source of electrical energy to a piece of equipment. An acceptable disconnect must have a LOTO device attached so it is inoperable while work is being performed.

Energized: Connected to an energy source or containing residual/stored energy.

Energy Isolating Device: Mechanical device that physically prevents the transmission or release of energy including, but not limited to, the following:

• manually operated electrical circuit breaker

• electrical disconnect switch

• Manually operated electrical switches by which the conductors of a circuit may be disconnected from all ungrounded supply conductors, and by which no pole may be operated independently

• Any similar device used to block or isolate energy

Note: Push buttons, selector switches, or other control type devices are not considered isolation devices.

Ground: Conductor used to connect a piece of equipment to earth, to prevent the buildup of voltage that may result in undue hazards to equipment or personnel.

HEC Operator: Assigned individual to isolate equipment in preparation for establishing a LOTO, applies and removes the locking devices. The HEC Operator and HEC Supervisor may be the same person.

HEC Supervisor: All qualified Tampa Electric Company’s employees assigned the responsibility of issuing and releasing LOTO.

Job Lead: A person or supervisor who exercises guidance for a group or crew of Authorized Employees, and coordinates with the HEC Supervisor to ensure adherence with Solar HEC LOTO Program. A Job Lead may be a Tampa Electric Company employee or a Contractor.

Live-Dead-Live Checks:

1. Verifying (and confirming) the functionality of electrical testing equipment prior to beginning work with an energized conductor or circuit.

2. Testing the circuit to verify de-energized state. 3. Re-verify the electrical test equipment to re-confirm functionality with an energized

conductor or circuit.

Lockbox: Lockbox for storing the key(s) to the orange equipment locks of an active LOTO.

Lock Out Device: Device designed to hold energy-isolating devices in safe positions to prevent machines or equipment from being energized or manipulated. Lock out devices include locks in conjunction with cables, circuit breaker blocking devices, and fuse blocking devices, etc.

Lock Out Tag Out (LOTO): A set of practices and procedures to safeguard workers from hazardous energy releases.

APPENDIX A - GLOSSARY

Primary Lockbox: This lockbox contains the key(s) to the locks on the energy isolating devices. The primary lockbox is maintained in each sites’ substation control house. This lockbox serves as a central isolation point for the HEC Supervisor, Job Lead and Authorized Employee(s) to place their individual locks. This is the lockbox from which associated satellite lockboxes are derived.

Qualified Electrical Worker: A worker who has demonstrated skills and knowledge related to the construction and operation of electrical equipment and installations and has received safety training to identify the hazards involved and reduce the associated risk. See OSHA 29 CFR 1910.269 (a)(2)(ii).

Additionally, NFPA 70E National Electric Code Article 110.2 (D)(1) requires that only a qualified person (worker) perform work on or near exposed and energized electrical conductors or circuit parts. A qualified person must be knowledgeable of:

• Safe work procedures and precautionary techniques

• Selection and use of PPE, including insulation and shielding materials, insulated tools and test equipment.

• Emergency practices, such as the ways to release a victim of shock who is in contact with an exposed conductor or circuit part.

Satellite Lockbox: Lockbox to which authorized employees may apply their identifiable personal lock to afford themselves individual protection from hazardous energy while performing servicing or maintenance activities.

Servicing: Workplace activities such as, but not limited to, construction, installing, adjusting, inspecting, testing, modifying, and maintaining machines or equipment where an employee may be exposed to the unexpected energization or start-up of the equipment or release of hazardous energy.

Tag: An openly displayed card, ticket, plastic marker, etc. securely attached to a piece of equipment as a label to give information, warning or instruction. Accident prevention tags have standard signal words, symbols, and colors to convey a danger, warning, caution, or information.

Work Scope: Written details of the job(s) to be performed, equipment or systems to be worked on, and any details necessary to determine what systems or equipment must be isolated to ensure the safety of personnel performing work.
