We actually had a quick tour by Bob Moulton (the Telescope Operator) before he went to bed and, the next morning we had another quick tour of parts of the telescope). During the regular tour we found some issues with housekeeping, which were all taken care of within an hour. Our impression of the two employees we spoke with was that they both are concerned with the safety of the staff and the observers. To our knowledge there have been no major or lost time accidents at this telescope in 12 or more years in operation. That is a good indication that employees are safety conscious. The major concerns at the SMT are: First, we think there should be training in lockout-tagout and each employee should have his or her own lock, and tag. Next, there should be a lockout box purchased which can accommodate several locks because at present the key can be removed at the consul and keep Tucson employees from moving the telescope but if more than one person needs to lock out, then, the key system does not work. (Look at the lock box being used at the LBT or reference the photo below). 1

There was also some concern for handling of cryogens. We did see some safety gloves and masks, but came away with the opinion that cryogenic training for all operators and technical personnel would be good, and needed. We asked for copies of the crane inspections and were told that they did not know when the last inspections were made or where copies were. We suspect it has been quite some time since the last inspection and it should be done soon.

We asked about safety concerns and one concern is the lack of tie off points above the door platforms. It is not obvious where one could install tie off points on the platforms, except possibly removing sheet metal siding and welding hooks onto the structure behind. Perhaps when Risk Management is on the mountain they could lend some assistance in this matter. Also, there is no cable system on the inside of the roof panels like the one on the outside. A cable tie off on the inside would make the changing of seals much safer, and certainly easier.

We also think that each employee who agrees to climb and does so should have his or her own safety harness and lanyards. We were told that when anyone works on the platforms or roof, there are at least two people and that policy must be enforced.

There were also concerns expressed about road conditions such as cars with chains that do not fit, and winch safety for observers driving in the snow and ice. Operators recommended a protocol be developed for all Mt. Graham visitors and staff and that it be implemented by MGIO as an organized effort to train all drivers.

Another concern of ours was that the interlock on the top floor back door was disconnected and under the present condition, anyone with access to the telescope could go up on the roof and be there when the door opens or the telescope moved. This is not good. If someone needs to work on the roof, the telescope should be locked out and if no one is on the roof, the door should remain locked or the interlock activated. Another possible solution would be to install a gate on the enclosed ladder to the roof and lock it out so that only the telescope operator would have a key and he then would be sure to know if someone was going to the roof and could lock out the telescope.

One last concern was the smell of sewer gas in the bedroom #1. This may not be poisonous but it was strong and noticeable and should be corrected.
