Why Korea?Dentistry 2

Strengths of Korean dental clinics

1 Superior technologyThe delicate and accurate dexterity of Koreans is well known in several fields. In particular, medical and scientific technologies that require sophisticated hand techniques, such as stem cell extraction, microscopic surgery, and robot surgery, prove that Korean scientists and physicians have extraor-dinary skills. For this reason, the number of overseas patients visiting Korea for medical and dental surgeries or treatments is increasing. This is particularly advantageous in the dental field, where most procedures require delicate and sophisticated manual techniques.

2 Standardized dentist training education Korea has been training dentists at 11 colleges and schools of dentistry with systematic curricula and clinical practices. As such, 11 specialty-curriculums certified by the Ministry of Health and Wel-fare have been used in training dental specialists every year since 1967. In addition, it is mandatory for all dentists to learn up-to-date techniques through supplement train-ings to maintain their dentist licenses. Through this system, the ability of Korean dentists can be always up-to-date.

3 Top-quality domestic implants and other dental materials The number of implant and dental materials certified by the Ministry of Food and Drug Safety used in the domestic market is estimated to be dozens, most of which are made in Korea. In particular, top market share manufacturers (e.g., Osstem, Dentium, Neobiotech, Megagen, Genoss) leading the domestic market with an increasing overseas market share have been well recognized by both domestic and foreign clinicians, in terms of long-term stability of treatment results. The results of lab-scale, preclinical, and clinical studies using materials, as well as of clinical practices, have been published and cited in many renowned SCI(E) journals, providing world-recognized excellence of domestic materials.

14 Efficient communicationThe Korean educational system includes foreign language courses. For this reason, many Korean dentists are fluent in English, and the number of physicians speaking Arabic has been increasing due to the increasing number of patients visiting from the Middle East. The number of physicians from Saudi Arabia and Kuwait coming to Korea for specialty training has been steadily increasing, efficiently solving problems in treating patients from Arabic countries. In cases of patients who speak a language that the physician does not speak, the major dental hos-pitals can get support from multi-lingual translators/interpreters during the entire dental treatment.

5 Extensive clinical experience According to the National Health Insurance Service, chronic periodontal disease is the second most common disease diagnosed in Korea. Severe periodontal disease, dental caries, and tooth fracture require implant after dental extraction, and due to the implant insurance service provided for patients over 65, implant treatment cases have increased. As stated previously, the number of foreign pa-tients has been increasing every year to receive dental treatment from experienced physicians who have completed systematic education. This has provided Korean physicians opportunities to earn experience in treating foreign patients. In addition, Korean dentists have a deep understanding of the biological and cultural differences of diverse foreign patients, allowing them to care for patients in a physical, as well as psychological, aspect.

6 Excellence of domestic digital devicesKorean IT has been leading the academia and the global market for the few past years. Such power of technology accelerated the development of digital devices used in dental practices, leading to the development of a domestic oral scanner (e.g., Medit), implant planning software (e.g., OnDe-mand), a 3D printer (e.g., Dentium 3D printer), and a digital guide implant surgery kit (e.g., Osstem OneGuide), all of which are widely recognized in the domestic and foreign markets. Numerous study results using these products are reported, and both the domestic and overseas demand for these products are expected to increase.

7 Great reduction in treatment period through digitalized processDigital dental devices reduce the overall treatment period and simplify the procedure, shortening the recovery period as well. For example, in an implant case of an overseas patient, the dentist can con-duct an implant surgery using the digitally manufactured surgical guide on the very day of the first visit and connect the digitally manufactured final protheses within a few hours of the surgery, thus completing the treatment in one day. Such feature can be very helpful for foreign patients visiting Korea to receive treatment from Korean physicians. It enables the patient to have shorter stays in Korea, thus lessening the patient’s burden and reduces after-surgery inconveniences.

Oral and maxillofacial surgery

Precision of oral and maxillofacial surgery using 3D simulation surgery and printing1 Screening For a CT image, it is easy to detect the lesion in the bone, but for an MRI image, it is easy to diagnose the lesion in the soft tissues. Therefore, for an accurate diagnosis, it is necessary to take the lesion from the soft tissue, convert to a 3D image, and combine it to the 3D image made from the CT. Screening requires less amount of bone reduction, protecting major anatomical structures while avoiding invasive additional surgeries like tracheostomy.

Final nesults of surgical simulation

Fibula modulation using a surgical guide

Conventional method Screening

2 Simulation Procedure of the mandular reconstruction using fibula free flap The amount of bone reduction and reconstructive surgery plans are carefully planned beforehand through simulation. In particular, reconstructive surgery requires creation of post-surgery face of the patient as similar as possible to his pre-surgery face. Surgery is planned considering for functional and psychological result of the patient, but application of such plan to the surgery is comparably difficult.

3 Production of 3D printing surgical guide A surgical guide is needed to reproduce simulation results in the operation room. A surgical guide is designed based on the 3D model and printed using the 3D printer.

4 Surgery and resultsApplying a 3D surgical guide allows simplicity, accuracy and promptitude of surgical procedure.

5 Evaluation of surgical resultThe surgeon runs a a final evaluation of the surgical result.

Prosthetics

1 Immediate and early implant placementThese are implant placement procedures right after extraction of teeth or within few weeks after teeth extraction. This way we can reduce the number of visits for surgery and also overall treatment period for patient. It has advantages of placing implants while healing potential of the bone is higher. Early implant placement can be successfully used in cases of teeth extraction due to the periodontitis.

Tooth fracture Immediate implant placement after tooth extraction

2 Implant placement and prosthodontics treatment with digital guide stentImplant guide stent is a device that helps the placement by accurately guiding an implant to its desired location. Depending on the implant used for the stent, a dentist can use a special surgical instruments to place the implant in its planned place. In some cases, a dentist can perform a flapless or minimal flap surgery, which is removal of soft tissue in the area for the implant without incision. If initial stability of implant at the time of the placement is good and a guide that allows the precise implant placement and a digitally designed and fabricated restorations, teeth in a day or teeth in a few hours can be performed.

3 Esthetic restorations Esthetic restorations, such as laminate veneer crown and all ceramic crown can improve discolora-tion, deformation of the teeth and spaces between anterior teeth.Laminate veneer can be choose for small amount of the gap between teeth, slightly crooked teeth, small amount of discolored teeth. For this treatment, thin layer of enamel which is outer surface of the tooth could be removed for creating space for new restoration.

Compared to laminate veneer, all-ceramic restorations needs much more tooth reduction, but it can be selected for cases of severe tilting of front teeth, fracture of teeth, large decayed teeth and replacement of old crowns with problems.Depends on tooth condition, zirconia or other ceramic materials can be used and most of the labora-tory procedures usually doing by digital way, these days.

2 Aesthetic appliance - Clear Aligner Using a digital tooth set up software the serial tooth movement is printed using a 3D printer. Then, the printed models are used for manufacturing the clear aligner. In Korea, digital setup programs have been developed with high prediction of tooth movement and are widely used in clinical practices.

Enamel matrix derivative applied to the destructed periodontal tissue

matrix derivative

1 Periodontal regenerative treatment using enamel matrix derivativeEnamel matrix derivative is made of proteins that form cementum during teeth development like Amelogenin, and its application to the conditioned root in the tissue destructed area induces regen-eration of the periodontal tissue. Enamel matrix derivative can be used by itself or along with bone graft material or barrier membrane, and many years of clinical practices show biologically stable result and improved periodontal attachment.

2 Patient-customized CAD/CAM-based barrier membraneWith the development of digital dental devices, practitioners can 3D-analyze a patient’s destruct-ed periodontal tissue using the CT image, analyze the soft tissue formation image extracted from the oral scanner, design a barrier using a computer software, and use the 3D printer to create a patient-customized barrier in the surgery. Such customized barrier reduces the surgery time while stabilizing the surgical outcome, contributing to the gradual development of guided tissue regener-ation technique.

3 Microscopic free gingival graft In the past, free gingival graft was performed with bare eyes, which necessarily had limitation in delicacy. Since soft tissue graft requires fine technique for successful outcome, surgery under mi-croscope and assisting devices has been introduced. Under microscopic view, delicate manipulation of soft tissue became possible, and devices assisting the surgeon technique offer higher accuracy in achieving the graft from donor site and applying the graft to recipient bed. Through these tech-niques, the outcome and prognosis of gingival graft can be improved and more reliable.

4 Non-Prosthetic Orthodontic Treatment It refers to an orthodontic technique that uses miniscrew to close the missing space without pros-thetic restoration such as implant and bridge by protracting the molars or wisdom tooth to the edentulous region. This non-prosthetic orthodontic treatment that uses the patients’ own sound tooth proved to have therapeutic effects.

3 3D printing-based customized orthodontic treatment Conventional orthodontics brackets do not reflect each patient’s characteristics, and they cannot be used in patients with severe skeletal discrepancy. In this case, 3D CAD software allow manufac-turing of customized wire and braces for specific treatment purposes through virtual simulation of the patient’s final occlusion. The manufactured customized brackets are placed to the patient teeth using the indirect bonding tray made by the 3D printer. The customized bracket can be used for the nonsurgical treatment of patients with skeletal malocclusions, such as asymmetry and mandibular prognathism.
