    2011-2012 SLICE: Student Leaders in Civic Engagement Member Contract

    SLICE members are required to:

    Attend all SLICE meetings, on Thursdays from 1:00-2:30pm.You can only miss two meetings eachsemester. Meetings start promptly at 1:00 pm. Being late more than 15 minutes is considered an absence.

    Enroll as a full-time student with a minimum of 12-18 credit hours per semesterMaintain a minimum 2.5 GPA each semesterComplete 100 direct service and training hours per semester. Fifty-Five of the 100 hours must be direct

    service. Forty-five hours are weekly meetings, e-mail communications, and workshops.

    Be a Utah resident File a FAFSA (www.fafsa.ed.gov) and have the results sent to SLCC (Federal SLCC Code: 005220). If

    you have questions, call Financial Aid at 801-957-4663.

    Include a copy of your high school transcript (for new students) or SLCC transcript (for continuingstudents). Unofficial transcripts are acceptable.

    Submit official service hour tracking logs every week.Submit reflections (using KWL charts) on service, presentations, leadership development, etc.Participate in monthly team activities, which may include evenings and weekends with appropriate noticeAttend a mid-semester and end-of-semester meeting with the Thayne Center for Service & Learnings

    Service Leadership Coordinator.

    Earning Your Tuition Waiver

    1.A Letter of Counsel will be given as a warning if you fail to follow the above requirements.2.At the end of the semester, if you have not completed your 100 hours, a hold will be placed on your records until

    either the hours are completed or the cost of your tuition has been paid back to SLCC.

    3.If a tuition wavier has been awarded and you leave the program before your contract is fulfilled, you areresponsible to repay the college the cost of the waiver.4.If any requirements of this contract are not met, you are not eligible for future SLICE membership.

    Academic Status:

    1.Enroll full-time, a minimum of 12-18 credit hours per semester.2.Earn a minimum semester GPA of 2.5. If your semester GPA is between 2.0 and 2.5 at the end of the term, you

    may petition to continue with the SLICE program on a probationary status. If approved, you will be required toestablish an academic plan approved by the Service Leadership Coordinator. If your semester GPA is below 2.0 a

    the end of the semester, you may petition to continue in the SLICE program under the following conditions: 1)

    You have a monthly review to assess your academic progress; and 2) You arrange for a least an hour a week of

    tutoring.

    Probationary Review:

    All SLICE members will complete a mid-semester review with the Service Leadership Coordinator. If you are missing

    any of the criteria listed in this contract, you will be placed on SLICE probation for the remainder of the semester. You

    must establish a plan with the Service Leadership Coordinator for getting off probation. If you are still on probation by theend of the semester, you are not allowed to participate in the SLICE program in the next semester.

    If you are placed on probation at the end of the semester, the probation period lasts until the middle of the next semester.

    If you are still on probation at the middle of the next semester, you may be asked to leave the program at that point you

    will be fully responsible to repay your tuition waiver.

    Its especially important to maintain all requirements if you are interested in applying to become a second-year SLICE

    Mentor. Above all, please discuss any difficulties you are having with the Service Leadership Coordinator. We will do

    what we can to help you succeed in this program and in your educational goals.

    By signing below, I commit to adhere to the criteria outlined in this contract. If extenuating circumstances exist

    they will be discussed immediately with the Service Leadership Coordinator. I understand that my failure to meet

    any of the above conditions may result in probations and/or termination as a member of the SLICE Program. I

    also understand that if I do not meet the above conditions, I may be responsible to repay my tuition wavier.
