City and County of San Francisco Gavin Newsom, Mayor Department of Public Health San Francisco General Hospital Department of Pharmaceutical Services 6 January 2010 To Whom It May Concern: Jonathan Mueller has been employed at the San Francisco General Hospital Department of Pharmaceutical Services as a pharmacy technician on an ‘as needed’ per diem basis since October 7, 2007. As inpatient supervisor for the last two years, I have been Jonathan’s direct supervisor and have been responsible for his annual performance evaluations. As a per diem technician, Jonathan is required to work in all areas of the inpatient pharmacy where a need arises. He has demonstrated the ability to quickly learn and to master the requirements necessary to work in each of the various shifts. These assignments range from unit dose filling, delivery, restocking automated dispensing cabinets, triaging phone requests and managing requests at the pharmacy walk up window. Additionally, he is most often assigned shifts in IV and Chemo compounding. He is an excellent worker, notably accurate and efficient. He also possesses very good interpersonal and communications skills

6 January 2010 To Whom It May Concern: Jonathan Mueller has been employed at the San Francisco General Hospital Department of Pharmaceutical Services as a pharmacy technician on an ‘as needed’ per diem basis since October 7, 2007. He also possesses very good interpersonal and communications skills which are demonstrated by the good working relationships with other pharmacy staff as well as his ability to handle the demands from nursing and medicine on the phone and at the window. His greatest assets are his adaptability and willingness to work cooperatively, a true team player. Because he has reached the annual fiscal cap for per diem staff, SFGH Pharmacy will not be able to rely on him as heavily as we have earlier in the year. Jonathan would be an asset to any organization. If positions are available, we would certainly attempt to hire him. I wholeheartedly recommend him Sincerely, Terrence Wong, Pharm.D.SFGH Inpatient Pharmacy Supervisor
