1.0 Scope
1.1. PartiestotheAgreement
ThisdocumentservesasanagreementofservicestobeprovidedtotheaforementionedstakeholderbyFS.Thisagreementappliestothebuilding(s)and/orfacilitiesnotedinthecoverpage.
1.2. ProductsandServices
FSwillprovideacomfortable,functional,safe,andreliableenvironmentthroughBasicServices,
RechargeServices,UnfundedProjects,OperationalProjectsandCapitalRequests.
BasicServices:coversthescopeofworknecessarytoproperlymaintaincampusbuildingsandgrounds.
Providedatnoadditionalcost,theyinclude,butarenotlimitedto,standardcustodialcoverage,
mechanicalsystemmaintenance,buildingenvelopemaintenanceandgroundsmaintenance.These
servicesareprovidedperthisSLA.BasicServiceislimitedwithintheparametersoftheoperatingbudget
perfiscalyearandaredefinedinto,butnotlimitedto(8)eightservicecategories:
1. Architectural:FinisheswillbeincompliancewiththeNFPAandIBCstandardsandcodes.
Decorativefeaturessuchasornateironworkandwoodworkwillbereviewedonacase-by-case
basis.
2. Custodial:Refertocustodialservicesone-sheet(Section5.0)
3. Electrical:Lighting,poweroutlets,powerdistributionandfirealarmsystemswillbemaintained
perNFPA,IBCandlocalstandards.
4. Elevatorsandlifts:Allunitswillberepairedandmaintainedtoallowforsafeoperationsthat
conformtoindustrystandards.Anelevatormechanicmustrespondwithin30minutesorlessfor
entrapmentsduringregularworkinghours,MondaythroughFriday6:00A.M.-6:00P.M.,and
within60minutesduringoffhours.
5. Grounds,wasteandsnow:Landscaping,snow-removal,exteriortrashremovalandgeneralclean-
upofuniversitypropertywillbemadeperAPPAstandards.Snowremovalprioritizationwillbe
madetoaccessiblepathsandramps.
6. Mechanicalandutilities:Heatingandcoolingsystemswillbemaintainedtoprovideoccupant
comfortwithinASHRAEstandardsforastandarddesignday.Theseincludeheatingandcooling
seasons:winter:68-72degreesF;summer:70-74degreesF.
7. Pestcontrol:Interiorandexteriorinspectionandmonitoringofbuildingsandgroundswillbe
maintainedonamonthlybasisbyauniversity-contractedserviceprovider.
8. Plumbing:Hotwater,coldwaterandsewagelineswillbemaintainedperlocalstandards.
RechargeServices:coversworkrequestedbythestakeholderthatisoutsideofBasicServices.Theseitemsinclude,butarenotlimitedto,specialcustodialrequests,updatingand/orenhancingofficespaces
andspecialeventsservice.AstakeholdercanrequestanestimateforanyscopeofworkoutsideofBasic
Servicesforbudgetplanning.Rechargereportingcanbemadeavailableonrequest.
UnfundedProjects:FSwillstrivetocompleteallworkwithinthescopeofBasicServices,however,
situationsmayarisewherethebuildingmaintenanceneedsoutpacetheallottedbuildingoperating
budget,resultinginworknotbeingcompletedduringthefiscalyear.Unfundedprojectswillbe
prioritizedacrossthecampus.Astakeholdercanrequestforanunfundedprojecttobereviewedforthe
currentyearandforfuturefiscalyears.
OperationalProjects:FSoperationsstaffmanagesprojectstomaintainandenhancethebuilt-
environment.Requestsforestimatesdonotincurafee,buttheworkitselfmaybearechargeservice.
Pleasecoordinatewithyourzonemanagershouldyouhaveadditionalquestions.

CapitalRequests:FSprojectmanagersareabletoservewithdesign,constructionandrenovationneeds.
Additionalinformationonmeans,methodsandapprovalscanbefoundontheFSwebsiteforcurrentand
futurefiscalyears(QuickLinksalsoavailablebelow).
1.3 ResourceAssumptions
TheFSdepartmentwillperformduediligencetoprovideservicesasoutlinedbelow,providedthatFS
funding,staffingandmaterialresourcesaremaintainedatcurrentlevels.
1.4 Contacts
TheprimarycontactlistcanbefoundinSection6.0.Shouldissuesarise,thesuccessionofcontactswill
followtheescalationactionoutlinedin2.3DisputeResolutionandEscalation.
1.5 Renegotiations
ThisSLAmayberenegotiatedatanytimeandmaybeinitiatedbyeitherparty.Servicesunderterm
agreementsmaynotbeabletoberenegotiated.Shouldtheservicereceivernotagreetothetermsof
theSLA,FSbasicserviceformatshallbeutilized.
2.0 ServiceDetails2.1 ServiceCategories
2.1.1 Urgentissues:
Anyissuedeemedurgentneedstobecalledintothework-centerat773.834.1414.Awork
orderwillbecreatedfromthecallandaworkordernumberwillbeassigned.Urgentissues
aretypicallycategorizedaspriority1orpriority2butmayincludeservicesrelatedto
programmaticfunctionsorevents(seetablebelow).
2.1.2 Generalorscheduledissues:
Therequestorshouldsubmitaservicerequestatfacilities.uchicago.edu.Aworkorderswill
beassignedandtherespectiveshoportradewillrespondwithin7workingdays.Ifthereare
questionsastoiftheworkorderisabasicorrechargeservice,therespectiveshoportrade
groupwillworkwiththezonemanagertonotifytherespectivestakeholderforfundingand
scheduling.Generalorscheduledissuesaretypicallyprioritizedaspriority3orpriority4(see
tablebelow).
2.1.3 PM
AllPMrelatedtoarchitectural,mechanical,electricalandplumbingwillbeperformedbased
onthemanufacturer’srecommendationsordocumentedhistoricalevidence(RuntimePM).
PMwillbeplannedandscheduledbyFS.Thestakeholderwillbecontactedonlyintheevent
thattheworkmayimpactoperatingconditionsofthebuilding.Additionalinformationon
PMcanbemadeavailableonrequest.PMworkordersaretypicallycategorizedaspriority3
orpriority4(seetablebelow).
2.2ServiceDeliveryExpectations
FSaimstomeetcustomerexpectationsbyachievingtheperformancelevelsforresponse,workorder
completion,preventivemaintenance,correctivemaintenanceandeffectiveuseoflabor.The
performancelevelsarebasedonUniversityofChicagodata,whereavailable,andareconsistentwith
establishedAPPAlevelsofservice2.

2.3 DisputeResolutionandEscalation
Disputeresolutionandescalationmethodscanbemadewithfollowingsteps.Ateachstepinthe
processbelow,awrittendecisionistobeprovidedfortherecord.Issuesareconsideredresolved
whenaprocessisidentifiedtoresolvetheissueand,wherepossible,canbeavoidedmovingforward.
2.3.1 Level1–ZoneManager
Ifanyworkperformedunderthisagreementdoesnotmeetthestandardssetforthherein,
theaforementionedstakeholdershouldcontacttheZoneManagerforresolution.Ifthe
failuretomeettheestablishedstandardsforperformancedoesnotcriticallyimpactnormal
operations,theissuemaybedeferreduntilthenext,scheduledmeetingwiththeZone
Manager.
2.3.2 Level2–Sr.DirectorofOperations
IftheZoneManagercannotresolvetheissue,theissuecanbeescalatedtotheSr.Director
ofOperations
2.3.3 Level3–AVP,FSOperations
IftheissueisstillnotresolvedtheissueistoberaisedtotheAVP,FSOperations
2.4 FundingandResponsibilities
2.4.1 TroubleCalls
Responsetoatroublecallwillnotbedelayedpendingresolutionoffinancialresponsibility.Thedivisionofresponsibility,however,perSection4.0applies.
2.4.2 WorkOrder(Requested)
RequestedworkthatispartoftheBasicServicesshallbecompletedatnoadditionalcostto
thestakeholder.ShouldtheworknotbepartoftheBasicServices,anaccountnumbershall
beprovidedpriortothecommencementofwork.
2.4.3 WorkOrder(RoutineMaintenance)
Allworkrequiredduetonormalwearandtearwillshallbecompletedatnoadditionalcost
tothestakeholder.
2.4.4 PM
FSiscommittedtocompletingallPMworkwithintheFSbudget.IfFScannotcompleteall
requiredPMwithinthebudget,thestakeholderwillbeadvised.
2.4.5 Billing
Allmaterialsandlaborareloggedoncompletedworkorders.Whencompletedandclosed,
Maximowilldebittheappropriateaccountnumber.Rechargereportscanbemadeavailable
onrequest.
2.5 ServiceHours
FSisstaffed24hours/day,7days/week.Normalbusinesshoursare8:00A.M.–4:30P.M.Monday
throughFriday,excludingholidays.Allworkorderswillbescheduledduringnormalbusinesshours,
unlessotherarrangementshavebeenmade.
