SEMINAR DIARY FROM FELLOW

Michaela Dankova, MD Motol University Hospital

Prague, Czech Republic March 3-9, 2019

March 3, Sunday evening. I started my journey early in the morning and I arrived in Salzburg at 12am, so I could take a look at the city before the beginning of the seminar. The seminar takes place at Schloss Arenberg, a 19th century building with genuine atmosphere. The first meeting was at 7pm., Prof. Trinka and prof. Fink welcomed us and explained the organization of the masterclass. We had the opportunity to greet with colleagues from many countries of Europe, but also from Mexico, Ethiopia and Tanzania. March 4, Monday. We started in the morning with Pre-Seminar test, that was quite a challenge, but introduced us to themes of the lectures and helped us to set to goals for the next week. First lecture by Prof. Sellner lead us through the autoimmune myelopathies and learned us very useful mnemonics how to recognize typical signs of various pathologies. We looked back to the future with prof. Fink talking about mobile stroke units and get the latest update on stroke treatment. Prof. Trinka explained us the new classification of epileptic seizures and epilepsies, that makes communication between neurologists easier and clearer. Finally, Prof. Salman showed us model case presentation. March 5, Tuesday. Tuesday morning was dedicated to emergencies in pregnancy and strokes. Nowadays, more women are at a risk of stroke during the pregnancy and we should be able to deal with it. Dr. Pikija gave us some recommendations how to build and lead a modern stroke unit. Although we cannot change the architecture of our hospitals, we can find other ways, how to improve our care of stroke patients. Prof. Killer-Oberpfalzer showed us all available techniques of endovascular treatment. The afternoon brought the first round of the case presentations, all of them very interesting and instructive. Very exciting were the exotic cases, especially the cases with a good ending. March 6, Wednesday. We discussed the news in etiology, diagnosis and therapy of Multiple sclerosis with Dr. Wipfler, Dr. Chin made for us a perfect overview about nerve conduction studies and peripheral neuropathies, followed by Prof. Sellner with his very practical lecture about optic neuritis. Afternoon was sunny, a perfect time for exploring Salzburg. Many of us visited Hohensalzburg Fortress, admiring beautiful panorama of the city and mountains, Mozart’s Birthplace, or enjoyed a coffee in some of the cozy cafeterias. Finally, we tried the local beer in Augustiner Bräu, the biggest Austrian beer tavern.

March 7, Thursday. It is almost the end of the week, but we continued with devotion with our lectures. Dr. Salman Al Jerdi taught us to assess the comatose patient and explained the principles of neurocritical care. The lectures with Dr. Al Jerdi were actually very entertaining, as he used a lot of examples from real clinical practice and online quizzes, so we could compete with ourselves a little bit. Dr. Sarva gave us an update on genetics and treatment of dystonia, and finally, we learned how to manage an epileptic status – one of the most common emergencies in Neurology. In the afternoon, we listened to the second round of case presentations, again very interesting cases and exciting stories, showing how are we dealing with neurological diseases in our home countries. In the evening, we enjoyed a beautiful musical performance of accio piano trio. We spent the rest of the evening with company of our professors, sharing funny stories from our clinical praxis. The seminar was a great experience. I am thankful that I could attend lectures taught by excellent professors and met colleagues from so many countries. They are an inspiration for me and I hope that we will stay in touch. Michaela Dankova, MD (Czech Republic)

SEMINAR DIARY FROM FELLOW

Laura Mendoza, MD Instituto Nacional de Cancerologia

Mexico City, Mexico March 3-9, 2019

March 2, Saturday evening. I arrived to Salzburg at 6:00 pm from México city. I came with a friend from my hometown who also was selected to take the seminar. We were tired from the trip, but very excited about the experience we were about to have. We loved the view of the city from the plane, with the mountains and the beautiful green areas. We took a taxi from the airport to Schloss Arenberg, it was a 15-minute ride. We were very impressed about the place when we arrived, we found a very lovely building, with a great history behind. A very nice young man received us at the reception and gave us clear instructions about our room and available services at Schloss Arenberg. After leaving our belongings in our nice and very comfortable room, we went to have dinner in a traditional restaurant in downtown. The food was delicious. After that we returned to Schloss Arenberg and went to sleep. March 3, Sunday evening. I went for a walk around the city. Salzburg is a beautiful city with a very rich cultural background, so I spent the morning and part of the afternoon in the Hohensalzburg Fortress, where besides the museum and the historical information, you get to watch a great view of Salzburg, because it is located above the rooftops of the district. At 7:00 o’clock in the afternoon, we had a welcome reception, where Dr. Wolfgang K. Aulitzky (Medical Director of AAF) and Dr. Fink introduced themselves. They talked about the historical background and objectives of the seminar. Then we have a great dinner where we get to talk with our colleagues and start to know each other. March 4, Monday. At 8 o’clock, we took the pre-seminar test. We were explained that we will take again this test at the end of the seminar after hearing all the conferences. The first masterclass was given by Dr. Sellner, it was about autoimmune myelopathy, a very interesting subject for me, as I deal with this kind of patients in my country. The professor explained in a very practical way how to identify radiologic and clinical signs of different myelopathies. I learned things I know it is hard to find in the books and I think I will never forget them. After that Dr. Fink gave us two excellent talks about stroke, he is an excellent teacher and has tremendous experience in the field. The next masterclass was given by Dr. Eugen Trinka, I think this was the best class I have ever received about seizure classification. Something I really loved was the fact that you could ask as many questions as you want and they would give you very professional answers in a very kind way. We took a photo with the faculty outside the building. We had lunch and dinner which were very tasty and where I got to talk with colleagues from all over the world, it is so great to know different cultures and ways to practice medicine. After dinner I went to enjoy the city for a while and then went back to read about what I learned that day.

March 5, Tuesday. In the morning, we kept learning about stroke, it was very interesting to learn how this is managed in another country, this gave me ideas to implement in my country when I get back. The we had coffee break and lunch, during which as a fellow you have the great opportunity to talk in a more informal but nice way with the faculty and again, ask as many questions as you want. In the afternoon the fellow’s cases presentations started. I presented my case and at first I was kind of nervous, but at the end it was a good experience, since you get a lot of opinions about your case and you learn a lot. The cases presented by my colleagues were super interesting, it is very nice to see another kind of diseases and the way they are approached and treated in other parts of the world. Then, since all of us knew each other better, went together to know a little bit more the city of Salzburg and returned on time for dinner. March 6, Wednesday. In the morning we started a new topic. Professor Peter Wipfler taught us about diagnosis of multiple sclerosis and modern treatment. We got an update on the subject, specially about the new treatments available for this disease. I understood the importance of making an accurate diagnosis, since there are many mimics of the disease and the early treatment is very important. After that we got into electro diagnostic studies for peripheral nervous system diseases. I am a neurophysiologist so I loved the topic. We share experiences with professor Russell Chin and also with my colleagues, specially about variants of Guillain Barre Syndrome and its prognosis. In the coffee break, after enjoying a nice coffee and piece of delicious chocolate cake we went back to the Grand Hall and saw a very practical and nice masterclass about optic neuritis, I learned a lot of clinical tips to diagnose patients with this entity. Then professor Dr. Wolfgang Staffen gave us a very interesting talk about Molecular Imaging of Dementia, we learned a lot about new technologies in functional brain imaging, a growing filed that is very useful in this kind of patients. I was happy to see that every day, there are more advances in the subject. After a tasty lunch, we had a free afternoon and I went with my new friends from very different parts of the world to the museum of Mozart, to St. Sebastian cemetery where Paracelsus tomb is located, and to Salzburg museum. Then we bought some souvenirs and went back to Schloss Arenberg to rest. We walked a lot, but it was very interesting to get to know more about the city of Salzburg. March 7, Thursday. In the morning we learned a lot about assessment of the comatose patient and neurocritical care with Dr. Salman Al Jerdi, who by the way is a magnificent teacher. Then we got a very interesting masterclass about dystonia, we got to watch a lot of clinical videos that Dr. Harini Sarva brought from her clinic in New York. Very informing clinical cases. After this, Dr. Trinka gave us an excellent talk about management of status epilepticus, which was great. After dinner, I got to experience some Austrian culture with a chamber music concert in Schloss Arenberg – it was very beautiful. We enjoyed it a lot. March 8, Friday. The last day was full of very interesting topics. Dr. Johann Sellner talked about immune mediated encephalitis, which is a very challenging subject. We kept learning about neuromuscular disorders with Dr. Chin and after coffee break we received a class about EEG, which is one of my favorite topics. Finally, we learned about Parkinson’s disease from Dr. Sarva, and after lunch we got to do the post seminar test. Looking back at the week, I noticed how the time flew, because we had a very nice time in this place. I´m very happy to have found new friends, to have learned so much. I think this was a very good opportunity and I feel very lucky I was selected to attend this seminar. It was a great experience I wish all doctors in my country could have. I hope I will keep in touch with all the people I met and to have the opportunity to attend more seminars like this. I want to thank all the staff in this organization for this great opportunity. I will never forget this experience and all the new knowledge that brought to my medical practice. Thanks for everything. Laura Mendoza, MD (Mexico)
