Date: April 2016 Enclosed, find the preprinted answer sheets for your district’s upcoming Science 4 test. What To Do After The Answer Sheets Arrive:

• Review the answer documents and labels to make sure you have the correct sheets and labels for your school. • Call Bill Adam or Matthew Coleman, Testing Coordinators at 518-862-5310 / 5355, or send an email to

[email protected] immediately if there is a problem with the test materials. What’s New This Year?

• District Return Test Count Sheet: this form must be completed for every exam every time answer documents are sent to NERIC or Regional Scoring Site.

• Unused labels and extra answer sheets – SHOULD NOT BE RETURNED TO NERIC. KEEP THEM IN THE DISTRICT UNTIL NERIC HAS COMPLETED PROCESSING.

• N-24-2 Form for new students – this sheet should be returned to NERIC (Scan to [email protected]). For any writing on answer sheets, use black No. 2 pencil only. Please DO NOT write in, place labels on, or make any marks in the area at the top of the answer sheet where NERIC has preprinted the District Number, Student ID, CD, and School Number.

• Extra Sheets have been included to provide for any student without a preprinted answer sheet.

The Extra Answer Sheets are printed first for each Building/Grade/HR or Building/Grade/Alpha. The student name field is blank and the student number starts with "89''. Please print the student's name, local student ID number, grade, date of birth, and homeroom number

in the shaded area only of the answer sheet (see-attached sample).

• Answer Sheet Class Roster is a list of your students and their student numbers. Extra sheet numbers are listed with a blank field for you to record the student name and ID number of

the student who used each extra sheet.

If you use an “Extra Sheet” please print the student's name, Grade, DOB and actual student ID number on Extra Sheet page 1 in the places provided.

It is not enough to record this information on the answer sheet as noted above; you must also record that information on the Class Roster as well.

Make a copy of the Class Roster for your district CIO and for your records.

• N-24 – For each student using an “Extra Sheet”, please record complete demographic information on the N24-2 form (also attached, and see attached SAMPLE for reference).

Please submit one sheet for each grade level. For each student whose preprinted answer sheet contains incomplete or incorrect information (i.e.

Date of Birth, Homeroom, Local ID Number, etc.), include that student on an N24-2 form. The data you provide on the N24-2 form is used to complete scoring of the test alone.

Send a copy of all completed N24-2 forms to your district CIO to insure that these students are in the data warehouse prior to NERIC scanning the answer sheets. If these students have not been added to the Data Warehouse in BOTH demographics and enrollment, and pushed to Level 1, the assessment data cannot be loaded. These data in the Data Warehouse will be used by NYSED for all statewide reports. IMMEDIATELY following the last day of the make-up period, 6/8/16 or ASAP, give a copy of the following to your district CIO: Preprinted Class Roster with notes, and completed N24-2 Forms. **Be sure to make a copy for your records**.

• Preprinted Labels – Two [2] sets of Preprinted Labels are included with the answer sheets. Labels are to be affixed to the front of Written Test Booklet and the Performance Test Booklet. If using an Extra Sheet print the Extra Sheet number, Student Name, Student ID, District & School Name

on the front of the Written Test Booklet and the Performance Test Booklet. NOTE: There are no Extra Answer Sheet labels.

• Out-of-district students – Your district CSE may have placed students in BOCES or out-of-district settings.

You should immediately forward these students' answer sheets and booklet labels to the BOCES or out-of-district class.

After test administration is complete, the teacher in these settings will return the answer sheets and all booklets directly to the home school.

• Security – After Written Test Part II and Performance Test-Form A are bubbled on Page 2, collect all answer sheets,

placing extra sheets on top. All answer documents are to be locked in a secure area to await shipment to NERIC.

• Regional Scoring – Districts participating in Regional Scoring please follow directions issued by your Regional Scoring Center.

When Written Test Part II is completed, retrieve the Performance Booklet A and the answer sheets from secure storage and match each student's answer sheets with the student's Test Booklets.

Place Test Performance Booklet in the Written Test Booklet face up (you should see the Student Label) and slip the answer sheet inside the back cover of the combined booklets with the tracking marks inside the booklets (see sample Answer Sheets).

DO NOT FOLD the answer sheets. Pack the combined documents for shipment to your Regional Scoring Site and be sure to include all

out-of-district answer sheets. You should bundle the combined documents with adding machine tape, NOT RUBBER BANDS. Also include in that shipment answer sheets with a reason not tested bubble filled in, class roster,

completed N24-2 Form, and completed District Return Test Count Sheet. Please refer to the Packaging Answer Sheet Documents Checklist.

• For district not participating in Regional Scoring – if your district is not participating in Regional Scoring you should send the answer sheets to NERIC Written Test Part II and Performance Test, Form A are bubbled on Page 2.

Please count and return all preprinted and extra answer sheets, include in that shipment answer sheets with a Reason Not Tested bubbled in, class roster, completed N24-2 Form, and completed Return Test Count Sheet.

Please refer to the Packaging Answer Sheet Documents Checklist. Place the District Return Test Count Sheet on top of the bundled answer documents. Place in a sturdy box or large reinforced manila envelope and ship to the address below. Do not ship Test Booklets to NERIC. Ship answer sheets, rosters, N-24s and District Return Count Sheets to:

WHAT TO DO AND WHAT NOT TO DO: IMPORTANT INFORMATION – WHAT ANSWER SHEETS SHOULD BE RETURNED TO NERIC?

1) EVERY PRE-PRINTED ANSWER SHEET MUST BE RETURNED TO NERIC (even if students have a REASON NOT TESTED).

2) NEW STUDENTS WITHOUT A PRE-PRINTED ANSWER SHEET MUST HAVE THEIR EXTRA SHEETS RETURNED TO NERIC (even if students have a REASON NOT TESTED). Example: District A received 50 answer sheets for 50 students. They also have two

new students. The number of answer sheets that should be returned to NERIC are as follows:

50 sheets – doesn’t matter if they were absent/refused. 2 sheets for the two new students – doesn’t matter if they were absent or refused. 52 total sheets returned.

ANSWER SHEETS – DO’s AND DON’Ts 3) DO NOT write in, place labels on, or make any marks in the area at the top of the answer

sheet where NERIC has preprinted the District Number, Student ID, CD, School Number, Level and Form.

4) The Group Code is not used by NERIC, please leave blank.

5) For those students who have IEP/504 test accommodations, the test administrator should bubble in all the testing accommodations used by the student at the time of the test.

6) For those students who have ELL Accommodations, the test administrator should bubble in

all the testing accommodations used by the student at the time of the test.

7) Reason Not Tested – Return all Science 4 answer sheets where a REASON NOT TESTED is bubbled.

8) For those students who are taking the Science 4 in a Translated Edition, bubble Chinese, Haitian Creole or Spanish.

BUBBLING Students and Scorers:

9) Must use No. 2 pencil only. 10) Make heavy black marks that fill the circle completely. 11) Should erase cleanly any answer they may wish to change. 12) Should make no stray marks on the answer sheet.

REASONS NOT TESTED – WHAT THEY MEAN – IMPORTANT

Absent: Students who are absent for any part of the test or the entire test, will be counted as not tested in verification reports and for accountability calculations; whether or not there are any response records.

Absent for entire test: for students who miss all parts of the test and are unable to attend the make-up sessions.

Absent Written Test: for students who miss the Written test and are unable to attend the make-up sessions.

Absent Performance Test: for students who miss the Performance test and are unable to attend the make-up sessions.

Refused to take entire test: for students who refuse to take the entire exam. These students will be considered to have "no valid test score" and will be counted as not tested in verification reports and for accountability calculations.

Administrative error, no score: for a student who is unable to take the exam due to an administrative problem or for students who take the test and it is determined after-the-fact that an administrative error has taken place. You must inform SED and NERIC of any misadministration. Please see Coding of Invalid Tests in the School Administrators Manual. Not enrolled at the time of test bubble must be filled in only for students who:

1. Have moved out of the district prior to test administration. Be sure to inform your CIO.

2. EXCEPTION - Preprinted answer sheet with incorrect grade level - if you receive a preprinted answer sheet for a student but the grade level is incorrect, DO NOT bubble “Not Enrolled at Time of Test.”

3. WHAT TO DO: On the answer sheet draw a line through the wrong grade and write the correct

grade. (DON’T USE THE SHEET FOR TESTING BUT IT NEEDS TO BE RETURNED TO NERIC.)

On the Class Roster next to the student’s preprinted name, draw a line through the wrong grade and write the correct grade.

Use an extra answer sheet of the correct grade level to administer the test. Complete the Extra Answer Sheet Roster and list the student on an N24-2 form. Be

sure to inform your CIO of the error in grade.

who is incapacitated by illness or injury during the test administration, and make-up periods, and has on file documentation from a medical practitioner that the student is too incapacitated to be tested at the school, at home, or in a medical setting.

Medically Excused students, are considered to have no valid test score, and must be reported with a final score of 999 and a standard met code of 93.

NOTE: These students are excluded from the numerator and the denominator of the participation and performance accountability calculations.

Taking NYSAA: for a student who will be taking the NYS Alternate Assessment in lieu of the ELA Exam.
