SC3 Changes to individuals or premises
The law says you must write and let us know about certain changes within social care establishments
and agencies and organisations that run these.1 You can use this form to tell us about:
changes to any individual (except the manager) who is part of the registered provider. This
includes the director, secretary, clerk, trustee, treasurer or other similar officer of the organisation (organisations only)
changes to the responsible individual (organisations only)
changes to the manager of a voluntary adoption agency, local authority adoption service or local authority fostering service
the absence of a registered manager from an adoption support agency, children’s home, independent fostering agency or residential family centre
changes to your registered premises or when you acquire additional premises.

About this form
Section A – Details of social care provider: this section must be completed.
Section B – Notice of change, individuals who are leaving: use this section to tell us
about any individuals who are leaving.
Section C – Notice of change, individuals who are joining: use this section to tell us
about individuals who are joining.
Section D – Notice of change, new responsible individual/children’s home
director2: please complete this section if you are providing the details of a new responsible
individual or children’s home director in section C.
Section E – Notice of absence, registered manager: use this section to tell us about
the absence of a manager.
Section F – Changes to premises: use this section to tell us about changes to your
registered premises or when you have acquired additional premises.
Section G – Agreement: this section must be completed.
Section H – Additional information: use this section to record the Unique Reference
Numbers (URNs) of any additional establishments or agencies this form relates to and if
you need additional space.
There are additional guidance notes below for each section. This form must be signed by the current
responsible individual or member of the registered provider. We cannot accept a notice of a new responsible individual from the incoming responsible individual.
Section A − Details of social care provider
(A1–A5) We need to know about the registered provider or local authority service the information in this form relates to. If the changes apply to a particular setting we also need to
know the setting details.
Section B – Notice of change, individuals who are leaving
(B1–B3) This section asks for information about individuals who are leaving (this includes
anyone who is leaving one role to take up another role within the organisation, for example from
treasurer to secretary). Where the registered provider is an organisation we need to know about any individuals who hold office within the organisation, for example the director, secretary, clerk,
trustee or treasurer. If the responsible individual is leaving you must provide us with the name of the new responsible individual, and we expect you to do this within 14 days of the person’s
leaving date.3
If the manager is leaving an adoption support agency, children’s home, independent fostering
agency, residential family centre or residential holiday scheme for disabled children, he or she

must apply to voluntarily cancel their registration as a manager using our form Application for cancellation of registration (social care provision).4
When a new manager starts work at an adoption support agency, children’s home, independent fostering agency, residential family centre or residential holiday scheme for disabled children we
must be notified. The new manager must apply for registration with us by completing form SC2.
We expect the application to be made within 28 days of the date of employment. Please refer to our Guide to registration for children’s social care services (www.gov.uk/government/publications/guide-to-registration-for-childrens-social-care-services ) for timescales by which you must tell us about a manager leaving and when a new manager should
be appointed and registered with us. Please use section H if you need more space.

Section C – Notice of change, individuals who are joining
(C1–C2) This section asks for information about individuals who are joining (this includes anyone
who has moved internally to take up a new role within the organisation, for example from chairperson to secretary). Where the registered provider is an organisation we need to know
about any individuals who hold office within the organisation, for example the director, manager,
secretary, clerk, trustee or treasurer.
If you have told us about a new responsible individual or a new children’s home director you must
also complete section D.
Please do not use this form to tell us about a new manager for an adoption support agency, a
children’s home, an independent fostering agency, a holiday scheme for disabled children or a residential family centre. New managers for these establishments and agencies must apply for
registration with us by completing form SC2. We expect the new manager to apply for
registration within 28 days of the date of their employment. Please refer to our Changes to children’s social care services that are registered and/or inspected by Ofsted for more information
about the registration of a manager in an already registered establishment or agency.
Please use section H if you need more space.
Section D – New responsible individual / children’s home director
(D1) This section must be completed if you have told us about a new responsible individual or a new children’s home director in section C. A responsible individual and a children’s home director
must meet certain requirements.5 You can find more information in the relevant regulations and in our publications: Guide to registration for children’s social care services and Changes to children’s social care services that are registered and/or inspected by Ofsted.

Section E – Notice of absence, registered manager8
This section does not apply to residential holiday schemes for disabled children.
(E1) The law states that where the registered manager of an adoption support agency, a
children’s home, an independent fostering agency or residential family centre proposes to be absent for a continuous period of 28 days or more, written notice must be given to us. We must
be notified no later than one month before the proposed absence starts (or within one week of the absence occurring where there is an emergency).
(E2) Certain information must also be provided about the proposed absence. If at this time you
do not know who will manage the establishment or agency during the absence you must provide details about how you will appoint someone and by what date.
(E3) You must provide details of the individual who will manage the establishment or agency during the absence. If you have not appointed someone at this time you must tell us when you
have.
You must also tell us when the registered manager has returned. You must do this no later than
seven days after the date of return.

Section F – Changes to the premises
This section does not apply to residential holiday schemes for disabled children.
If you want to register a new voluntary adoption agency branch, please do not use
this form; you must complete form SC1.
(F1–F9) This information will help us decide if any further action needs to be taken.
(F2) Please tell us whether you have obtained planning permission, if this is required. You will need to provide written verification that the local authority, where the setting is located, has
confirmed to you that planning permission is not necessary, or that you have the necessary
planning permission/s.
You should also ensure that any building work complies with the Building Regulations. Ofsted may at any time ask that you provide evidence that the local authority is satisfied that the building
work meets the Building Regulations.
(F6) If you intend to change the establishment or agency’s statement of purpose, please send us
a copy of the amended statement.
(F7) You can use this form to tell us about changes to the premises but if you would also like to change the conditions of registration, for example a children’s home wishes to provide care and
accommodation for more children, you must apply to vary or remove the conditions of registration. To do this please contact us on 0300 123 1231.

Section G – Agreement
(G1) This form must be signed by the current responsible individual or member of the registered
provider. Where the registered provider is an organisation this could be the director, manager,
secretary, clerk, trustee, treasurer or other similar officer of the organisation. We cannot accept a notification of a new responsible individual from the incoming responsible individual.
We are not required to carry out any checks on the managers of local authority fostering services, local authority adoption services or voluntary adoption agencies.
We ask for confirmation that the legal entity that is registered with us has not changed. If it has
changed you will need to contact us as you may need to apply for a new registration.
G Agreement G1 I agree that Ofsted may at any time carry out checks on the responsible individual or a member of
the registered provider. By signing this form I confirm I have informed all relevant persons that Ofsted may at any time carry out checks on their suitability to provide a registered service under the
Care Standards Act 2000 including their suitability to work with children.
