Instructions:
1. Fill up the blanks and print this document in LONG BOND paper. EXCLUDE this part of the document and only print the form.
Everything MUST be computerized, EXCEPT the Signature of the class president.
2. ONLY blanks with asterisks (*) must be filled up.3. Please DON’T change the setting of the form. Ex. Fonts, Font size, etc.4. If the number of rows exceed the total number of students in your
class, please kindly ERASE the excess before printing the document or vice versa.
5. ATTACH your Registration Card (using paper clip) along with your payment.
6. Please PUT your payment in a proper small envelope, handmade or not.
7. Please be reminded that we will NOT be entertaining individual payees. If there are irregular students in your class, please be kind and INCLUDE them in your section.
8. Please be reminded that the COLLECTION of payment for first years is on June 15, 2015 while the higher years is on June 22, 2015. Payments will be UNTIL 5:00pm.
9. Please REMIT your payments in the SC Office thru SC Treasurer.
