To provide the tools and supports that family physicians need to enhance their ability to deliver primary and secondary stroke prevention programs and improve the quality of care for post-acute stroke patients

Objective of Collaborative Stroke Care

Expected Outcomes

Creation of formal linkages between practising family physicians and their peers

Provision of quick access to neurologist colleagues in caring for patients with cardiovascular illness of the brain

Enhancement of outcomes in primary and secondary stroke prevention
