Het ECW biedt wondzorg aan alle cliënten met complexe wonden.
Voordat een cliënt door het ECW in wondbehandeling kan worden genomen dient er een verwijzing te zijn van een specialist, huisarts of SO.
Er wordt een indicatie wondzorg geregeld.
Met de verwijzer zal door het ECW overleg plaatsvinden betreffende de wondzorg van de cliënt.

Methods
Design: case report study, approved by medical ethical committee and with patient
informed consent, conducted in two nursing homes and one ambulatory wound centre in
The Netherlands. Twenty-two non-malnourished patients (i.e. with a normal BMI and no
signs of undesired weight loss) with a pressure ulcer, leg ulcer or diabetic foot ulcer
existing for at least three weeks were included. All participants were offered one to three
servings per day of a specific ready to drink arginine-enriched ONS (per serving: 200 ml,
250 kcal, 20 g protein, 3 g L-arginine, 250 mg vitamin C, 38 mg vitamin E (α-TE), 9 mg
zinc, 1.5 mg carotenoids) in addition to their regular diet and standard wound care, for a
maximum of 12 weeks. Besides patient characteristics, information on wound size (cm2),
product intake, patients’ product appreciation, and photographs were collected every
month.
Conclusion
Extra nutritional support with a specific ready to drink arginine-enriched ONS has already
proven to be effective in the healing of PUs (van Anholt 2010), and seems to be beneficial
for the healing of other types of chronic ulcers as well. Therefore, it is advisable to perform
more in-depth research on this nutritional intervention in patients with different types of
chronic wounds

Objective
Although many wounds will heal without delay or complications, both acute and chronic
wounds may be affected by intrinsic factors e.g. comorbidities and inadequate blood
supply and/or extrinsic factors e.g. infection and insufficient nutrition (Vowden et al
2008). Chronic wounds are more complicated because the biology of the healing
process has significantly changed (Demling 2009).
In the last decade, wound research has led to new wound healing interventions but
delayed wound healing is still a major burden for health care in general and patients in
particular. Nutrition is a relevant mediator for skin regeneration. Adequate nutritional
support therefore plays a pivotal role in wound healing. Based on the results of a
previous randomized controlled trial in patients with pressure ulcers (PUs) (van Anholt
2010), a series of cases has been conducted to explore the effect of a specific arginine-
enriched oral nutritional supplement (ONS) in patients with PUs, leg ulcers (LUs) or
diabetic foot ulcers (DFUs). The primary outcome parameters were: 1) wound healing
progress, i.e. wound size reduction, 2) patients’ concordance and appreciation of the
specific ONS.

De nieuwe categorie decubitus ‘depth unknown’ wordt opgesplitst in
‘suspected deep tissue injury’ en ‘unstageable’. Deze categorie is van
toegevoegde waarde omdat het verloop afwijkt van de andere categorieen
decubitus. Daarentegen zijn de interventies en inzet van preventieve
maatregelen er ook op gericht om diepere weefselschade te voorkomen.
Het toevoegen van deze categorie in de nieuwe internationale richtlijn uit 2014 is de aanleiding geweest om
een artikel te schrijven ter verdieping van de nieuwe categorie decubitus:
‘depth unknown’. De verdieping van onbekende diepte.
