INSTRUCTIONS - SUPPLEMENT SETUP SYSTEM
All supplements for extra duties/responsibilities entered through the SUPPLEMENT SETUPSYSTEM default to the effective dates below. For payroll purposes the employee’s starting dateis used and not the first day students report to school.
AUGUST 22 for Returning Teachers Paid full 212 or 260 duty days
AUGUST 15 for New Teachers Paid full 217 or 256 duty days
AUGUST 22 for Paraprofessionals Paid full 212 or 260 duty days
AUGUST 19 for New Paraprofessionals Paid full 215 duty days

It is imperative that these dates be used in order to receive full payment (or for full seasonif an athletic supplement). Otherwise supplements will be pro-rated based on the effectivedate of the supplement indicating actual assumption of extra duties. Those employees in anExtended Year School will be made effective according to the calendar approved by the Board.
The Supplement Setup System cannot be utilized for the following situations:
! if it is questionable whether or not an employee will perform thesupplemental duty
! part-time employees must be processed by memorandum (see forms inElectronic Salary Supplement Handbook http://pers.dadeschools.net)
! supplements that are not effective the first day of the schoolyear(excludes Senior High Athletic Coaches)
! Part-time coaches are processed by the School Athletics andActivities Division and special instructions were given at theAthletic Directors meeting held in August. These employeesreceive a lump sum payment after completing their specificcoaching assignment.
! Supplements requiring an approved waiver must be submitted on theSupplement Waiver Request Form, FM-4509 Rev. (09-94), This formmust be used when requesting the following:
< Extra Teaching Period Supplements (ETPS) which do not meetcontractual provisions
< A teacher is to receive two Extra Teaching PeriodSupplements. The first ETPS should be submitted throughthe AUTOMATED SUPPLEMENT SETUP SYSTEM.
< A teacher is to receive more than three extra duty salarysupplements. The first three supplements should besubmitted through the AUTOMATED SUPPLEMENT SETUPSYSTEM process. ETPS are exempt from the three-supplement limit.

INSTRUCTIONS - SUPPLEMENT SETUP SYSTEM
Schools will input supplements Tuesday, September 10 through Friday, September 20. Supplementrequests will be reviewed for approval by the supplement approving offices from September 23through October 4. The first pay date for approved supplements will be October 18th for allemployees. Retroactive salary adjustments will be paid according to the Payroll Department.
All supplements are assigned and approved annually by the respective principal and/orsupervisor and entitlement to such supplements must be determined for each school yearor at such other time as may be specified by the Chief Personnel Officer for PersonnelManagement and Services.
The maximum number of supplements any one employee may receive is three (excludingextra teaching period salary supplements). Under extraordinary circumstances, exceptions maybe granted only by the respective Region Superintendent and Deputy Superintendent for SchoolOperations and must be submitted on the Supplement Waiver Request form. SUPPLEMENT WAIVERFORMS ARE NOT REQUIRED FOR EXTRA TEACHING PERIOD SUPPLEMENTS REQUESTEDAFTER OCTOBER 31, 2002.
There is no column advancement for the 2002-2003 school year. Supplement column placementwill be automated and schools will not be required to input column placement except forMiddle School Intramural (code 650) and Disabled Sports Coaching (code 803)supplements. (See Guidelines for Column Placement.)
EXTRA TEACHING PERIOD SUPPLEMENTS (ETPS)
Eligibility:Teachers who teach an extra period shall be paid additional compensation equal to their daily rate ona pro-rata basis of one-eighth of the annual salary, excluding supplements. The followingrequirements must be met for an extra teaching period supplement to be authorized:
1. Secondary instructional staff members who teach an extra teaching periodduring the regular work day shall not be required to work beyond 7 hours and20 minutes. In order to provide students with greater course options, secondaryinstructional staff members who elect to teach an extra period beyond theregular work day shall receive a duty-free planning period.
2. Extra teaching period supplements for district-funded enhancement programsshall be authorized only for instructional purposes (i.e., courses which generatecredit as determined by the State Course Code Directory).
3. To become eligible to receive an extra teaching period supplement, secondaryteachers must be currently teaching five instructional periods and elect to teacha sixth instructional period. Exceptions may be granted only by the respectiveRegion Superintendent and the appropriate Deputy Superintendent.
4. Elementary teachers who teach an extra period in a secondary school shall beeligible for the supplement, subject to meeting the above requirements.

Comments required:
All requests for ETPS must indicate the total number of classes taught and thespecific course title taught. Information will be verified by reviewing theteacher’s workload schedule in the Integrated Student Information System(ISIS). Please make sure master schedules are updated in ISIS prior tosubmitting requests for ETPS as those teachers not displaying 6 classes in ISISwill be denied.
Alternative Method:
Teachers working beyond the regular school day that are not eligible for an Extra Teaching PeriodSupplement, may be eligible for compensation on the “AT” salary schedule. Compensation is basedon degree held and total number of hours previously taught. (The number of hours taught can befound on screen (16) of application (09) in the Personnel Reporting System (PERS)). Teachers paidhourly should be setup on job code 1802 on the ‘B’ payroll.

Title I Language Arts EXTRA TEACHING PERIOD SUPPLEMENTS (ETPS)
The following supplement code is used for schools, authorized by Grants Administration, using theirTitle I allocation to cover the cost of the extra period supplements for personnel providing instructionalservices to eligible students.

Home Language Assistance Program (HLAP) for Secondary Schools
Full-time teachers providing tutorial services in HLAP, in addition to their regular teaching assignment,are eligible for extra teaching period supplements (ETPS). A supplement waiver request, program6624, Home Language Assistance Program (HLAP) for Secondary Schools is required and is availablefrom the Division of Bilingual Education and World Languages. Schools may input the supplement afterthe waiver is approved.

ALTERNATIVE SUPPLEMENT MODEL AND EDUCATION EXCELLENCE COUNCILS (EEC)SUPPLEMENTS
A percentage (up to 15 percent) of a school’s total supplement allocation and/or entitlement (not toinclude extra teaching period supplements) may be utilized exclusively at the discretion of the principalto establish supplemented activities he/she deems necessary to meet the school’s unique operationaland programmatic needs. Schools may obtain their supplement allocation online – please referto page 25. Supplements established in this regard shall be made part of the school’s annual plan.Additionally, such supplements shall not exceed the value of an extra teaching period supplement forthe employee in question.
Individual school sites implementing the alternative supplement model shall have their individual planssubmitted annually (no later than March of each year) to the Planning/Oversight Committee for reviewand recommendation to the Superintendent and UTD Executive Vice President and approval by theBoard.
Alternative Supplement Model and EEC Supplements for the 2002-2003 school year wereapproval by the Board August 21, 2002 A listing of the designated supplement codes forauthorized schools will be posted on the Electronic Salary Supplement Handbook(http://pers.dadeschools.net) after the supplement setup process and schools will inputthese supplements through the supplement RPA screen beginning October 14, 2002.

INTERDISCIPLINARY TEAM LEADER AND TEACHER AS ADVISOR SUPPLEMENTS (MIDDLESCHOOLS ONLY)
Eligibility:
In order to qualify for the Teacher as Advisor Supplement, the school must have in existence a full-scale school-wide advisor-advisee program.
In order to qualify for the Interdisciplinary Team Leader supplement, the following criteria must be met:
! The team leader must be the leader of an interdisciplinary team which consistsof at least three members.
! The team must have a majority of the students they teach in common, and havea common team planning time.
! The team leader is responsible for the coordination of a program consisting ofacademics, common learning skills and personal development.
! There must be proof of existence of interdisciplinary teaming, i.e., master schedule,master seat inventory, Teachers in Teams (MAC school) etc. in ISIS.
Special consideration:
Approval is required by the appropriate Region Office, based upon submission of proof of existence ofa school-wide advisory program.
EXCEPTIONAL STUDENT EDUCATION
Supplements for Teachers, Counselors and Paraprofessionals of the EH, PH, TMH and/or PhysicallyImpaired supplement codes 804 and 801, will be programmatically assigned to teachers, counselors(object code 5144 or 5116) and paraprofessionals (object code 5145) in programs 6710, 6740, 6760,and 6821 during the period September 9, 2002 through October 4, 2002.
A special screen(OPTION #2) is available in the AUTOMATED SUPPLEMENT SETUP SYSTEMto display employees that will be programmatically assigned these supplements. Schoolsshould review this screen and notify Wage and Salary of any employees listed in error due to wrongprogram number and also place the employee into the correct program number. Failure to do so willresult in an overpayment for the employee.
Schools must input ESE supplements for those eligible employees hired or reassigned exceptionalstudent education classes or in program 3301 after October 4, 2002 through the Electronic SupplementRPA, beginning October 14, 2002 - see page 26 for instructions.
DEPARTMENT/SUBJECT/GRADE CHAIRPERSON (ELEMENTARY SCHOOLS ONLY)
Allocation:
One Department/Subject/Grade Level Chairperson shall be provided for each ten (10) teacher unitsallocated per school, including art, music, physical education, bilingual education and exceptionalstudent education units. Every elementary school shall have a minimum of three (3) departmentchairpersons.

DEPARTMENT HEAD (SECONDARY SCHOOLS)
Eligibility:
Department Heads must supervise a minimum of three (3) full-time teachers (not support staff,paraprofessionals, office employees, or other non-teaching job positions) to receive the smalldepartment head supplement (code 590), or supervise a minimum of six (6) full-time teachers toreceive the large department head supplement (code 570). School principals must submit the nameof the department upon entering the supplement. In addition, school principals must maintain a rosterof teachers under the supervision of the department head upon request by the Athletics and ActivitiesDepartment.
SECONDARY PROGRAM SPECIALISTS
Beginning with the 2001-2002 school year, Secondary Program Specialists can only receive theSecondary Program Specialist supplement (code 807). Department Head responsibilities must beassigned to someone other than the Secondary Program Specialist.
INTRAMURAL/ACTIVITIES SPONSOR (ELEMENTARY SCHOOLS ONLY)
Eligibility:
The intramural before or after school play supplement is for the specific purpose of offering physicaleducation or sports activities before or after school. The Elementary School Activities Sponsor (ESAS)supplement should be utilized for activities such as computer, debate, drama, music and other clubactivities.
Allocation:
Supplements will be allocated at the same level as the physical education teacher allocation. If a schoolis allocated two physical education teachers, it will be entitled to two full supplements of a combinationof intramural and activities supplements. In the case of fractional physical education allocations, thesupplement allocation will be rounded up to the next whole number. Each Intramural and/or ActivitiesSponsor supplement assigned is rounded against the school's intramural allocation.
Special consideration:A full-time intramural supplement will be interpreted to be five hours of work per week for the entireschool year. A fractional intramural supplement would reflect the following time commitment:

ACTIVITIES (SECONDARY SCHOOLS ONLY)
Middle schools are allocated a maximum of five (5) Club Sponsors and two (2) Academic ActivitiesSponsors. Schools must use code 470 for the 500 Role Model Sponsor and not utilize a Club Sponsor.Class Sponsors (i.e., Class of 2000) in the Senior High Schools should use a Senior High Service ClubSponsor (code 471) and not a Club Sponsor. In addition, the National Honor Society Sponsor at theSenior High level should also be paid as an Senior High Service Club Sponsor (code 471).
Comments required:
Senior High Service Club Sponsor, Club Sponsor, Performing Group and Department Headsupplements require that the name of the club/department be designated.
Special consideration:
For supplements such as Newspaper and Debate, extracurricular activities are necessary in order forthe teacher to be eligible for the appropriate supplement. All clubs and organizations approved by theprincipal to operate within the school for which the principal is responsible must comply withadministrative regulations.
The charter and constitutions setting forth the purposes, qualification for membership, and the rulesof conduct of each approved club or organization, and list of student officers shall be kept on file by theschool and always be available to all students and instructional personnel. Principals should review andbe familiar with Board Rule 6Gx13-5C-.042, Organizations–Clubs and Student Activity Groups.

DISABLED SPORTS COACHING SUPPLEMENTS
Eligibility:
Coaching supplements for personnel working with the Disabled Sports Coaching program shall be paidto such individuals based upon the disabilities and seasons coached. A maximum of three incrementsshall be paid to an individual and each school is limited to five supplements. The following incrementsare available:

Persons receiving one supplement are listed as code 8031. Those receiving two supplementsare listed as 8032 and three supplements as 8033. Please note there is no columnadvancement for coaching experience in the ESE Sports Program.

The specific disability (ies) and season, as highlighted above in bold, must be indicated, in addition tothe column number.

MIDDLE SCHOOL SPORTS PROGRAM
Eligibility:
Coaching and intramural supplements for personnel working in the Middle School Sports Program shallbe paid to such individuals, except that a maximum of four coaching supplements shall be paid to anindividual, providing that there is no overlap of sport seasons (Refer to the Middle Schools AthleticCalendar).
Paraprofessional I’s or Security Monitor I’s may receive supplement if: (1) has a valid coachingcertificate, (2) there are no instructional employees interested in the supplement(s). At the discretionof the principal, the combination of coaching and intramural supplements are established as follows:
! Twelve $380 intramural supplements shall be allocated to each middle school, if needed.Each supplement shall be for a six-week period of time. Intramural supplementsmay not be paid for a "try-out time" for coaching a sport (see next paragraph).
! Coaching supplements shall be $670 per sport, with the exception of bowling, since thebowling season will be a minimum of three weeks in length. The bowling supplementshall be $330. All other sports with coaches receiving supplements shall be a minimumof six weeks in duration. A coach shall not receive more than one supplement per sport,and a coach shall not coach more than two teams in the same sport.
! Middle schools shall have no more than 16.5 coaching supplements allocated as follows:
< Sports entitled to two supplements per school, male and femaleteams (one each):

Special consideration:
Part-time coaches cannot be submitted through the Supplement Setup System or electronic supplementRPA. Supplements for coaches that are active part-time M-DCPS employees or substitute teachersshould be requested by memorandum (Part-Time Assistant Coach Employment) and submitted toSchool Athletics and Activities.
Coaching supplements, for those employees working in one location and coaching at a differentlocation, must have the Alternate location number entered upon entry of supplement into theAutomated Supplement Setup System. Schools will not be charged for those supplements performedat other schools. Supplements will not be approved if the Alternate location number is not entered.
The positions of Athletic Director, Business Manager, and Athletic Trainer are distinct positions.Bargaining unit members shall not be authorized to hold any more than one of these positions duringthe same school year. The Athletic Director, Business Manager, and Athletic Trainer may not coach anysport.
Except where agreed to by the Board and the Union, full-time and/or part-time non-instructional employees may not serve as coaches; however, non-instructional employeeswho obtain a coaching endorsement may be eligible to serve as a coach.. Additionally, saidnon-instructional employees are not eligible for regular supplements specified in theMDCPS/UTD contract, except upon agreement of the parties.
All Athletic supplements and provisions of the M-DCPS/UTD Contract, Appendix E, are in compliancewith Title IX regulations. If is necessary for one person to coach both boys and girls, when theallocation table calls for separate coaches and the seasons run concurrently, the coach shall be paidthe regular supplement for the one assignment. Such assignments shall be requested by the principaland approved by the administrative Director for School Athletics and Activities.
Allocation
Eight assistant coach (discretionary) supplements are to be paid in accordance with the appropriatesport. Use football codes only when the discretionary supplement is used for football. All discretionaryassistant coaching supplements are to be paid the amount listed under that supplement, with theexception of second semester football. The specific sport must be indicated for discretionarysupplements in the comments section.

ATHLETIC TRAINERS
This supplement is valid for classroom teachers who carry a full teaching schedule and performadditional duty as Athletic Trainer. Full-time Athletic Trainers (job code 0981) are not authorized toreceive this supplement as this is their primary job responsibility.
TELEVISION PRODUCTION TEACHERS
Eligibility:
In order to qualify for the Television Production Teacher Supplement, the teacher must meet thefollowing criteria:
! Teacher must be full-time, teach a minimum of one period of television production (asdisplayed in ISIS), be responsible for the television production studio, and sponsordocumented student activities in television production beyond the regular school day.
! One supplement available per eligible secondary school; school must have a televisionproduction studio.
! Schools with large TV production programs that utilize more than one TV productionteacher must secure an approved supplement waiver form or the supplement will notbe paid.

EXTRA-CURRICULAR SALARY SUPPLEMENT ALLOCATIONS
Extracurricular and activity supplement allocations will be based on the formula as outlined in the2002-2003 School Allocation Plan K-12 schools. A review of the actual number and cost ofsupplements authorized will be conducted monthly using the online supplement allocation system.Please refer to page 25 for instructions in using this application. The account of schools for which costof actual supplements exceeds the entitled allocations will be adjusted proportionately by unusedallocations (if any) of schools that do not exceed their entitlement.
Elementary Schools: $4,274 for each elementary school plus $5.72 per unweighted FTE
*Middle Schools: $40,227 for each middle school plus $7.28 per unweighted FTE
*Senior High Schools: $113,630 for each senior high school plus $3.64 per unweighted FTE
*Note: Additional supplements may be purchased from the school’s -02 discretionaryaccount, except those supplements that are controlled in number by contractualallocation.
These supplement allocations are based primarily on athletic and activity supplements. Supplementallocations for special schools, such as the Alternative Schools, Magnet only Senior High Schools andExceptional Education Centers, will be established on an individual basis as designated in the SchoolAllocation Plan. Supplements listed below will not be counted against a school's supplemententitlement. Schools are not automatically authorized or eligible for all of these supplements.
! Teacher or Counselor of the Emotionally, Profoundly, Physically Impaired, or TrainableMentally Handicapped
! Paraprofessional/School Support Personnel Working Exclusively with the Emotionally,Profoundly, Physically Impaired or Trainable Mentally Handicapped

GUIDELINES FOR COLUMN PLACEMENT: REGULAR AND ATHLETIC SUPPLEMENTS
1. Each employee who is presently receiving a supplement will remain on the same supplementcolumn for the 2002-2003 school year. Employees will retain supplement column experienceregardless of the M-DCPS location he/she is employed, so long as the employee earned acreditable year of supplement service and the supplement duty was in the same supplementfamily. (A supplement family is comprised of similar types of supplements as shown on the nextpage.)
! An employee changing or adding supplements within the same supplement family andmoving from a higher supplement to a lower supplement will retain all supplementexperience and be placed on the same column in the lower supplement.
! An employee changing or adding supplements within the same supplement family andmoving from a lower supplement to a higher supplement will start on the column of thehigher supplement which is greater than the previous supplement amount.
2. If an employee earns a creditable year of supplement service but then does not perform thesupplement service for a period of time, he/she shall be placed on the same column whenresuming that supplement or a supplement within the same family.
3. If an employee does not earn a creditable year of supplement service, the employee will startat the same column when he/she resumes that supplement or a supplement within the samesupplement family.
4. All employees hired outside of M-DCPS, regardless of experience, will start at column one.
5. Supplements such as the Substitute Locator, Lead Teacher, Television Production, Role Model,Middle School Sports, and Extra Teaching Period do not have supplement columns. Thesupplement column designation, therefore, is 0 (zero).
6. Any concerns or request for waivers regarding creditable service and appropriate columnplacement shall be resolved through a joint decision by the employee's supervisingadministrator and the appropriate supplement-approving office.
7. Please note that supplement column placement is automated for the 2002-2003 school year. Schools will not be able to change column assignments;however, Wage and Salary upon written request will correct column placementwhere necessary. Examples include, employees who performed supplement duty priorto 1990 or switched supplements within a supplement family.

HIGHLIGHTED CHANGES FOR THE 2002-2003 SCHOOL YEAR
The following changes have been implemented to further facilitate the data entry ofsupplements:
! There is no column advancement for the 2002-2003 school year.
! Schools may view their supplement allocation online and track supplementcategorical expenditures each month which also includes part-timeemployees and part-time contracted coaches. The online screen alsodisplays which supplements are excluded and not counted against’s theschools overall allocation. This information is updated monthly.
! Schools can view online those supplements not approved beginning October7, 2002. The reason and approving office responsible for that supplementare displayed next to the supplement title. The approving office phonedirectory may be obtained by pressing the PF-1 key.
! The screen will only display supplements for those employees at your schoolwho performed extra duty activities for one semester or more. Theseemployees only require action code ‘C’ to continue supplement for the 2002-2003 school year.
! The supplement RPA screen in the Personnel Reporting System (PERS) willautomatically generate column placement except for supplement codes 650 (MiddleSchool Intramural) and 803 (Disabled Sports Coaching)
! Schools may view a listing of salary supplements assigned to staff online beginningin November. Information on this application will be sent to school principals underseparate memorandum.
! Supplement Waivers are not required for Extra Teaching Period Supplements(ETPS) requested after October 31, 2002.

SUPPLEMENT ALLOCATIONS
Supplement allocations are calculated each October using the school’s FTE from the ISIS file. The topof the screen reflects the formula amounts used to calculate the ALLOCATION TOTAL. Each monththe personnel database will be tabulated to determine supplement expenditures by category. Part-timeemployees submitted on the part-time employee supplement request form are reflected after approvaland are encumbered against the school’s allocation since these employees are not paid until the endof the school year.
Those schools that contract coaching supplements and submit payment through the pink part-timeemployee hired as a contracted coach form will see those payments reflected after paychecks havebeen issued to the part-time employee. Those contracted coaches submitted through a purchase orderare not reflected in this category and must manually be counted against the school’s overall allocation.
The TOTAL SUPPLEMENT FOR YEAR and ALLOCATION DIFFERENCE are monthly snapshots ofthe schools expenditures. An average expenditure report will be calculated in May to determine actualexpenditures for the school year.
Schools that are participating in the Alternative Supplement Model program must ensure that thiscategory does not exceed 15% of the ALLOCATION TOTAL.

INSTRUCTIONS FOR THE ELECTRONIC RPA SUPPLEMENT
Supplements requested after the September 20 deadline, and for the remainder of the school year,must be submitted on the Supplement RPA Screen beginning on October 14, 2002. The RPAscreen is accessible through the Personnel Reporting System (PERS), Application "8" - PersonnelActions (Electronic RPA), and Option "6" - Supplement Shift. The Supplement Setup System(SUPP) will be unavailable after Friday, September 20, 2002 to input supplements.
Signing onto the Personnel Reporting System (PERS):
To access the Supplement/Shift screen, do the following:
1. Enter your employee number, work location, personal password, and PERS(Application) on the sign-on screen. Press <ENTER> key.
2. Select Application "8" (Personnel Actions). Press <ENTER> key.
