Occurrence Reporting categories
I. Aircraft Accidents / Serious Incidents (according ICAO Annex 13)
II. Other Accidents and Incidents (according Swiss Law)
- Incidents linked to fueling and deicing procedures- System / infrastructure failures- Wrong informations- Turnaround incidents- Injured personell, dangerous goods, FOD…
III. Safety Enhancement Informations: nothing happened… (Lessons learned for other employees, occurrences which almosthappened, failure of a specific airport system)

Letter Airline Operator to Unique and FOCA, 23.8.07
In reference to the incident occurred on the 17th of February 2007, in Zurich airport, allow us firstly to apologize for the delay in giving an answer, due to an administrative error.
As soon as we received the notification of the incident, we took actions in order to avoid repeating such incident. We included a company NOTAM, which we describe below:
“Do not request Taxi Clearance until Push-back manoeuvre has been completed and clear signal has been granted.”

Incident indicators (an example)Near miss on the apron (Cat. H10)•Cat I: a collision was near and did not happen just by coincidence or ended with the abrupt stopping of either the aircraft or the vehicle•Cat II: near miss on apron without abrupt stopping maneuver (the minimum safety distance was not maintained [200m in front, 100m behind an aircraft])•Cat III: a near miss on apron between two aircrafts•Cat IV: Push-Back incidents

In a Nutshell
• Comply with ICAO Standards and Recommended Practises (SARPs) and actively manage non-compliances
• Document and manage safety relevant procedures• Collect, analyse accidents, incidents and any other safety relevant
information (provided by employees)• Encourage a just reporting culture (Just Culture)• Define and communicate the meaning of „Safety First“ • Analyse operational hazards on a regular basis• Provide safety oversight of contracting companies by audits and
inspektionen• Establish a suitable safety organisation• Communicate Safety Informationen• Engage in (international) working groups (FOCA, ICAO, ACI, FSF)
