Map Box Lifting Lug
1st October 2015 | Issue Number 47
Within the past two days there have been two separate site incidents involving our Map Box Lifting Lug. In both cases and upon lifting operations the Lugs have failed along the welds between the lifting eye to box section corner post insert (see pictures below). Therefore with immediate effect all Map Box Lifting Lugs (Map.Lug) must be removed from hire fleet and quarantined until further instruction. Although it is prohibited to send any further Lugs on hire both Operational and Technical management will advise a suitable alternative should we need to fulfil any hires. Longer term, further directives will follow. According to G42 some 163 lugs are currently on hire. Where practicable to do so, Desk, Ops and Sales functions must swiftly collaborate to arrange their inspection and if necessary replacement with a suitable alternative (again as advised).
For further assistance please consult your Regional Operations Manager. This bulletin is mandatory. Ensure you understand what is required, communicate to your staff, and display for future reference. Failure to comply with this bulletin may result in disciplinary action being taken.
