Your membership is important to us. Primary membership for you and one dog is $25; each additional dog is $10. Each

member will receive the combination to the gate at the Park. Membership runs from May 1 to April 30. The Park may

temporarily close in the Spring due to mud and/or inclement conditions that may be deemed hazardous to owners and

dogs. The membership fee is reduced after November 1st to $10 for the first dog and $5 for each additional dog. NO

REFUNDS for overpayment.

READ THE BACK OF THIS FORM, THEN READ AND CHECK ALL BOXES BELOW BEFORE SIGNING! I understand that such activity may expose me and/or dog(s) under my control to the possibility of physical injury and/or property damage and

that the Waste Management of New Hampshire Inc. and Rochester Dog Owners’ Group, Ltd. makes no representation or warranty with respect

to the potential dangers, whether foreseen or unforeseen, associated with such activity while in or on the premises. I hereby for myself and

anyone else who might claim on my behalf, promise not to sue; and waive, release, and discharge Rochester Dog Owners’ Group, Ltd. and

Waste Management of New Hampshire Inc., and all other affiliated companies, directors, officers, employees, and agents, for any and all claims

or liability of any kind arising out of the activity described above, including but not limited to injury to me, dog(s) under my control, my

property or any other claim arising out of said activity, even though those claims or that liability may arise out of negligence or carelessness on

the part of persons or parties named in this Waiver and Release.

I understand that my membership will be canceled if I provide the lock combination to any non-member.

I accept all the risks associated with engaging in the activities described above, and the possibility of personal injury and/or property damage or loss resulting there-from.

I have read and agree to abide by all membership rules on the reverse side of this form. I also agree to follow all Rochester NH Dog

Park rules and policies. I have read and fully understand this waiver and release. In signing this form I acknowledge my acceptance.

Rochester NH Dog Park Rules

Users of this facility do so at their own risk. Neither Waste Management, Inc. of NH nor the Rochester

Dog Owners’ Group, Ltd. shall be liable for any injury or damage caused by any dog, human, act of

nature or accident incurred by the use of this Park. This Park is for members and their guests only.

Any other use is considered trespass.

Members are liable for damage or injury inflicted by themselves, their guests or their dog(s).

Glass containers and human food are not permitted within the fenced areas.

Members must clean up after themselves and their pets. Specifically, this means scoop the poop and

dispose of properly, and fill in any holes dug by their dog(s).

Dog food, treats, chews or bones are prohibited.

Smoking, alcohol and drug use are strictly forbidden.

Dogs must be licensed, vaccinated and free from communicable illness and parasites.

Without special permission, there is a limit of two dogs per member per visit.

Members must be 16 years of age or older to properly handle a dog at the Park.

Dogs must be leashed while outside the fenced-in area. Please unleash your dog as you enter the free play

area. This is for the safety of your dog.

No professional trainer, walker or expert shall conduct business at the Park except by invitation of the Board

of Directors. This Park is NOT a training facility for anyone, including members.

Dogs must be under the control of and in view of the member at all times.

The Small Dog Area is for dogs 25 pounds or less. If your dog is over 25 pounds, you must get the

permission of those members currently utilizing that area before entering. As a courtesy, you MUST

remove your dog to the appropriate size area if requested to do so.

Aggressive dogs must be removed from the Park immediately.

Leaving dogs unattended is prohibited. Animal control will be contacted.

Puppies under four months of age are prohibited.

No female dogs, while “in heat,” are allowed.

Choke, prong, pinch, shock and spike collars MUST BE REMOVED inside the Park area.

Failure to abide by these rules may result in a dog or member/handler being banned from

the Park. Please be a courteous and considerate pet owner. These rules are for the safety

and protection of you and your dog(s).

REMEMBER: This is a dog playground. Use of this Park is a privilege, not a right.
