Botryoid odontogenic cyst is a rare multilocular variant oflateral periodontal cysts. Man, 55, complained of numbness andpain on palpation in the lip and chin area. Six years previouslythe patient was diagnosed with botryoid odontogenic cyst.Clinical examination revealed intact oral mucosa, no signs ofswelling, and response to cold when tooth pulps were tested.Panoramic radiography revealed a multilocular radiolucencyinvolving the anteroposterior jaw. An excisional biopsy wasdone, with the microscopic examination showing multiplecystic spaces lined by nonkeratinized cuboidal squamousepithelium consisting of a few layers, focal epithelial plaques,and clear vacuolated cells. The pathologic condition was iden-tified as botryoid odontogenic cyst. Another radiograph taken 3years later indicated recurrence and confirmed the histopatho-logical diagnosis. Size and multilocular pattern probably arethe main factors associated with recurrence. Long-term follow-up of patients diagnosed with botryoid odontogenic cyst isnecessary.
PE-291 - RED LED AND LASER PHOTOBIO-MODULATION IN THE PREVENTIONANDTREATMENTOF ORAL MUCOSITIS: EXPERIMENTAL STUDY INHAMSTERS. MARIA DO ROSÁRIO SANTOS FREIRE,RAFAEL FREITAS, FÁBIO COLOMBO, ALBERTOVALENÇA, APARECIDA MARIA CORDEIRO MARQUES,VIVIANE ALMEIDA SARMENTO. UNIVERSIDADE FED-ERAL DA BAHIA.
Animal research evaluated laser (660-nm) and LED(670-nm) irradiation and oral mucositis (OM) development afterchemotherapy (Che) with 5-fluorouracil. In the preventivegroups, photobiomodulation started 1 day before Che and wasgiven every 48 hours (Ia, IIa, Ib, IIb). For therapeutic groups(IIIa, IIIb, IVa, IVb), irradiation started the fourth day after Che.Animals were sacrificed 7 or 14 days after d(0). In the positivecontrol group, hamsters were submitted to Che without photo-biomodulation and sacrificed at 7 (Va) or 14 days (V). In thenegative control group, no procedure was done and animals weresacrificed at 7 or 14 days (Vc and Vd, respectively). Clinicalchanges and healing of OM occurred to varying degrees afterphotobiomodulation. Histologically, inflammation, predomi-nately with lymphoplasmocytes, which tended to diminish withtime; epithelial atrophy; hyperemia; fibroblast proliferation; andangiogenesis were observed. The best results were obtained inthe preventive laser and LED groups.

The study of dental dermatological diseases has presentedthe oral manifestations of these disorders that represent an in-tegral part or a prodromal sign of disease. In this contextpemphigus vulgaris is an important component. This study de-scribes oral lesions in patients with pemphigus vulgaris,emphasizing location, clinical features of the lesions, and thesocioeconomic profile of patients. This case study was con-ducted from August to October 2011 and followed the routineoutpatient dermatology iMIP. The women selected, ages 43 and65 years, had oral manifestations of the pemphigus vulgaris.This disease affects the oral cavity often as the initial site ofmanifestation. It is critical that dentists understand this diseaseand be able to make a diagnosis early and use that knowledge toguide treatment. Multidisciplinary care is an importantapproach.

Adenoid cystic carcinoma (ACC) is a rare malignant glan-dular tumor that usually affects women and occurs in the fifth andsixth decades of life. The palate is most commonly involvedintraorally. This report presents two ACC cases with distinctclinical presentations. White woman, 20, reported swelling on theright side of the hard palate measuring 2.5 cm in diameter. It wasslightly bluish, smooth, fibrous, and painful on palpation and hadbeen present for 1 year. Asian woman, 42, presented an asymp-tomatic ulcerated fibrous nodule on the left side of the palatemeasuring 3 cm in diameter, with no color change. Both patientswere referred by dentists, and in each the diagnosis of ACC wasachieved through incisional biopsies and histopathologicalstudies. Both patients were referred to an oncology facility andunderwent surgery and adjuvant radiotherapy. The surgical defectwas restored prosthetically, and patients are being followed upperiodically.

Treatment for B-cell non-Hodgkin’s lymphoma is com-bined therapy with cyclophosphamide, vincristine, doxoru-bicin, and prednisone. The monoclonal antibody rituximabwas later added (R-CHOP). Recently, osteonecrosis of thejaws caused by monoclonal antibodies has been observed.Man, 39, positive for human immunodeficiency virus (HIV)infection, had B-cell non-Hodgkin lymphoma. After 6 ses-sions of R-CHOP therapy, he complained of lesions in theinterdental papilla of the jaws. These were initially diagnosedas necrotizing ulcerative periodontitis but reclassified asosteonecrosis based on the observed lack of bleeding andnecrotic appearance. Ciprofloxacin and low-power laser ther-apy were initiated. After 5 weeks, the area had totallyre-epithelialized. Monoclonal antibodies are an excellentprotocol for malignant tumors, but antiangiogenic effects andneutropenia may increase the risk of jaw osteonecrosis.Patients should therefore be closely supervised by the dentist

Salivary duct cyst is an uncommon lesion of the minorsalivary glands that is rare in the upper lip. Man, 78, had a2-cm � 1-cm painless lump on the upper lip with color similarto mucosa and unknown duration of development. An exci-sional biopsy was performed, followed by histopathologicalevaluation. Microscopically, a dilated salivary gland ductcontaining eosinophilic material was noted, consistent with amucinous epithelial lining with two to three layers of cuboidalcells and salivary gland mucosal acini showing mild atrophy.This was consistent with the diagnosis of salivary duct cyst.After 1 year of follow-up, no signs of recurrence have devel-oped. This case emphasizes the importance of dentistsincluding this condition in the differential diagnosis of lesionsin the upper lip, even though this condition is relatively rarefor this anatomical site.

Schwannoma (neurilemmoma) is a slow-growing benigntumor of the nerve sheath derived from Schwann cells. Approx-imately 25% to 48% of all schwannomas are seen in the head andneck region. In rare cases, these lesions may affect the oral cavity.They are usually asymptomatic, do not recur, and rarely undergomalignant transformation. Intraorally, the tongue is the mostcommon site, followed by the palate and floor of the mouth. Man,66, had an asymptomatic nodular lesion on the tip of tongue forabout 4 years. The main clinical proposed diagnoses were benignmesenchymal tumors and salivary gland tumors. An excisionalbiopsy was performed and the histopathological analysis led to adiagnosis of schwannoma. The patient recovered well and is infollow-up with no abnormalities.

Schwannoma is an encapsulated benign neural tumor thatmainly affects the tongue of adult patients. Boy, 7, was referredfor evaluation of a soft, painless, slow-growing nodule in theupper lip lasting 6 months and measuring 1.5 � 1.5 cm at thegreatest diameter. The patient underwent an excisional biopsy.
The microscopic features included a circumscribed proliferationof spindle cells representing Verocay bodies and composed ofpalisading nuclei adjacent to spaces filled with eosinophilic fila-ments (Antoni A pattern) and hypocellular areas against a myxoidbackground (Antoni B pattern). The tumor cells were stronglypositive for S-100 protein; perineurial cells were positive forepithelial membrane antigen. The definitive diagnosis wasschwannoma. The patient has been clinically followed for 1 yearwith no recurrences. Clinicians should consider schwannoma inthe clinical differential diagnosis of upper lip nodules in pediatricpatients.

Sjögren’s syndrome is a complex, chronic, systemic,autoimmune disease that mainly affects the exocrine glands,especially the salivary and lacrimal glands, leading to drynessof the oral and ocular mucosae. Woman, 54, reported oraldryness and bitter taste in the mouth. The diagnosis of hypo-salivation prompted an investigation for Sjögren’s syndrome.Using the revised international classification criteria for Sjög-ren’s syndrome, there were positive findings for ocular symp-toms, oral symptoms, ocular signs, and involvement of thesalivary glands. Negative findings were present for microscopicanalyses of focal lymphocytic sialodenitis in the minor salivaryglands and for anti Ro(SS-A) and anti La(SS-B), which cor-roborates secondary Sjögren’s syndrome. These findings weredetected because of an oral complaint. The patient was thenreferred to a rheumatologist to investigate other potential dis-eases, such as rheumatoid arthritis and systemic lupus eryth-ematosus.

Oral mucositis usually occurs as a result of chemotherapy forpatients with oncohematologic diseases. Young man, 16, wasreferred to a dental office with painful lesions on the lips andcomplaints of dysgeusia, dysphagia, and trismus. The patient hadbeen diagnosed with acute lymphoblastic leukemia, B-cell line-age, and was undergoing chemotherapy with MADIT. Oroscopyrevealed multiple bleeding, ulcerated lesions in the upper andlower lip and in palate, leading to a diagnosis of a grade IVmucositis, based on the World Health Organization (WHO)classification. The oral lesions were treated with cryotherapy andAd-muc, which has anti-inflammatory and antibacterial actionincluded in its healing formula. Cleaning the mucosa with salinesolution and chlorhexidine gluconate was also done to improveoral hygiene. After a week the patient showed a mild decrease inthe mucositis, but he died 8 weeks later from an exacerbation ofhis underlying disease.
