Tick all hazards that apply

Access Animals/Insects Audience/Public Cables Communication Failure Confined Spaces Costume/Makeup Derelict buildings/Dangerous

Structures Electricity Fire/Flammable Material First Aid Food Hygiene Glass Heights Hostile/Remote environment Machinery Lighting Lone working Night Operation Noise

Hazard: Animals/Insects

What could cause harm?

The public use this area to walk their dogs so could bite if not on a lead or trained properly

The likelihood of this happening?

It is very unlikely as most owners have control of their animals

How much harm?

If bitten it could cause serious harm including: infection or rabies

Apply the following priorities, how will the risk be managed?

(1) Eliminate (2) Reduce or Substitute (3) Isolate (4) Control (5)Personal Protective Equipment

Hazard: Cables

What could cause harm?

If we use an extension cable to get electricity to our lighting; band and crewmembers could trip over

The likelihood of this happening?

This is very unlikely as there are few electricity points to use when on the fields

How much harm?

If someone did trip over; their injuries could vary from minor scratches to broken limbs

Apply the following priorities, how will the risk be managed?

(1) Eliminate (2) Reduce or Substitute (3) Isolate (4) Control (5)Personal Protective Equipment

Hazard: Audience / Public

What could cause harm?

Some people in the public can be unpredictable and find what we are doing offensive so could try to harm the band or crew

The likelihood of this happening?

This is very unlikely

How much harm?

If harmed it could vary in what the person has done

Apply the following priorities, how will the risk be managed?

(1) Eliminate (2) Reduce or Substitute (3) Isolate (4) Control (5)Personal Protective Equipment

Hazard: Structures

What could cause harm?

We are using a bandstand in the park. It is structurally safe however there are stone steps up to it. So if someone fell while filming or transporting equipment up it they could get hurt

The likelihood of this happening?

There is a medium chance of it happening

How much harm?

If someone fell it could be serious with injuries such as: broken limbs and dislocation to minor grazers and band equipment breaking

Apply the following priorities, how will the risk be managed?

(3) Eliminate (2) Reduce or Substitute (3) Isolate (4) Control (5)Personal Protective Equipment

Hazard: Lighting

What could cause harm?

If we do use studio lights on set they will get hot so band members and crew could touch them and burn themselves. Plus, they could fall over and break.

The likelihood of this happening?

There is a low chance of this happening

How much harm?

If the lights did fall it could give someone a bad burn and possibly a major graze

Apply the following priorities, how will the risk be managed?

(2) Eliminate (2) Reduce or Substitute (3) Isolate (4) Control (5)Personal Protective Equipment
