Amendment 1:
This amendment is raised to change some of the Mandatory Evaluation Criteria (Attachment 2 of the RFSA), and as a result, change one of the articles of Part 6B - Bid Solicitation. Summary of changes:
(a) The number of cases required with the arrangement for each stream and category changed from three (3) to one (1).
(b) Possession of first-aid, confined space, fall arrest, WHMIS and category 1 diving certificates for the applicable stream and/or category are still mandatory but no longer required with the arrangement.

The RFSA is amended as follows:
Change #1: Delete the Attachment 2 – Mandatory Evaluation Criteria included in the original RFSA in its entirety and replace with the attached Revision 1.
Change #2: Delete B6.3.2 Bid – Service Request Form Part 2, Box 49 to 52 – Name, Function, Company and Email in its entirety and replace with the following:

The Bidder must provide the name, function, company name if applicable and email addresses of all proposed individuals for the Work (either on or off the vessel). All proposed individuals other than Category 8 (Dive Survey) must have the following valid certificates:
(a) First-aid (b) Confined Space (c) Fall Arrest (d) WHMIS
Proposed individuals for Category 8 (Dive Survey) must meet the minimum competency requirements of the CSA Z275.4-02 and possess the following valid certificate:
(a) Category 1 Diving Certificate
Note: All proposed individuals (employees and subcontractors) must be pre-qualified by Canada. If the Bidder would like to propose a substitute that was not pre-approved, the Bidder must contact the Supply Arrangement Authority to request the pre-qualification and approval of the individual prior to submitting a bid. The Bidder must allow at least 72 hours to the Supply Arrangement Authority to complete the pre-qualification. The Bidder remains responsible to provide its bid by the bid solicitation closing date (Box 44).
It is not mandatory to submit the certificates of the proposed individuals to be pre-qualified but they could be requested by Canada before contract award or when the Work is being completed. If requested, the Bidder must provide the valid certificate(s) within 48 hours of the request.
