Overview

• Donor: Government of Alberta― Human Services― Alignment with Municipal Affairs capital grants

• Funds available: $1,000,000/year• Contract Period: June1, 2015 - March 31,

2016• Program Components:

― Permanent Supportive Housing― 30 program spaces― High acuity individuals ― Master Lease with CHF

Target Population

• Prioritization through Coordinated Access and Assessment (CAA)

• Individuals over 18 years of age• Chronic and Episodic Homelessness• High Acuity SPDAT Score• No Exclusionary Criteria• Existence of complex co-occurring conditions

RFP Eligibility

• Two contractual components: ― Master Lease Agreement ― Program Funding Agreement

• Strong financial/governance policies & processes • Use Homeless Management Information System to

report outcomes • Adhere to CHF Case Management Standards• Participate in Coordinated Access and Assessment

Project Advisory Committee

• Six member panel• Up to three (3) members external to CHF• Up to three (3) CHF staff• Three part-process

Proposal Preparation

• A checklist of required proposal documents to be submitted is included in Schedule A of each RFP

• Ensure all Mandatory Requirements are met• Proposal packages should be submitted 3 hole

punched for consistency, containing all information and responses to all sections.

• Do not use the agency name anywhere other than the front page of the proposal submission.

Proposal Preparation

• Submit eight (8) hard copies and one (1) electronic copy of your proposal. Schedule B to be in excel format (not pdf)

• 1 USB key with all documents in electronic format• Proposals must be submitted in a sealed

package, the outside of the sealed package should be prominently marked with the RFP title and reference number, the full legal name and return address of the Proponent and the Proposal Submission Deadline date and time.
