1 DISCLAIMER

This induction booklet has been prepared by Carlisle Homes.

The intent of this induction booklet is to provide non-exhaustive information regarding statutory obligations and various laws that govern safe building practices. This in no way exonerates Employers, their employees, contractors, agents or other persons of their personal responsibilities to ensure compliance with the governing requirements. Each Employer, employee, contractor or other person must comply with their own obligations under relevant legislation at all times.

The induction booklet is designed to provide a greater level of knowledge to Employers, their employees, contractors and agents so that they can better identify hazards and risks that may prevent incidents, injury and illness to themselves and others that may be affected by their undertakings when working on Carlisle Homes sites.

Carlisle Homes and its related entities do not accept any liability resulting from any act or inaction taken in reliance upon or in connection with this bookle

2 INTRODUCTION

By signing this induction booklet, you acknowledge you have read and understood the contents of the booklet and agree to comply with all of the requirements set out in the booklet (so far as they are reasonably practicable), and in all relevant OH&S Acts, Regulations, Codes of Practice and Guidance Material. You also acknowledge that you will only undertake work that you have the relevant expertise, qualifications and knowledge to undertake safely and that you have been engaged by Carlisle Homes because you have that specialist expertise, qualifications and knowledge.

The induction booklet is intended to assist all duty holders in meeting their shared OH&S responsibilities,

including Carlisle Homes and the people engaged by it to undertake the required works i.e. Contractors/Suppliers and their employees, contractors, agents or other persons. A copy of the ESP can be found on site in the electrical meter box.

· Comply with all relevant legislation, regulations, and codes of practice/compliance and industry standards

· Provide workers with the appropriate guidance, resources and support to enable them to carry out their work safely

· Develop a culture that encourages all employees to participate in the safety process and to raise safety issues if they arise

· Monitor and record safety performance and improvement

3 SITE SPECIFIC HAZARDS AND RISKS

The induction booklet is intended to provide general information regarding OH&S responsibilities and Carlisle Homes’ safe systems of work. All duty holders are required to identify, assess and control any hazards and risks to health or safety that are specific to a particular site or circumstances.

Carlisle Homes will provide its Contractors/Suppliers with information it has to assist it to identify, assess and control any hazards and risks to health or safety that are specific to a particular site or circumstances prior to that Contractor/ Supplier commencing works.

Any site specific hazards identified by the Contractor/Supplier are to be reported to Carlisle Homes immediately. If the hazard is of a serious nature that could significantly affect the health or safety of any site personnel whatsoever, those persons must cease works immediately, cordon off the area (if safe to do so) and alert Carlisle Homes immediately.

4 INDUCTION BOOKLET PROCESS

Carlisle Homes representative inducts Contractor/Supplier into induction booklet prior to commencing

works on site

ÈThe Employer must sign the

“Commitment & Acknowledgment form”.

This form is returned to the Carlisle Homes office and added to the preferred Trade/Supplier database

ÈThe Employer must induct their employees,

contractors & any other persons into the Induction booklet prior to commencing works on site and provide documented evidence of this to Carlisle

Homes. All necessary risk assessments and SWMS MUST be completed.

HAS ALL BEEN COMPLETED CAN

WORK CAN START ON SITE

ÈThe trade or supplier must then induct any

other employees attending a Carlisle Homes site and provide documented evidence of this to Carlisle Homes (a sign off register must be

maintained which is attached).

Note: This form shall be made available to Carlisle Homes via request within a 24hr period as required.

5 PYRAMID SUBCONTRACTING

Where a subcontract is let by one contracting party to another, any subsequent re-letting would constitute pyramid subcontracting and would contravene Carlisle Homes policy.

Pyramid subcontracting works is strictly forbidden unless without prior written authorisation from Carlisle Homes.

6 INFORMATION, INSTRUCTION, TRAINING AND SUPERVISION

Prior to the commencement of work, ALL personnel undertaking work of any kind on Carlisle Homes’ sites must have been provided the necessary information, instruction and training regarding safety. At a minimum, this will include Construction Industry Training (Construction industry induction card) and training regarding High Risk Construction Work, with appropriate records maintained and provided to Carlisle Homes.

Each Contractor / Supplier is responsible for ensuring their employees, contractors and others have received the necessary information, instruction and training to enable them to undertake their job safely. All information, instruction and training must be provided by appropriately qualified persons.

All workers undergoing work on site are to provide evidence of trade and task competency, to provide all necessary paperwork relating to hazardous substances they may be confronted with, and the correct selection, fitting and training of personal protective equipment (PPE).

At all times, the level of supervision necessary to enable workers to undertake their job safely must be provided by appropriately qualified persons. Apprentices or other workers with less experience are not permitted to be left on site UNATTENDED whilst they are performing construction work.

7 ONSITE MINIMUM QUALIFICTION REQUIREMENTS

The following qualifications are mandatory for all workers undertaking work of any kind on Carlisle Homes’ site:

· Construction Industry induction card

· Training regarding Safe Work Method Statement (SWMS) for High Risk works

· Appropriate licences for plant and equipment

First aid training is recommended for all personnel. At a minimum, Contractors and Suppliers must ensure appropriate levels of personnel with first aid training and appropriate first aid equipment are available at all times.

Evidence of the above qualifications must be provided to Carlisle Homes prior to commencing work.

8 CONSULTATION

A safe workplace is more easily achieved when everyone involved in the work communicates with each other to identify hazards and risks, talks about any health and safety concerns and works together to find solutions. The site supervisors may use the onsite visits, phone, email or text to discuss safety conditions or requirements. Contractors or Suppliers must:

· Call a Carlisle Homes representative if there is a safety concern

· Communicate with other persons on site regarding OHS conditions or requirements

9 HEALTH AND SAFETY CO-ORDINATION PLAN

Responsibilities

Every person has an obligation to take reasonable care for their own safety and the safety of others. However, the following table sets out an overview of the key areas of responsibility regarding health and safety.

10 RISK MANAGEMENT

Work through the following list to determine the best safety measures:

Hazard: Anything in the workplace that has the potential to harm people.

Risk: The possibility that the harm (eg death, injury or illness) may occur in the workplace when exposed to a hazard.

1. Eliminate the unsafe work practice, plant or equipment.

2. Substitute the work practice, plant or equipment with a safer alternative;

3. Isolate the work practice, plant or equipment; or

4. Re-design the workplace, plant or equipment to make it safer.

5. Change Work Methods change the way the work is done to make it safer

6. Provide Protective Equipment give workers appropriate safety equipment or clothing.

When working on a residential construction site you must identify any potential hazards assicuated with the proposed work, assess the risks involved and develop controls to eliminate, or minimise, the risk. The risk management process must be carried out in consultation with your employees, must be undertaken by appropriate qualified persons and documented as set out below.

You must not commence construction work unless:

· All employees working on Carlisle Homes sites have undertaken and signed off on a full Carlisle Homes’ induction.

· An assessment of the risks associated with the work has been undertaken and provided to the principal contractor in the form of a documented risk assessment and SWMS.

· A site specific risk assessment has been undertaken before undertaking work on site.

· All employees and contractors have been trained in and understand the requirements of the risk assessment and SWMS.

If site conditions change, if the scope of work changes, if the personnel undertaking the work changes, if the plant or equipment to be used to undertake the work changes or if an incident occurs, then the SWMS must be reviewed and, if necessary, revised.

11 OHS NON -COMPLIANCE

All contractors and suppliers on site or in the workplace must comply with any health and safety rules and procedures. Failure to completely follow any instruction related to health and safety procedures will result in the following disciplinary steps:

1. “Three strikes”. This involved verbal discussion, Written warning, off site re-induction.

2. “First and final”. This involves a written final warning and off site re-induction.

3. “Instant dismissal”. Works to cease and no further work will be provided.

12 HIGH RISK CONSTRUCTION

You must maintain a SWMS for any High Risk work undertaken on the project and provide a copy to Carlisle Homes before commencing work. Carlisle Homes will provide the principal contractor with a copy of the SWMS, including any changes made to the SWMS.

In addition to the above general requirements, if you are doing any of the following tasks on a Carlisle Homes site, then a detailed SWMS is required:

· Where there is a risk of a person falling more than 2 metres.

· Work involving demolition

· Removal or disturbance of asbestos

· Structural alterations that require temporary support

· Confined space

· Trench or shaft if excavated depth is greater than 1.5 metres

· Near mains pressure gas

· Energised electrical installations or services

· Adjacent to roadways or railway

· Movement of mobile plant

· Extremes of temperature

These are only examples of high risk work which would require a SWMS and a detailed SWMS may be required in other circumstances not

included in the above list.

13 SAFE WORK METHOD STATEMENTS (SWMS)

A Safe Work Method Statement must:

· Identify work that is High Risk Construction work being done

· State the hazard arising from that work

· Describe how the risk will be controlled

· Describe the manner in which the risk control measures are to be implemented

· Be reviewed by an appropriately qualified person

All work must be undertaken in compliance with the SWMS.

The SWMS must be updated where necessary (but only by an appropriately qualified person). It may be necessary to update the SWMS if risk controls are inappropriate, if site conditions change, if the scope of work changes, if the personnel undertaking the work changes, if the plant or equipment to be used to undertake the work changes or if an incident occurs.

The completion of a detailed Safe work method statement (SWMS) is a legal requirement for all high risk work and may be required in other circumstances.

14 SITE HOUSEKEEPING/ SITE ACCESS

· All trades are to clean up their own rubbish on a daily basis

· All windblown rubbish is to be immediately placed into the site rubbish bin/cage

· Stockpiled items (Eg. Bricks, Roof tiles, Plaster) to be placed as directed

· Site to be left “Trade Ready” for the next contractors

· Bend nails over in redundant timber

· Need to maintain clear access to the front gate, doors, garage, toilet and water tap.

· Only Carlisle Homes authorised/inducted personnel and delivery drivers are allowed to access site

· Failure to comply with site housekeeping requirements may result in cleaning recharges to help maintain compliance

15 FENCING

· Carlisle Homes lock code is 4777

· Do not tamper with temporary fencing

· Gates and fence panels to be closed when leaving site

· Gates to be opened inwards (not over footpaths)

· Failure to comply with fencing requirements may result in cleaning recharges to help maintain compliance

· Maintain silt protection barrier when applicable

· Notify the site supervisor of any damage to the fencing

16 SITE AMENTIES

The toilets are there for your use. Please do not tamper, or abuse.

· Toilet must be supplied in working order

· Must not be placed on sloping ground

· Must not be used to store building materials

· Keep the area in front of the toilet clear.

· Have a door that is hand lockable

· Please let the site supervisor immediately know of any issues

· Make every effort to flush toilet

17 SITE SIGNAGE

All site signage that is displayed should be carefully read and understood as it may be displaying specific advice relating to site condition. Carlisle Homes’ site supervisors use these signs to convey important site information.

Never cover, remove or wilfully damage signs that are displayed on site, as they are positioned for a safety reason. Contact your site supervisor if a sign requires attention.

18 PUBLIC SAFETY

It is everyone’s responsibility to ensure that members of the public are protected from the activities that occur on the building site. You are responsible for conducting a site specific hazard identification process regarding public safety before commencing work. However, the following general rules apply to all Carlisle Homes’ sites:

· Cover trenches or holes, uneven surfaces

· Remove all objects in public areas

· Barricade all works area adjacent to footpaths or roads

· Daily housekeeping which includes sweeping footpaths and roadways

· Appropriate signage

· Don’t block footpaths with cars or equipment

· Do not leave material on neighbouring property including waffle pod fix down reo

· Implement any additional site security as required (based on risk assessment)

· Ensure crossover protection (if in place) is returned to its correct position when leaving site.

19 GENERAL HAZARDS

You are responsible for ensuring a site specific hazard identification process has been undertaken before commencing work. However, the following general rules apply to all Carlisle Homes’ sites:

· All star picks or stakes need to be capped and identified

· All sharp objects are to be identified and covered

· All cutting tools must use their own dust prevention apparatus

· All equipment and tools must be appropriately maintained by qualified persons and used in the correct manner as recommended by the manufacturer’s guidelines and / or Australian Standards

· Prior to use of any equipment and tools, a pre-start check must be completed

· Nail guns must be used in conjunction with an applicable SWMS, including the requirements to keep hands, arms and legs well clear when firing

· Guarding must be maintained and kept in place on all tools in accordance with manufacturer’s guidelines and relevant Australian Standards

· Temporary timber propping of verandas or alfresco areas is prohibited in all circumstances. The final post must be used.

20 ELECTRICAL EQUIPMENT

ALWAYS

· Test and tag electric leads, tools and equipment every 3 months.

· Visually inspect electric leads, tools and equipment for damage before each use.

· Use safety switches (also called RCDs) when using electrical tools and equipment.

· Use a competent person to repair damaged electrical leads, tools and equipment.

· Elevate electrical leads from ground with the use of lead stands, insulated hook devices.

· Ensure your generator is tested and tagged and not used in a confined space

NEVER

· Use damaged electrical leads, tools and equipment.

· Use electrical leads, tools and equipment in damp or wet conditions.

· Place electrical leads in areas where they may be damaged (e.g. on the ground, through doorways and over sharp edges).

· Use modified electrical tools and equipment.

· Use domestic power outlets that are unsuitable for industry.

· Use extended electric leads beyond Industry Standards (piggy backing).

· Use double adaptors.

· Run power leads from neighbouring dwellings or across roads

21 LADDERS AND STEPLADDER

Portable ladders are one of the least stable but most commonly used tools for working at height. Portable ladders require users to be more vigilant about the risk of falling when working at heights. Before using a ladder, consider if there is a safer way of completing the task.

If a ladder is required to be used, the following requirements must be followed:

· Carlisle Homes considers Work platform ladders to be a higher control level when work at height

· Ladders must be used in accordance with the requirements of Australian Standard AS/NZS 1892: “Portable ladders”.

· Ladders are to be positioned on a stable footing, to extend 900 mm above the step off point

· Angled appropriately, 4 in 1

· They are to be secured to prevent unsafe movement.

· Whilst ascending and descending the ladder, both hands must be freely available for gripping purposes

· If tools are necessary to carry out the work, they should be restricted to light weight hand tools

· Electric or high-powered equipment should NOT be used to perform the task from portable stepladders that are not designed for the operation

· Never overreach. Move ladder to the appropriate position

· Stepladders should always be used in a fully opened position with stabilizing bars locked.

· Person’s feet should not extend NO HIGHER than the third tread from the top.

· Ensure no one works underneath the ladder

· Never use stepladders adjacent an open void such as window, doorway or stairwell.

The inability to comply with any of the above requirements would indicate that stepladders are inappropriate and a alternate methods of working at height is recommended.

22 SCAFFOLDING

All modular scaffolding MUST be installed to conform to Australian Standard AS/NZS 4576: “Guidelines for scaffolding”.

All scaffolding installed to a working height of 4 metres must be installed by a licensed Scaffolder, with a Handover Certificate issued and an identification label (SCAFFTAG) to be displayed on the scaffold to ensure full compliance.

Any necessary modifications that are required to be made to the scaffold to enable work to be performed should be carried out by a licensed or competent person with the scatuires extensive modifications, the Site Manager should be advised immediately. After modifications are performed or the structural integrity of the scaffold has been compromised, the scaffold should be inspected by a licensed Scaffolder and any concerns rectified immediately.

Scaffolding should be inspected at intervals not exceeding 30 days, with issues documented and rectified. The components of the scaffolding system are all necessary to provide the scaffold with the structural integrity and safety required and should not be tampered with and left in an unsafe condition.

ALWAYS

· Have scaffold installed to comply with Australian Standards.

· Have scaffold altered only by a licensed or competent person.

· Have scaffold installed with appropriate ladder or stair access.

· Carry out regular visual inspections and document concerns.

· Advise your Site Supervisor of scaffolding concerns, i.e. platform height or length incorrect for intended work.

· Have appropriate mechanical aids and equipment necessary to load materials onto the scaffold.

· Provide sufficient protection from falling debris to public and trade.

· Leave scaffold “Trade Ready” for next person Eg No loose concrete, etc

NEVER

· Remove scaffolding components to leave the scaffold unsafe, which includes handrails, toe boards, braces etc

· Manually throw mortar or bricks above shoulder height onto scaffolding.

· Discard debris or unused materials from scaffolding with persons below.

23 BRICKLAYER TRESTLES

All trestles must be commercially manufactured and set up on a sound base, fully planked and not overloaded. Fall hazard must not be more than 2 metres in height.

All scaffold platforms over two metres must be fitted with a guardrail, mid-rail and toe-board / kicker. Bricklayers are not to alter scaffolding. Scaffolding suppler must alter and resign off on scaffold tag.

Safe access to trestles must be available.

24 ROOF AND FALL PROTECTION

Physical fall protection railing is installed to control the fall hazard where roof and floor construction work is performed. The fall protection railing is installed by an accredited installation company to conform to appropriate Industry Standards.

· All heights above 2 metres must have fall protection in accordance with Codes of Practice/Compliance

25 VOID PROTECTION

Void platforms are installed to cover openings such as stair voids, veranda porticoes and swimming pool cavities. They are an integral control measure for fall hazards. They are installed to comply with Australian Standard AS/NZS 1576.1: “Scaffolding - General requirements”. The installation of the platform consists of an access opening to allow safe access via an Industrial grade ladder, the opening access hatch must be kept closed at all times unless being accessed. The protection platforms MUST at all-time be kept in a safe workable condition. If for construction reasons the platform requires modification, contact your site manager for advice.

26 MOBILE PLANT & POWERED EQUIPMENT

You are responsible for conducting plant and equipment specific hazard identification, risk assessment processes and safe work method statements before commencing work using mobile plant and powered equipment. However, the following general rules apply to the use of mobile plant and powered equipment on all Carlisle Homes’ sites:

· Contractors must complete pre-start safety checks on their mobile plant and equipment daily, before use. The pre-start checks must be documented and provided to Carlisle Homes if requested.

· Any faults or defects must be rectified prior to use.

· All equipment must be in good working order and maintained by qualified persons in accordance with manufacturer’s guidelines and Australian Standards.

· All persons must be competent and licensed operators as required by local / state authorities

· Be aware, stay alert and heed all safety warning signs, they are there for your protection

· Wear the appropriate personal protective equipment when working with equipment.

· Operators must secure plant and equipment whenever left unattended, including locking out and tagging out the plant and equipment if appropriate.

· Employees must consult with others in close proximity to work areas. If there are issues please contact your site supervisor

· Safe work method statements and risks assessments must include safety requirements to protect the public, on site employees, and contractors, including no go zone requirements. This may include witches hats, barricades or signage

27 NO GO ZONES

No Go Zones are designated regions that apply to work near OVERHEAD or UNDERGROUND ASSETS. The term “NO GO ZONE” is utilised in order to provide a definition for safety clearances surrounding both overhead and underground services.

Overhead services

If you are working with plant “near” an overhead service, you are required to conform to specific work practices.

· Between 0mm to 3000mm including an aerial line into another house a permit from the asset owner is required. This will set out specific requirements that you need to follow.

· You may work Between 3000 mm to 6400mm from an overhead electrical cable on a pole a spotter is required and 8000 mm if the electricity cable is on a tower line.

· A safe work method statement must also be developed and followed by all employees.

If you are working on or installing scaffolding the following requirements apply. A permit to work in the no go zone if the scaffolding is:

· Under 5000mm from top rail to power lines

· Under 4600mm from the side of the scaffold to power lines

· A safe work method statement must also be developed and followed by all employees

If the design envelope of the plant is able to encroach on this clearance in its working position, then the employer must obtain permission from the electricity company; or alternatively Employers will be deemed to have gained permission if they have developed safe systems of work and they are working in accordance with these guidelines. If you cannot comply with the “NO GO ZONE” safety procedures, then NO WORK should be undertaken without specific permission from the electricity company.

Underground services

Prior to the commencement of works, you must determine if your work practices are to interfere with underground assets. If so underground assets are required to be identified and you are to determine if this work will encroach the NO GO ZONE clearances surrounding these services. If you are working with plant “near” an underground service and following the NO GO ZONE guidance material you may work no closer to the asset than:

300 mm for individuals and 500 mm for plant and equipment, or

3000 mm in the case of any assets registered under the pipeline act, or

Any electrical cable with an in-service voltage greater than 66kv.

For principle assets requiring significant integrity of the material surrounding the asset, greater clearance may be required. Information regarding these clearances can be obtained with the initial asset location enquiry.

Please contact your site supervisors before works start if you have any concerns regarding working around overhead or underground services.

28 TRAFFIC MANAGEMENT

You are responsible for ensuring appropriate traffic management arrangements are in place before commencing work. However, the following general rules apply to all Carlisle Homes’ sites:

· Traffic management shall be organised in consultation with the site manager for:

· Deliveries to site

· Site activities where public could be affected

· A safe work method statement must be developed for all traffic management tasks

A traffic management plan must be drafted by an appropriately qualified person and provided to Carlisle Homes prior to the commencement of works.

Appropriate worksite traffic management is generally achieved by planning and designing a system based on

Australian Standard AS 1742.3: “Manual of uniform traffic control devices – Traffic control devices for works on roads” and in accordance with VIC Roads requirements.

When traffic management has been arranged and in place, ALL Trades are to conform to the instructions given by the Traffic Controller. During the course of your construction works if you believe that your work activities may affect either road or public users, contact your site manager for further direction.

29 FIRST AID

All trades are required to adequately equip themselves with suitable and appropriate first aid equipment and ensure that the equipment is continually maintained / updated / replaced in accordance with Australian Standards.

A member of each work team must have suitable first aid training in order to render assistance if required.

If an incident occurs on site that injures a person, immediate first aid treatment should be provided (IF SAFE TO DO SO and the person providing first aid is qualified to do so). The injured worker should then be referred for treatment to a local medical practitioner or hospital.

All injuries sustained on site MUST be verbally reported to your Site Manager.

30 EMERGENCY PROCEDURES

In an emergency, the first priority must be the safety of the site and the care and wellbeing of the injured person as well as your own safety.

The Carlisle Homes signage will have appropriate phone numbers on it in an emergency. Purchase orders will also have site contact details on it.

We suggest that you enter the local emergency medical numbers and the number of your site manager into your mobile phone. In an emergency remain calm and if calling the Emergency Services (000), be prepared to give them the following information:

· Exact address of the accident

· The telephone number you are calling from

· The number of injured people

· Is the injured person conscious?

· Is the injured person breathing?

· The type and extent of injuries

· State if other emergency services are required

· Answer any further questions.

· Stay at the scene until given permission by the Emergency Services to leave. Do not disturb or move anything at the incident site unless it is to minimise harm, make the site safe or deal with the emergency itself.

31 INCIDENTS AND ACCIDENTS

If an injury, serious incident (e.g. falling brick wall or pit collapse) or near miss does occur on site you must immediately notify a Carlisle Homes representative.

If you are requested to do so by a Carlisle Homes representative, an injury/ incident form should be supplied to Carlisle homes to document what actually happened.

The incident scene following a serious injury or dangerous incident must not be disturbed until advised otherwise by Worksafe or a representative of Carlisle Homes unless the disturbance is to minimise harm, make the site safe or deal with the emergency itself. Severe penalties apply if you disturb an accident scene following a ‘reportable’ incident.

If you are an employer of another person working on site, then you may also have responsibilities to report serious incidents to Worksafe. In some cases both the principal contractor and the employer of the injured person may be required to report.

Before reporting any incident, you must first seek advice from your site manager or safety manager.

The following types of incidents are notifiable to WorkSafe and must immediately be verbally reported to Carlisle Homes:

Serious injury

Where an incident results in:

· The death of any person;

· A person requiring treatment as an in-patient in hospital;

· A person requiring immediate medical treatment by a registered medical practitioner for the following types of injuries:

· the amputation of any part of his or her body;

· a serious head injury;

· a serious eye injury;

· the separation of his or her skin from underlying tissue (such as degloving or scalping);

· an electric shock;

· a spinal injury;

· the loss of a bodily function; or

· serious lacerations.

Dangerous occurrence

Where an incident exposes a person in the immediate vicinity to an immediate health or safety risk:

· the collapse, overturning, failure or malfunction of, or damage to, any plant that are required to be registered under the Regulations;

· the collapse or failure of an excavation or any shoring supporting an excavation;

· The collapse or partial collapse of any part of a building or structure;

· An implosion, explosion or fire;

· The escape, spillage or leakage of any substance including dangerous goods; or

· The fall or release from a height of any plant, substance or object.

32 AGENTS ACCESS TO SITE

All Agents must comply with the following minimum requirements prior to being granted access to any Carlisle Homes building construction site. This process must be repeated if the Agent requires multiple visits to the same site at the various stages of construction, Note if previously collected insurances are still current they will not need to be submitted again.

Agents Must:

1. Receive an Induction into the Carlisle Health & Safety Co-ordination Plan & Site Rules.

2. Provide Construction Induction – CI Card details.

3. Provide SWMS for all High Risk activities that will be undertaken and or are anticipated that will be undertaken.

4. Provide Public Liability and Work Cover Insurances, preferably certificates of currency.

5. Only access and or work on site whilst Carlisle Site Management/Representative is present.

6. Follow all OHS instructions as per the Carlisle Health & Safety Co-ordination Plan and any additional OHS requirements that are deemed necessary by Carlisle Site Management/Representative.

7. A Copy of the Health and Safety Co-ordination Plan is to be given to the representative undergoing the induction, providing they have met all the above criteria.

Note: the Health & Safety Co-ordination Plan, Commitment & Acknowledgment Form must be handed into your Manager and filed at head office once complete (and before any work commences).

33 MATERIAL SAFETY DATA SHEETS

No dangerous goods or hazardous substances are to be brought onto the site unless they are accompanied by a Material Safety Data Sheet (MSDS). All products that fit these criteria are to be used to conform to the strict control methods outlined in the MSDS. At all times, the welfare of other trades, Employees and members of the public on and surrounding the site is to be given the highest priority, with the appropriate notification and signage to be displayed if required.

34 PERSONAL PROTECTIVE EQUIPMENT

You must use safety and other personal protective equipment (PPE) when and wherever required by OH&S legislation, codes of practice, Australian Standards or industry practice. The PPE must be maintained in good working order and updated or replaced in accordance with manufacturer’s guidelines and Australian Standards.

Each individual Contractor, Trade and their employees are responsible for the provision of PPE. Any faults or defects of any equipment should be reported immediately. Safety equipment including gloves, safety goggles, earmuffs, steel cap boots, high visibility vests are important items that must be used when appropriate.

When out in the sun:

· Ensure that your UV protection is maintained when the sun is the strongest

· Cover as much of your exposed body as possible. Exposed bare backs are unacceptable.

· Wear a hat preferably with a wide brim that covers your neck.

· Apply SPF 30+ or higher sunscreen. Reapply every two hours.

35 MANUAL HANDLING

Manual handling covers a wide range of activities including lifting, pushing, pulling, holding, carrying. People often attempt to carry out tasks manually that should be done in another way. There are many situations that direct manual handling can be either eliminated or drastically reduced by alternate methods or by employing mechanical equipment. If the load is too heavy or unsafe to manually lift, then seek instruction from your site manager or supervisor.

Some general rules regarding lifting and manual handling are as follows:

· Use mechanical equipment where necessary.

· Plan the lift by estimating the load and knowing exactly where it is going to be placed.

· Ensure that access to the area is clear.

· Avoid lifting excessively heavy loads.

· Avoid bending your back to lift the load- bend your knees and keep your back straight.

· Hold the load as close to your body as possible.

· Avoid twisting while lifting or carrying.

36 HOURS OF WORK

All council by laws will be adhered to during weekdays and weekends. In the event works outside of normal working hours are required, approval must be obtained from the site manager. When working on weekends the following will be adhered to:

· Your site manager must be notified prior to commencement of any works on sites. Including type of work and duration.

· In the event of an incident please ensure an appropriate first aid equipment and qualified personnel are on site.

· At least two persons must be on site at all times.

· When working outside normal hours, mobile communication must be available at all times.

· A text message or a phone call must be made to the Carlisle site manager upon arrival and departure if outside standard site working hours.

37 OTHER RULES

The following are additional safety rules that must be complied with at all times on site.

· Illegal drugs and alcohol are strictly prohibited on site. No person is permitted to go onto or to work on a site who is under the influence of drugs or alcohol;

· No smoking on site;

· Bullying, sexual harassment, racial insults or other similar behaviour will not be tolerated from any person to any other person at any time;

· Horseplay or pranks must not be engaged in;

· Children who are not employed or legally involved in the building activities are not permitted on site at any time;

· Pets or animals are not permitted on site at any time.

· Noise pollution from HIGH volume radios must be kept to a minimum - keep the volume reasonable for all.

· Safety instructions must be obeyed at all times.
