Requesting 1172in MilConnect

- Go to https://milconnect.dmdc.osd.mil/milconnect/- Then click on “Sign In” button located on the top right hand side of the page

- Click on the appropriate “Login”

- Then Click “OK” button

- If you pick CAC, Click “Login” and select the appropriate certificate and click “OK”

- Click “More Goals” button located on the bottom right hand side of the page

- Click “View ID card info”

- Click “OK” button

- Click “Print Family List”

- Select your family members- Click “Proceed”

- Read PA statement- Click “I Agree”

- Click “Proceed”

- Click “Display Form” and DD FORM 1172-2 will populate
