WHEREAS, Honorable Michael Angelo Melgarejo was elected as SK Chairman of Barangay Estancia and at the same time as PPSK President of Kalibo, thus making him Ex-Officio Member of the Sangguniang Bayan of Kalibo;

WHEREAS, Hon. Melgarejo was able to introduce meaningful pieces of legislation and participated extensively in the deliberations of this August Body;

WHEREAS, Hon. Melgarejo conducted several activities such as training and workshops, cultural shows and sports festival for the SK Members of the sixteen (16) Barangay’s of the Municipality of Kalibo;

WHEREAS, Hon. Melgarejo also conducted feeding programs in the sixteen(16) Barangay’s during the conduct of Sikad Kalibo and has proven himself to be capable leader and event organizer;

WHEREAS, Hon. Melgarejo ably stood up for the welfare of the meat and fish vendors at the Kalibo Public Market and the Tricycle Operators of the Municipality in the deliberations on legislative proposals that would directly affect them;

WHEREAS, the youth and people of Kalibo have been privileged to have Hon. Michael Angelo Rebaldo Melgarejo as a member of the Sangguniang Bayan who unselfishly dedicated himself to competent, accountable and responsible public service;

WHEREAS, the Sangguniang Bayan as a collegial body enacts and adopts responsive legislative measures and policies vital to the socio-economic development and environmental health of the Municipality of Kalibo. It provides the legal framework and directions to effectively and efficiently steer local governance;

WHEREAS, the invaluable services rendered by former SB Member Ananias V. Solina as Chairman of the Committee on Barangay Affairs and NGO’s has actively participated in the deliberation during sessions and committee meetings/ hearings of the Sangguniang Bayan of Kalibo which greatly contributed to productive and quality legislation of the Local Government Unit of Kalibo for three (3) successive years and thus, helped the Municipality attain its present growth and progress;

WHEREAS, Mr. Ananias V. Solina as Ex-Officio Member of the Sangguniang Bayan of Kalibo From 2010 to 2013 has supported for the passage/enactments of ordinances and resolutions that are necessary to the municipality’s development and progress especially those legislative measures that contribute to effective governance and are essential to the promotion of the general welfare;

WHEREAS, Liga President and Ex-officio Member Ananias V. Solina has won the respect of other Punong Barangays of Kalibo by his style of leadership in steering the Liga ng mga Barangay in terms of extending assistance in any form for the effective and efficient delivery of services and for the successful implementation of priority programs and projects of the barangays. This kind of leadership and services rendered by former SB Member Ananias V. Solina has already been provided during his incumbency as President of the Liga ng mga Barangay of the Province of Aklan and Ex-officio Member of the Sangguniang Panlalawigan of Aklan in the years 2002-2007;

WHEREAS, Mr. Ananias V. Solina through his effort as Liga President has made strong representation in the assignment of personnel in the extension office of the Municipal Mayor to provide accounting services and handle/ facilitate various transactions of the barangays;

WHEREAS, after successful completion of three (3) productive years as member of the Sangguniang Bayan of Kalibo, Mr. Ananias V. Solina deserves to be recognized and acknowledged for his exemplary performance and accomplishments in the local legislative body;

WHEREAS, it is the sentiments of this August Body to convey profound appreciation and recognition to Mr. Ananias V. Solina former Ex-Officio member of the Sangguniang Bayan of Kalibo for his dedicated efforts, contribution, cooperation and whole hearted support to the overall programs and projects of the Municipality of Kalibo;

NOW THEREFORE, in view of the foregoing premises, upon motion made by Hon. Ariel O. Fernandez and Hon. Rodillo L. Policarpio, duly seconded be it RESOLVED as it hereby RESOLVED to express profound appreciation and recognition to former SB Member Ananias V. Solina for his contribution to the development and progress of the Municipality of Kalibo as President of the Liga nga mga Barangay and Ex-officio Member of the Sangguniang Bayan of Kalibo.

RESOLUTION CONVEYING SINCERE APPRECIATION AND HEARTFELT GRATITUDE TO PUBLIC AND PRIVATE AGENCIES/ENTITIES/CORPORATIONS, FOUNDATIONS, NON-GOVERNMENT ORGANIZATIONS, INSTITUTIONS AND INDIVIDUALS FOR THEIR INVALUABLE ASSISTANCE/DONATIONS TO THE VICTIMS OF SUPER TYPHOON YOLANDA IN THE MUNICIPALITY OF KALIBO

FERNANDEZ WHEREAS, the Municipality of Kalibo, center of government, education, trade and commerce and economic activity of the Province of Aklan was hit by strong wind with a gustiness in excess of 215 kph. brought by Super Typhoon Yolanda. It destroyed homes and buildings, agricultural products and other infrastructure and facilities, Two Thousand Seven Hundred Seventeen (2,717) houses were totally toppled and Seven Thousand Two Hundred Fourty Two (7,242) partially destroyed leaving Nine Thousand Eight Hundred Seventy (9,870) families or Thirty Seven Thousand Eight Hundred Seventy Four (37,874) individuals in want, grief and misery;

WHEREAS, stockpiles of relief goods were deployed and distributed to homeless and deprived residents but this did not last as people are asking for more foods and shelter to include livelihood and support for their damaged crops, plants trees, etc.;

WHEREAS, after the aftermath of Super Typhoon Yolanda, the town was left behind with destroyed houses and buildings, damaged agricultural products, infrastructure, lifelines and public utilities which is now the responsibility and obligation of the Local Government Unit of Kalibo to provide help/assistance to victims of the said disaster for them to fully recover and rehabilitate their lost and destroyed properties and belongings;

WHEREAS, the Local Government Unit of Kalibo has availed the overwhelming response and assistance from generous sectors of society for their donations of assorted goods, medicines, cash, rice and used clothings, etc. which was of great help to the victims of calamity in their road to recovery;

WHEREAS, the people of the Municipality of Kalibo deeply appreciates and recognized the concern, support and assistance of various sectors of society who has voluntarily extended assistance/donations to the Municipality of Kalibo, to wit:

WHEREAS, the Municipal Disaster Risk Reduction and Management Council of Kalibo carried out one of the most massive and complicated evacuation in the history of this Ati-Atihan town;

WHEREAS, the logistics involved is so massive and so complex that our resources thinned out as fast as people leave their homes for the first time in their lives to save themselves from possible flash flood brought by heavy rains and storm surges, and deluge-like winds directly carried by Typhoon Yolanda classified officially by the Philippine Atmospheric Geophysical and Astronomical Services Administration or PAGASA as Public Storm Warning Signal Number 4;

WHEREAS, the Disaster Response Committee under Chairpersonship of Hon. Ariel O. Fernandez with Mr. Efren Trinidad as Vice Chairperson and the personnel of the Kalibo Auxiliary Police, Ms. Malou Lachica and personnel of the Office of the Municipal Mayor, Ms. Edith De Lemos and the Municipal General Services Office, and Engr. Emerson Lachica and Municipal Engineering Office provided technical and administrative operational support before, during and after the onslaught of Super Typhoon Yolanda serving as personnel and staff of the Administrative Operations Center at the 2nd Floor Conference Hall handling the logistical and administrative needs of the MDRRM Council in support to the Emergency Operations Center located at the opposite end of Magsaysay Park that ran Special Operations Units and Volunteers handling high risk, high stress and dangerous operations to effect a successful Preemptive and Forced Evacuation Campaign and Search and Rescue Operations;

WHEREAS, the Subcommittee on Health Services/Hospital-Based Care and Emergency Medicine under Dr. Makarius Tel-Aviv Dela Cruz III performed to the best of their profession and then some by staying in their assigned Evacuation Centers and have not asked for relievers or complained from the start of the forced evacuation on November 7, 2013 up until the Super Typhoon Yolanda subsided and even during the Flash Flood Watch that ended the next day, November 9, 2013;

WHEREAS, the Subcommittee on Preemptive Evacuation, Early Warning, Security and Fire Suppression with the PNP Personnel under Supt. John Lemuel Villafranca and SPO4 Fred Policarpio and the BFP Personnel under Fire Insp. Donnie Torre performed above and beyond their call of duty by assisting the Municipal Disaster Risk Reduction and Management Council in the conduct of Early Warning using the sirens of emergency vehicles to alert residents of possible flash floods due to heavy rains and storm surge, providing security personnel during forced evacuation, and providing transport vehicles for disaster evacuation work;

WHEREAS, the Subcommittee on Relief Assistance, Evacuatio Kitchen and Camp Management under Ms. Lolly Espino performed at the best of their abilities, patience and dedication to social work when they experienced the most complex and logistically terrifying evacuation ever conducted by the Municipal Disaster Risk Reduction and Management Council where not just hundreds have been evacuated but thousands with several undesignated Evacuation Centers opening up greatly lessening their manpower but despite this hurdle, they managed to overcome these difficulties without being relieved from the start of the preemptive evacuation, during the forced evacuation, during the onslaught of the Super Typhoon, and after the storm has passed;

RESOLVED, as it is hereby RESOLVED, to commend the members of the Municipal Disaster Risk Reduction and Management Council’s Disaster Response Committee that included the Office of the Municipal Mayor, the Municipal General Services Office, and Municipal Engineering Office; Subcommittee on Health Services/Hospital-Based Care and Emergency Medicine under Dr. Makarius Tel-Aviv Dela Cruz III; the Subcommittee on Preemptive Evacuation, Early Warning, Security and Fire suppression under Supt. WHEREAS, pursuant to Executive Order No. 90, the National Housing Authority (NHA) is the sole government agency engaged in direct housing production and mandated to meet the housing needs of the low and marginal families and to focus its efforts towards the housing needs of the lowest 30 % of the urban population;

WHEREAS, pursuant to Republic Act No. 7160 and Republic Act No 7279, otherwise known as the Local Government Code of the Philippines and the Urban Development and Housing Act of 1992, respectively, Local Government Units (LGU’s) are mandated to take principal roles in the provision of housing services to their constituents;

WHEREAS, pursuant to NHA Board Resolution No. 5565 dated February 14, 2013, the, Board has approved the proposal to engage the LGU’s s alternative Project Implementation of the Housing Materials Assistance (HOMA) Program, in cases when NHA cannot immediately provide assistance because of widespread damage to homes affecting several municipalities, where families affected are in far- flung and remote rural areas and where NHA has no project office;

WHEREAS, on November 8, 2013, Super Typhoon Yolanda hit and devastated the Municipality of Kalibo where Two Thousand Seven Hundred Seventeen (2,717) houses where totally toppled and Seven Thousand Two Hundred Forty Two (7,242) partially destroyed leaving Nine Thousand Eight Hundred Seventy (9,870) families or Thirty Seven Thousand Eight Hundred Seventy Four (37,874) individuals in want, grief and misery;

WHEREAS, the Local Government Unit of Kalibo hopes to undertake implementation of the proposed Housing Materials Assistance (HOMA) Program of National Housing Authority for families affected by November 8, 2013 Super Typhoon Yolanda in the Municipality of Kalibo;

NOW THEREFORE, in the view of foregoing premises, upon motion made by Hon. Ariel O. Fernandez and Hon. Rodillo L. Policarpio, duly seconded, be it RESOLVED as it is hereby RESOLVED to authorized Honorable Mayor William S. WHEREAS, Administrative Order No. 241, Section 5, provides that, “agencies shall institute a Performance Evaluation System based on a objectively measured output and performance of personnel and units”. Subsequently Administrative Orders were issued with the end view to establish a unified and integrated Results Based Performance Management System (RBPMS) to all government entities that the gave birth to the promulgation of CSC Res. No. 1200481 that set the guidelines in the establishment and implementation of Agency Strategic Performance Management System;

WHEREAS, the Civil Service Commission (CSC) Aklan Field Office enjoined, directed and pursued all government agencies and instrumentalities including local government units in the Province of Aklan to craft its Strategic Performance Management System to guide government agencies in the implementation of performance based incentive scheme to reward exemplary civil servants and deliver efficient and effective public service;

WHEREAS, in compliance with CSC directives the LCE of Kalibo organized a Performance Management Team to craft the SPMS which was presented to the Honorable Mayor William S. Lachica for indorsement to the Sanggunian;

WHEREAS, the LCE approved and indorsed the same to the Sangguniang Bayan for adoption and finding the System in compliance with CSC guidelines, the same is hereby approved and adopted;

NOW THEREFORE, in view of the foregoing premises, upon motion made by Hon. Daisy S. Briones, duly seconded, be it RESOLVED as it is hereby RESOLVED, to approve and adopt the Strategic Performance Management System of the Municipality of Kalibo;

RESOLUTION APPROVING THE LIST OF ANTI-POVERTY REDUCTION PROJECTS FOR THE MUNICIPALITY OF KALIBO

WHEREAS, Executive Order No. 43 s, 2011 dated 13, May 2011 of His Excellency Benigno Aquino III mandated the Cabinet to organize into clusters that shall serve as the primary mechanism of the Executive Branch towards the realization of the Social Contract with the Filipino People;

WHEREAS, the Human Development and Poverty Reduction Cluster (HDPRC) is tasked to improve the overall quality of life of the Filipino and translating the gains on good governance into direct, substantial benefits that will empower the poor and marginalized segments of the society;

WHEREAS, the Local Government Code (LGC) of 1991 mandated community to participate in the Local Development Councils and various organized Local Special Bodies, such as, local school and health boards, and Local Development Councils, among others;

WHEREAS, the National government agencies and other stakeholders shall pursue the Millennium Development Goal of reducing poverty from 26.5% in 2009 to 16.6% by 2015;

WHEREAS, the Good Governance and Anti- Corruption Cluster, the Human Development and the Poverty Reduction Cluster, and the Economic Development Cluster, are implementing Grassroots Budgeting (formerly Bottom-up Budgeting) to ensure the inclusion of the funding requirements for the development needs of the focus local government units (LGUs) as identified and approved by the HDPRC;

WHEREAS, the LGU has organized/reactivated the expanded Local Poverty Reduction Action Team (LPRAT), chaired by the Local Chief Executive and composed of the Chairperson of the Sangguniang Lungsod/Bayan Appropriation Committee, the Planning Officer, Agriculture Officer, Social Welfare and Development Officer, Budget Officer, Liga ng mga Barangay, Local Government Operations Officers, DSWD Municipal Links, a Pantawid Pamilya Parent-Leader, a Community Health Team Leader, LGU accredited CSOs, NGA accredited/recognized CSOs, NAPC identified basic sector representatives and a private sector representative;

WHEREAS, the LPRAT conducted a workshop to formulate a Local Poverty Reduction Action Plan (LPRAP) indicating therein unfunded priority anti-poverty projects;

WHEREAS, the LPRAT has prepared the list of priority projects for 2015 based on the LPRAP for allocation of budget from the national government agencies funds and other financial institutions for 2015;

WHEREAS, the list of priority projects was duly endorsed by the partner CSO representatives as proof of genuine participation on identifying the priority projects;

WHEREFORE, on motion of Hon. Ariel O. Fernandez duly seconded, be it RESOLVED, as it is hereby RESOLVED, that the list of priority poverty reduction projects identified in the LPRAP shall be submitted to the concerned government agencies for integration in their respective budget for 2015;

WHEREAS, it is the major thrust of the Local Government Unit of Kalibo to provide its people the basic health care services as mandated by R.A. 7160 or the Local Government Code of 1991;

WHEREAS, the mobilization of the Community Health Teams (CHTs) is a strategy employed by the Department of Health to guarantee that every family in the community is periodically visited and attended by a health provider to ensure achievement of the Millennium Development Goals and Universal Health Care. This strategy has a special focus on the Filipino families under the National Household Targeting System (NHTS) for the delivery of key health messages and increase utilization of health care services by the households;

WHEREAS, the Municipality of Kalibo had operationalized the Community Health Teams and recognizes the need to sustain its mobilization to ensure that all NHTS Households are visited, profiled, monitored, and are able to avail the different necessary health services;

WHEREAS, the Sangguniang Bayan deems it necessary to authorize the Local Chief Executive, Mayor William S. Lachica to enter into a Memorandum of Agreement with the Department of Health Center for Health Development Western Visayas for the attainment of the above aspirations;
