complaint relative to the service/transaction; or

Steps/Procedure
1). Complainant/s shall submit the
necessary documents; and the
documents shall be assessed if they
comply with the documentary
requirements provided in the checklist.
(See Checklist)
2) If the documents are complete, the
complainant/s will fill-up the
Investigation Data Form. The record,
shall be received, stamped and
docketed, then will be indorsed to the
Duty Inquest Prosecutor.
3) If the respondent/s opted not to sign
a waiver of Article 125 of the RPC, an
Inquest Disposition Form shall be
prepared by the Inquest Prosecutor.
4) If the respondent/s opted to sign a
waiver of Article 125 of the RPC, and
consequently requested a Preliminary
Investigation, a regular conduct of the
latter shall be done, the Inquest
Prosecutor shall issue Subpoena to the
respondent/s, for him/them to submit
his/their Counter-Affidavit and other
supporting documents within 10 day
period.

Steps/Procedure
1). Complainant/s shall submit the
necessary documents; and the
documents shall be assessed if they
comply with the documentary
requirements (See Checklist).
2) If the documents are complete, the
complainant will fill-up the Investigation
Data Form and thereafter the
complaint’s/s’, witness’/es’ (if any)
affidavit/s and the Investigation Data
Forms, will be subscribed/certified under-
oath.
3) The complaint/s, with all duplicate
copies and supporting documents, will
be received and docketed.
Complainant/s shall wait for a duly
stamped/ received copy with an
assigned NPS docket number.
4) The record/s of the complaint/s shall
be assigned in a sequential manner with
pre-determined pattern among
prosecutors in this office, thereafter the
conduct of Preliminary Investigation shall
begin.
