Uncorrected refractive errors at 43% is the primary cause of visual impairment. The number of visually impaired in the South-east Asian region (India excluded) stands at 27.9 million and in India it is 62.6 million. 1 The economic productivity loss dye to global burden of uncorrected refractive error is estimated at US$ 121.4 billion. 2 Additionally, there are significant number of people with near vision impairment- 49% to 83% among people of 35 years or older. Over 90% of those in need of near refractive correction in rural settings do not have the necessary spectacles. 3 It is increasingly realised that the uncorrected refractive errors can hamper performance at school, reduce employability and productivity, and generally impair quality of life. Yet the correction of refractive errors with appropriate spectacles is among the most cost-effective interventions in eye health care. It is obvious that there are significant numbers of people who do not receive appropriate correction and in many the correction may not be the appropriate. Appropriate refraction and dispensing of spectacles is a special skilled activity performed by both ophthalmologists invariably though it is increasingly realised that this could be done by trained optometrists (or ophthalmic technicians, such as ophthalmic assistants, vision technician and refraction nurses) with equal ease and competence. This is necessary in view of ophthalmologist man-power constraints in many countries. It is also will also release the ophthalmologists in more important eye care. Many semi urban and rural optical outlets also do refraction where the spectacles are delivered according to the prescription. Unfortunately they do not employ the right personnel for this skilled work. 1 Pascolini D, Mariotti SP. Global estimates of visual impairment: 2010. Trainers Training in Optical Dispensing (TTOD)

Background:

Globally, 285 million are estimated to be visually impaired and 39 million blind. Uncorrected

refractive errors at 43% is the primary cause of visual impairment. The number of visually

impaired in the South-east Asian region (India excluded) stands at 27.9 million and in India it

is 62.6 million. 1 The economic productivity loss dye to global burden of uncorrected

refractive error is estimated at US$ 121.4 billion. 2 Additionally, there are significant number

of people with near vision impairment- 49% to 83% among people of 35 years or older. Over

90% of those in need of near refractive correction in rural settings do not have the necessary

spectacles. 3

It is increasingly realised that the uncorrected refractive errors can hamper performance at

school, reduce employability and productivity, and generally impair quality of life. Yet the

correction of refractive errors with appropriate spectacles is among the most cost-effective

interventions in eye health care. It is obvious that there are significant numbers of people

who do not receive appropriate correction and in many the correction may not be the

appropriate. Appropriate refraction and dispensing of spectacles is a special skilled activity

performed by both ophthalmologists invariably though it is increasingly realised that this

could be done by trained optometrists (or ophthalmic technicians, such as ophthalmic

assistants, vision technician and refraction nurses) with equal ease and competence. This is

necessary in view of ophthalmologist man-power constraints in many countries. It is also will

also release the ophthalmologists in more important eye care. Many semi urban and rural

optical outlets also do refraction where the spectacles are delivered according to the

prescription. Unfortunately they do not employ the right personnel for this skilled work.
