INSTRUCTIONS
Requirements: The authorized permit or license holder (permittee) allowed to collect destructive devices must provide a signed statement that the weapon(s) or ordnance were not fired or discharged pursuant to California Code of Regulations, title 11, section 4130, subdivision (e). Permittees authorized to publicly display their destructive device inventory shall maintain records of when their collection was put on display in California. Current records must be maintained on this form, or a computer generated printout using the same format. (Cal. Code Regs., tit.11, § 4130, subd. (e)(2).) The permittee must make these records available to any peace officer or other persons designated by the Attorney General upon request. (Cal. Code Regs., tit.11, § 4130, subd. (h).) Permittees authorized to collect destructive devices shall submit to the Department of Justice (DOJ) an annual report of any public displays. The annual report shall be submitted on form BOF 026 or a computer generated printout using the same format as form BOF 026. The annual report shall be continuous from the prior year's renewal application annual report. The annual report shall include the entry number, date, time, location, purpose of display, and device(s) displayed. Or, upon termination of the permit, a final report detailing the disposition of all destructive devices must be submitted to DOJ. Completing the Report of Destructive Device Public Display Activity form: This form provides two entry sections for reporting two instances of up to three separate destructive devices. Make as many copies of this form as required to complete your reporting requirement. Please complete the following fields for all entries: - Entry Number - Make, Model, Serial Number and Caliber - Date - Permittee Signature and Date - Time - Dangerous Weapons Permit Number - Location - Reporting Period Dates - Purpose - Current Page Number - Total Number of Pages
