Dr. Richard Lopez, a primary care physician at Atrius Health, serves as Senior Vice President, Population Health and Chief Medical Officer, VNA Care. In this position, Dr. Lopez works collaboratively with senior clinical and executive leadership across a wide range of clinical initiatives. Julie Glendenning, MSN, RN, APN, WCC, Vice President Learning and Development, JourneyCare

Julie was promoted to Vice President of Learning and Development in June, 2016. She joined JourneyCare in 2012 as Director of Learning and Development. With over a decade of experience in hospice and palliative career, Julie previously was the attending provider for hospice patients on the residence at Agrace Hospice Care in Madison, Wis., and has worked in the acute care hospice area, managing pain and symptoms at end of life.

Stephanie Jackman-Havey, CPA, CFO & COO, VNA Care

Stephanie has over 20 years of experience in Senior Leadership roles in management. She oversees the Information Systems and administrative/facilities services at VNA Care. Stephanie assists in financial analysis to make informed decisions of streamlining, efficiencies and cost containment. She has developed internal/external financial and operational policies and audit controls along with streamlining the managed care and insurance verification departments.

As President of the Remington Health Strategy Group, and Publisher of the Remington Report magazine, Lisa has worked with over 5,000 home health, hospice, palliative care, private pay, community-based organizations and health systems in both a consultancy role and educator. Her expertise drives organizational “value” planning to increase revenues, identify new and existing growth opportunities, and leverage strategic partnerships across the continuum. Nationwide, leaders and their teams attend Remington Think Tank summits, executive leadership programs, peer-networking groups, and chronic care management collaboratives to improve performance, hardwire their organization’s future, and lead with vision. Chris has been a strategic leader at Advocate Health Care for more than 15 years. She is responsible for strategic development and business planning for Advocate at Home, Advocate Health Care’s home care division, which includes Home Care, Home Infusion, Home Equipment/Respiratory Therapy, Hospice and Post Acute partnerships. Chris works closely with Advocate Health Care’s hospitals, physician practice leaders and AdvocateCare, Advocate’s accountable care organization. In this role, she is responsible for creating strategy that supports the organization’s goals of growth and total cost of care.

Jessica Selby, Sr. Director, Post Acute Operations, Banner Home Care & Hospice

Jessica is the Senior Director of Post-Acute Operations for Banner Health. With a diverse background in home health and hospice operations along with process improvement management, she enjoys the opportunity to discuss the disruptive change necessary to begin developing and advancing the care delivery model through the integration of care across the continuum. Jessica’s interest in Banner Health developed as a result of a leadership consulting assignment for their home health program in 2013.

Gavin Ward, Regional Director Strategy, 24Hr Homecare

Gavin is the Regional Director of Strategy and Partnerships for 24Hr Homecare. His data-driven efforts to improve throughput (via a first-to-market innovative partnership with Uber), reduce length of stay, prevent readmissions, and transition patients to a lower cost of care have helped yield over 25 system funded partnerships in California, Arizona and Texas with hospitals, managed care groups, payers, and bundled payment participants.

Patty is the Director of Care Transitions at FirstHealth of the Carolinas, Inc., since November 1999. She serves as the overall leader and manager of Care Transitions which includes: two freestanding not-for-profit home health agencies; a complex care management program for patients not eligible for home health but who require ongoing support and management of their chronic disease; and the FirstHealth Center for Telehealth providing remote monitoring services across care settings. Under her leadership, FirstHealth developed and implemented clinical pathways of care for heart failure, chronic obstructive pulmonary disease, diabetes, cardiothoracic surgery and hip and knee replacements.

No registration after February 10, 2017. No walk-ins. See conference registration form on pages 10-11 for early bird discounts and conference cost. All attendees must wear their badges while attending the conference. If payment is not received by the last day of registration, we will not process the registration.

Conference Cancellation/Transfer Policy

All conference cancellations must be submitted in writing and faxed to The Remington Report at 949-715-1797. For conference cancellations received before January 12, 2017, the registration fee will be returned, less a processing fee of $150.00 per attendee. After January 17, 2017, no refunds will be made, however registration is transferable to another person from the same agency up until three weeks prior to the conference. We will NOT transfer registrations after this date under any circumstances. After this date, the registration fee is forfeited. Cancellation requests not received by the date required will not be eligible for a refund. No shows will not be refunded or able to transfer to future conferences. There are no exceptions to our policy.

Attendees may not pay for one attendee to attend the conference and bring two people to have one person attend one day and one person attend the next day. There are no substitutions. No refunds due to weather conditions. We do not accept walk-ins.

The Remington Report assumes no liability for non-refundable transportation costs, hotel accommodations or additional non-conference related expenses incurred by registrants. We are not responsible for lost items or materials. Space is limited. One conference handout book is provided to each paid attendee. We do not provide replacements.
