Prévention de la réintroduction du paludisme dans les régions où l ’éradication a été réalisée

(Extraits du Douzième Rapport du Comité de la Quarantaine internationale

adopté par la Dix-Septième Assemblée mondiale de la Santé)

. Le Comité a examiné la section 3 (Prévention de la réintroduc­tion du paludisme dans les régions où l’éradication a été réalisée) du dixième rapport du Comité d’experts du Paludisme.^ Le Comité rappelle que, dans son cinquième rapport,^ il a recommandé;

ï). que les personnes effectuant des voyages internationaux ne soient soumises aucune mesme sanitaire spéciale;ii) que des m e su ^ spéciales soient appliquées à divers groupes;iii) que des mesures appropriées soient prises contre les mous­tiques dans les zones frontières ou dans les centres de rassemble­ment vers lesquels sont dirigés les groupes susmentionnés, étant entendu qu’en vertu de l’article 103 du Règlement sanitaire international im pays qui s’estime en danger est habilité à preiidre les mesures visées;iv) que des dispositifs communs soient mis en place dans les zones frontières internationales, afin d’éviter le transport de la maladie d’un pays à l’autre;v) que soit institué un système d ’échange complet de renseigne­ments sur les mouvements de groupes de population et sur la sensibilité et la résistance des anophèles vecteurs aux insecticides.

Le Comité d’experts du Paludisme a distingué quatre catégories de voyageurs pouvant constituer un danger du point de vue de la propagation internationale du paludisme:

i) la main-d’œuyre employée par de grandes entreprises ou les groupes organisés encadrés par des responsables et possédant rm service médical propre, militaires par exemple;ii) 'les groupes nomades ou semi-nomades dirigés par un ou plusieurs chefs;iii) les groupes plus ou moins inorganisés, parfois non dirigés — pèlerins, manoeuvres, ouvriers agricoles saisonniers — qui se dispersent peu après être entrés dans le pays;iv) les groupes '— tribus par exemple — qui franchissent les frontières de façon imprévisible.

Le Comité estime que ces quatre groupes doivent être considérés comme tombant sous le coup des dispositions de l’article 103 du Rè^ement. En conséquence, le Comité ne juge pas nécessaire de modifier, comme l’a recommandé le Comité d ’experts du Palu­disme, les termes de l’article 103.

Le Comité sousent aux recommandations suivantes du Comité d’experts du Paludisme:

i) Les touristes et les personnes voyageant pour leurs affaires, qui, en général, sont des citadins et, au cours de leur séjour dans des pays impaludés, se logent dans des villes, devraient être dûment avertis:

а) des moyens de se protéger contre la maladie lorsqu’ils se rendent dans un pays impaludé;

б) de ce qu’il convient de faire s’ils ont des accès de fièvre après leur retour chez eux.

Prevention o f réintroduction o f malaria into areas from which it has been eradicated

(Extracts from the Twelfth Report o f the Committee on International Quarantine

adopted by the Seventeenth World Health Assembly)

The Committee^ considered section 3 (Prevention of réintro­duction of malaria into areas from which it has been eradicated) contained in the tenth report of the Expert Committee on Malaria.^ The Committee recalls that in its fifth report * * * it had recommended that:

(i) persons on international journeys should not be subjected to any special sanitary measure;(ii) special measures should be applied to various groups;(Ui) appropriate steps should be taken against mosquitos in frontier zones and in the 'centres where the above-mentioned groups assemble—it being understood that, under Article 103 of the International Sanitary Regulations, a country considering itself to be in danger is entitled to apply the measures indicated;

(iv) in international frontier zones of the countries concerned, common control measures should be adopted to avoid carrying the disease from one country to another; and(v) a system of full exchange of information on the movement of population groups and on the susceptibility and resistance of anopheline vectors to insecticides should be instituted.

The Expert Committee on Malaria defined four groups of tra­vellers which may present a danger in the international spread of malaria:

(i) labour forces of large enterprises or organized groups led by ofiicers or chiefs and provided with a medical service, such as troops;(ii) nomadic or semi-nomadic groups, under one or more chiefs;

(iii) imorganized or loosely organized groups which may be leaderless such as pilgrims, labourers, seasonal agricultural workers who soon disperse ^ te r entry into a country;(iv) groups such as tribes which cross borders in an irregular matmer.

The Committee is of the opinion that these four groups should be considered as groups covered by Article 103 of the Regulations. The Committee, therefore, considers that there is no need to amend Article 103 as recommended by the Expert Committee on Malaria.

The Comndttee is in agreement with the following recommenda­tions of the Expert Coirunittee on Medaria:

0) Tourists or businessmen, who usually live in towns and, when travelling through malarious countries, usually sleep in towns, should be offered information on:

(а) how to protect themselves against malaria when entering malarious countries; and(б) what steps to take should they have fever after returning to their coùntry of residence.

Un cas spécial à considérer est celui des membres d’expé­ditions scientifiques ou des chasseurs de gros gibier qui s’exposent davantage à l’infection.

ii) n conviendrait que les médecins chargés de veiller sur la santé des membres des équipages des navires et aéronefs connaissent bien le diagnostic et le traitement du paludisme, ainsi que les mesures de prophylaxie individuelle de cette maladie. Il faudrait aussi que les exploitants de lignes aériennes et les armateurs fassent en sorte que tous les membres des équipages des navires et aéronefs touchant des ports ou des aéroports de zones impa­ludées soient sounais à un traitement suppressif contrôlé d’une durée convenable.

(iii) Les étudiants, hommes d ’affaires ou membres de missions qui se rendent d ’une région impaludée dans des pays où l’éradi­cation du paludisme a été réalisée et où persistent les conditions propices à la transmission (zones de réceptivité) et qui, résidant généralement dans des villes, ne représentent par conséquent qu’un faible risque sous le rapport de la transmission, devraient néanmoins être invités à prendre des sporontocides au cas où ils devraient passer des nuits à la campagne. II y aurait lieu de leur remettre à l’entrée une carte d ’information ou d ’avertisse­ment appropriée. Le Comité juge préférable de leur remettre une carte d’avertissement plutôt que d ’instituer le système combiné questionnaire/carte d’information proposé en annexe au dixième rapport du Comité d’experts du Paludisme.

■iv) Les ressortissants d ’un pays de réceptivité rentrant chez eux après un séjour dans une zone impaludée devraient recevoir des indications plus détaillées.

Le Comité s’associe à la recommandation suivante du Comitéd’experts du Paludisme:

Le Comité d’ecperts du Paludisme,

Reconnaissant la valeur du système d’échange international de renseignements institué par l’OMS au sujet des cas de paludisme survenant dans des zones parvenues à la phase de consolidation ou à la phase d’entretien.

RECOMMANDE que l’Organisation mondiale de la Santé ajoute aux renseignements qu’eUe diffuse périodiquement:

i) une liste des ports et aéroports internationaux qui, bien que situés dans des zones impaludées, sont protégés contre le risque de transmission;ii) une liste des cas de paludisme importés dans tous les pays se trouvant à la phase d ’«itretien, avec classemoit selon l’espèce du parasite et le pays d’ori^ne;iii) une liste des localités où des souches de parasites résistant à la chloroquine ont été signalées.

En conclusion, le Comité estime qu’aucun amendement ne doit être actuellement apporté aux dispositions du Règlement sanitaire international ayant trait au paludisme, mais il croit nécessaire de consacrer une attention spéciale aux groupes de voyageurs visés par l’article 103 et, lorsqu’il y a lieu, de donner aux autres voyageurs intemationamq à teiu entrée dans le pays, une carte d’avertissement.

A special case would be that of scientific or hunting expe­ditions whose exposure to risk would be greater.

(ii) The medical ofiScers responsible for crews of ships and aircraft should be adequately trained in the diagnosis and treat­ment of malaria and in measures of personal prophylaxis. Oper­ators and shipowners should ensure that all members of crews of ships and aircraft touching ports and airports in malarious areas are subjected to supervised suppressant treatment during a suitable period of time.

(iii) Persons such as students, busmessmen, and members of missions originatmg in malarious areas and proceeding to areas from which malaria has been eradicated and where conditions for transmission persist (recipient areas), who would probably live in towns and therefore present little danger for transmission, should be advised to take sporontocidal treatment if they plan to spend nights in the countryside. A suitable information or wammg card should be given to these individuals on entry. The Committee is of the opinion that a warning card should be ÿven rather than the proposed combination questionnaire and information card contamed in the annex to the tenth report of the Expert Committee on Malaria.

(iv) Nationals of a recipient area returning home from a stay in a malarious country should be given more detailed information.

The Comndttee is in agreement with the following quoted recom­mendation of the Expert Committee on Malaria:

The Expert Committee on Malaria,

Recognizing the value of the international exchange of infor­mation carried out by the World Health Organization on the occurrence of cases of malaria in consolidation or maintenance areas.

RECOMMENDS that the World Health Organization should add to its periodic information the following items:

(i) a list of international ports and mrports which, althou^ located in malarious zones, do not present risks of malaria transmission;(ii) a list of malaria cases imported into all countries in the phase of maintenance, classifying the cases according to the spedes of parasites and their country of origin;(iii) a list of localities where chloroquine-resistant strains of parasite have been described.

In conclusion, the Commttee is of the opinion that there is no need at the present time to amend the International Sanitary Regulations in respect of malaria, but does consider that special attention should be given to those groups of travellers covered by Article 103, and that, where appropriate, other international trav e ll^ should receive ii^ormation by means of a warning card given to these travellers on entry.

Centre Mondial de la Grippe. — Les tests d ’inhibition de l’hémag­glutination ont montré que les trois souches de Virus B envoyées récemment de l’Inde (B/Coonoor/1/64, B/Coonoor/2/64 et B/ Coonoor/3/64) sont proches les unes des autres bien qu’elles pré­sentent de légères différences entre elles. Elles sont analogues, mais non identiques, à B/Johannesburg/33/58 et à des souches de virus B isolées récemment.

Les tests d’inhibition de l’hémagglutination ont également montré que les souches A2/Prague/l/64 et A2/Prague/2/64 sont plus proches de A2/England/12/64 que de A2/Netherlands/65/63 et de A2/Singapore/l/57.

World Influenza Centre. — I t has been shown by the hsnnagglu- tination inhibition test that the three strains of M uenza Virus B sent recently from India (B/Coonoor/1/64, B/Coonoor/2/64 and B/Coonoor/3/64) are closely related though they differ slightly from each other. They are similar to but not identical with B/ Johannesburg/33/S8 and strains of influenza Virus B isolated recently.

It has also been shown by the hæmagglutination inhibition test that the strains A2/Prague/l/64 and A2/Prague/2/64 are more closely related to A2/England/12/64 than to A2/Netherlands/6S/63 and A2/Singapore/l/57.
