General InstructionsWho Can Use this ApplicationIf you are a U.S. citizen who lives or has an address within the United States, you can use the application in this booklet to:• RegistertovoteinyourState,• Reportachangeofnametoyourvoterregistration

office,• Reportachangeofaddresstoyourvoter

registrationoffice,or• Registerwithapoliticalparty.

ExceptionsPlease do not use this application if you live outside theUnitedStatesanditsterritoriesandhavenohome(legal)addressinthiscountry,or if you are in the militarystationedawayfromhome.UsetheFederalPostcardApplicationavailabletoyoufrommilitarybases,Americanembassies,orconsularoffices.

New Hampshire town and city clerks will accept this application only as a request for their own absentee votermail‑inregistrationform.North Dakotadoesnothavevoterregistration.Wyominglawdoesnotpermitmailregistration.

How to Find Out If You Are Eligible to Register to Vote in Your StateEachStatehasitsownlawsaboutwhomayregisterandvote.ChecktheinformationunderyourStateintheState Instructions. All States require that you be a United StatescitizenbybirthornaturalizationtoregistertovoteinfederalandStateelections.FederallawmakesitillegaltofalselyclaimU.S.citizenshiptoregistertovoteinanyfederal, State, or local election. You cannotberegisteredtovoteinmorethanoneplaceatatime.

How to Fill Out this ApplicationUse both the Application Instructions and State Instructionstoguideyouinfillingouttheapplication.• First,readtheApplicationInstructions.These

instructionswillgiveyouimportantinformationthatappliestoeveryoneusingthisapplication.

• Next,findyourStateundertheStateInstructions.UsetheseinstructionstofilloutBoxes6,7,and8.AlsorefertotheseinstructionsforinformationaboutvotereligibilityandanyoathrequiredforBox 9.

When to Register to VoteEachStatehasitsowndeadlineforregisteringtovote.CheckthedeadlineforyourStateonthelastpageofthis booklet.

How to Submit Your ApplicationMail your application to the address listed under your State in the State Instructions. Or, deliver the applicationinpersontoyourlocalvoterregistrationoffice.TheStatesthatarerequiredtoacceptthenationalformwillacceptcopiesoftheapplicationprintedfromthecomputerimageonregularpaperstock,signedbytheapplicant,andmailedinanenvelopewiththecorrectpostage.

First Time Voters Who Register by MailIfyouareregisteringtovoteforthefirsttimeinyourjurisdictionandaremailingthisregistrationapplication,Federallawrequiresyoutoshowproofofidentificationthefirsttimeyouvote.Proofofidentificationincludes:• Acurrentandvalidphotoidentificationor• Acurrentutilitybill,bankstatement,government

check,paycheckorgovernmentdocumentthatshowsyournameandaddress.

VotersmaybeexemptfromthisrequirementiftheysubmitaCOPYofthisidentificationwiththeirmailinvoterregistrationform.IfyouwishtosubmitaCOPY, pleasekeepthefollowinginmind:• Yourstatemayhaveadditionalidentification

requirementswhichmaymandateyoushowidentificationatthepollingplaceevenifyoumeettheFederalproofofidentification.

• Donotsubmitoriginaldocumentswiththisapplication, only COPIES.

If You Were Given this Application in a State Agency or Public OfficeIfyouhavebeengiventhisapplicationinaStateagencyorpublicoffice,itisyourchoicetousetheapplication. If you decide to use this application to registertovote,youcanfillitoutandleaveitwiththeStateagencyorpublicoffice.Theapplicationwillbesubmittedforyou.Or,youcantakeitwithyoutomailtotheaddresslistedunderyourStateintheStateInstructions.Youalsomaytakeitwith you to deliver in person to your local voter registrationoffice.Note:ThenameandlocationoftheStateagencyor public office where you received the application willremainconfidential.Itwillnotappearonyourapplication. Also, if you decide not to use this applicationtoregistertovote,thatdecisionwillremainconfidential.Itwillnotaffecttheserviceyoureceivefromtheagencyoroffice.

Box 1 — NamePutinthisboxyourfullnameinthisorder—Last,First,Middle.Donotusenicknamesorinitials.Note:Ifthisapplicationisforachangeofname,pleasetell us in Box A (on the bottom half of the form) your fullnamebeforeyouchangedit.

Box 2 — Home AddressPutinthisboxyourhomeaddress(legaladdress).Donotputyourmailingaddresshereifitisdifferentfromyourhomeaddress.Donotuseapostofficeboxorruralroutewithoutaboxnumber.Refertostate‑specificinstructionsforrulesregardinguseofroutenumbers.

Note:IfyouwereregisteredbeforebutthisisthefirsttimeyouareregisteringfromtheaddressinBox2,please tell us in Box B (on the bottom half of the form) theaddresswhereyouwereregisteredbefore.Pleasegiveusasmuchoftheaddressasyoucanremember.

Also Note: If you live in a rural area but do not have a street address, or if you have no address, please show whereyouliveusingthemapinBox C (at the bottom of the form).

Box 3 — Mailing AddressIfyougetyourmailatanaddressthatisdifferentfromtheaddressinBox2,putyourmailingaddressinthisbox.IfyouhavenoaddressinBox2,youmust write in Box3anaddresswhereyoucanbereachedbymail.

Box 4 — Date of BirthPutinthisboxyourdateofbirthinthisorder—Month,Day,Year.Be careful not to use today’s date!

Box 5 — Telephone NumberMostStatesaskforyourtelephonenumberincasethere are questions about your application. However, you do nothavetofillinthisbox.

Box 6 — ID NumberFederallawrequiresthatstatescollectfromeachregistrantanidentificationnumber.Youmustrefertoyourstate'sspecificinstructionsforitem6regardinginformationonwhatnumberisacceptableforyourstate. If you have neither a drivers license nor a social securitynumber,pleaseindicatethisontheformandanumberwillbeassignedtoyoubyyourstate.

Box 7 — Choice of PartyInsomeStates,youmustregisterwithapartyifyouwanttotakepartinthatparty’sprimaryelection,caucus,orconvention.TofindoutifyourStaterequiresthis,seeitem7intheinstructionsunderyourState.

Ifyouwanttoregisterwithaparty,printintheboxthefullnameofthepartyofyourchoice.

If you do notwanttoregisterwithaparty,write“noparty”orleavetheboxblank.Donot write in the word “independent”ifyoumean“noparty,”becausethismightbeconfusedwiththenameofapoliticalpartyinyour State. Note: Ifyoudonotregisterwithaparty,youcanstillvoteingeneralelectionsandnonpartisan(nonparty)primaryelections.

Box 8 — Race or Ethnic GroupAfewStatesaskforyourraceorethnicgroup,inordertoadministertheFederalVotingRightsAct.TofindoutifyourStateasksforthisinformation,seeitem8intheinstructionsunderyourState.Ifso,putinBox8thechoicethatbestdescribesyoufromthelistbelow:• AmericanIndianorAlaskanNative• AsianorPacificIslander• Black,not of HispanicOrigin• Hispanic• Multi‑racial• White,not ofHispanicOrigin• Other

Box 9 — SignatureReviewtheinformationinitem9intheinstructionsunderyourState.Beforeyousignormakeyourmark,makesurethat:

(1)YoumeetyourState’srequirements,and(2)YouunderstandallofBox9.

Finally,signyourfullnameormakeyourmark,andprinttoday’sdateinthisorder—Month,Day,Year.Iftheapplicantisunabletosign,putinBox D the name,address,andtelephonenumber(optional)oftheperson who helped the applicant.

Application InstructionsBeforefillingoutthebodyoftheform,pleaseanswerthequestionsonthetopoftheformastowhetheryouareaUnitedStatescitizenandwhetheryouwillbe18yearsoldonorbeforeElectionDay.Ifyouanswernotoeitherofthesequestions,youmaynotusethisformtoregistertovote.However,statespecificinstructionsmayprovideadditionalinformationoneligibilitytoregistertovotepriortoage18.
