INSTRUCTIONS AND SUMMARY FOR RECORDER COMPLAINT TO CHARGING PARTY BASED ON A PATTERN OF CONDUCT

A Complaint involving a pattern of conduct must be brought within five (5) years of the earliest instance referenced in the Complaint. The ACBL Code of Disciplinary Regulations (CDR) requires a Complaint to be in writing and include certain things: (1) allege an act or conduct not in conformance with the CDR, (2) include evidence to support your accusation, (3) request that Charges be brought to the appropriate Disciplinary Body for a hearing, and (4) be submitted to the Charging Party with proper jurisdiction.

"To:" line Unit President if the pattern of conduct was discovered by the Unit Recorder or District President if the pattern of conduct was discovered by the District Recorder.

"From:" line Your name

"Date:" line Insert the date of submission of the Recorder Complaint to the Charging Party.

"Subject:" line Provide the name of the person you are accusing and their ACBL# if available.

Complaint

Fill in the brackets on the form with the proper information.

Prima Facie Evidence: Fill in the brackets on the form with the proper information.

In this area list each incidents involved in this pattern of conduct complaint. Include the date of the incident, the reporter of the incident, the reporter's ACBL #, a brief description of the incident, and the location of where the incident occurred. Examples are below:

June 11, 2014, Tom Jones, ACBL #1234567, reported that Jack was rude to his opponent at the 2014 Spring

NABC in Memphis, TN. (attached)

July 15, 2015, Ron Smith, ACBL #7654321, reported that Jack threatened his opponent and asked if he wanted to

"settle this outside" during the STaC game held at the ABC Bridge Club in Atlanta. (attached)

Based upon the above complaint, pursuant to the Code of Disciplinary Regulation (CDR) 5.2.1 (c), I hereby request that Charges be brought to the appropriate Disciplinary Body for a hearing. This statement is required in order for your submission to be considered a Complaint under the CDR.
