1 ELIGIBILITY & REQUIREMENTS

PROGRAM ELIGIBILITY

To apply for the Pilmico Jr. Veterinary Medicine and Animal Science Congress, the applicant must:

Be a Filipino citizen, 25 years old or younger at the time of the Congress Be a bonafide undergraduate Veterinary Medicine or Animal Science student of a School / University. Be a graduating student by the start of Academic Year 2015-2016. Must not have any failing grade or any disciplinary cases inside and outside campus. Be an active leader or o�cer of any student organization – student government, academic, cause-oriented, socio-civic or religious in nature.

PROGRAM APPLICATION REQUIREMENTS

The applicant must submit all of the following documents:

A fully completed application form. A certified true copy of the applicant’s college transcript for all undergraduate course work, ending with the grades from the last semester of SY 2014 - 2015. Certification from the School Registrar stating the overall cumulative GPA / GWA of the applicant and the percentage equivalent grade. Certifications supporting leadership experience, awards received, and community involvements. A signed recommendation letter.

All application documents submitted will become the property of the company and will NOT be returned to the applicants.

Completed applications should be submitted to our o�ces on or before April 24, 2015.

6 RECOMMENDATION

Choose one (1) evaluator, either academe or non-academe, who will write a recommendation letter for you.

TO THE APPLICANT

The academe evaluator can be any of the following:

Class Adviser School Guidance Counselor / Student A�airs O�cer School Principal / Dean Adviser of a school-based organization, where the applicant is a member of Teacher/Professor of the applicant in at least one subject

The non-academe evaluator can be anyone from the community (parish priest, barangaycaptain / councilor, or any person with authority) who can vouch for the potential and ability of the applicant.

Ask your Evaluator to write a recommendation letter, answering the questions indicated in the next page. Your Evaluators should sign the recommendation letter and put it inside a white letter envelope. The letter envelope should be sealed, signed across the flap, and returned to you.

The sealed envelope should be enclosed together with your completed application. Please note that unsigned and unsealed recommendation letters will not be honored.

6 RECOMMENDATION

TO THE EVALUATORS

Your candid assessment of the applicant will assist the screening committee of the Pilmico Jr. Veterinary Medicine and Animal Science Congress. We greatly appreciate your time and e�ort in writing a recommendation. This recommendation is to be returned to the applicant. Please seal and sign the back flap of the envelope.

On your professional letterhead, please type or write a one-page recommendation for the applicant, answering the following items below:
