RCDC Connections Special Edition: November 2014 RCDC: Together We Can! Riverside Community Diabetes Collaborative (RCDC) is working together to raise awareness about diabetes in Riverside and beyond! Riverside Community Diabetes Col- laborative is your local resource for diabetes news, events, and more! RCDC is a group of over 40 dia- betes-focused organizations and businesses that work together to strengthen community programs and services for the prevention and management of diabetes in River- side and surrounding areas. coUrse descriptionthe tcoYd making the connection continuing medical education program offers a unique opportunity to bridge patient and professional education, demonstrating the link between clinical lessons taught in the classroom to the real life patient experience. the ultimate goal of this activity is to educate caregivers about the most effective cutting edge strategies to help patients with diabetes reach their treatment goals and avoid the associated acute and chronic complications.

this activity will focus on four distinct areas of diabetes management, and integrate case presentations as well as an audience response system into the lectures to facilitate more active participation. the four areas are:

u promoting behavioral change in diabetesu oral agents for use in patients with type 2 diabetesu injectable agents for use in patients with type 2 diabetesu intensive management strategies for patients with type 1 diabetes

in addition, this course is being held in tandem with the 20th Annual taking control of Your diabetes patient education conference and will give providers the opportunity to observe and interact with people who have diabetes and are engaged in their own intensive educational environment. making that connection with patients, providers will acquire new skills and insight into what empowers patients with diabetes to develop healthy self-management of their condition.

tArget AUdiencethis course is designed for diabetes healthcare providers including: endocrinologists, primary care physicians, nurse practitioners, physician assistants, certified diabetes educators, and other medical professionals wanting to expand their knowledge of diabetes management.

• Diabetes Mini-Series: Seminars in February, May and August with healthcare professionals debunking diabetes myths and discussing the connection between diabetes, nutrition, physical activity and emotional health.

• Monthly Cooking Classes: A registered dietitian will show how to make healthy, easy meals to help keep diabetes under control!

The Big Blue Test is a program of the Diabetes Hands Foundation that rallies communities to experi-ence the impact that small changes can have on their health. Taking the Big Blue Test is easy:

► If you have diabetes, test your blood sugar (If you don’t have diabetes, you don’t need to test!)

► Exercise for at least 14-20 minutes.► Test again (if you have diabetes), and► Share your experience on BigBlueTest.org or through the app for iPhone or Android.
