In response to such request, Holistic Industries submits the following documents to facilitate the Department's evaluation of the company's three (3) distinct Management and Operations Profile timely submitted in September of 2015: l. Holistic Industries, Inc. 's new Certificate of Good Standing dated December 4, 2015; 2. Copies of the two loan agreements identified in the responses to Question C.12. Note, the attached agreements are short-form agreements. The long-form versions of the agreements are currently being drafted by company counsel. Upon finalization and execution, Holistic Industries shall submit the long-form loan agreements to the Department for review. At that time, Holistic Industries shall also submit an independent legal opinion evidencing that the agreements are compliant with Massachusetts law and the Department's Non-Profit Guidance. The company acknowledges that it must submit the long-form loan agreements and accompanying legal opinion prior to receiving a Provisional Certificate of Registration; 3. Holistic Industries, Inc. 1s m receipt of the Department of Public Health's (the "Department") Request for Information dated December 4, 2015. In response to such request, Holistic Industries submits the following documents to facilitate the Department's evaluation of the company's three (3) distinct Management and Operations Profile timely submitted in September of 2015:

l. Holistic Industries, Inc. 's new Certificate of Good Standing dated December 4, 2015;

As VP, he is responsible for daily operations, including interfacing with top customers, staffing, purchasing, advertising and promotions, and the company's award-winning website, Cellar.com. In its 66 years of existence, Schneider's has never been cited for a violation by DC's Alcoholic Beverage Regulation Administration.

36. Provide a summary of the RMD's policies and procedures for the provision of marijuana for medical use to registered qualifying patients with verified financial hardship without charge or at less than the

Holistic will offer registered qualifying patients with verified financial hardship, as such tennis defined in 105 CMR 725.004, subsidized access to products and services on a sliding scale basis detennined by need (potentially up to 100% off of retail prices). Specifically, Holistic will offer subsidies to patients who are verified recipients of MassHealth and/or Supplemental Security Income, and patients whose income do not exceed 300% of the poverty level, adjusted for family size.

To qualify for hardship subsidies, a patient must complete our Subsidy Request Application Fonn and submit proof of financial hardship, such as pay stubs, recent tax returns, MassHealth ID cards, or SSI letters, which will be reviewed by dispensary management who will ultimately decide the appropriate level of discount based on need in accordance with the company's approved subsidy qualifications fonnula. Qualifying patients will have to renew their participation in the financial hardship program on an annual basis.

In recognition of the diverse qualifying conditions, some of which are best addressed using vaporization as a means of administration, our RMD will offer discounts and/or deferred payment plans for financial hardship program participants to purchase vaporizer equipment. Such equipment can be relatively expensive for the highest quality products, so we will take steps to assure both affordability and access to these preferred medical devices.
