How to Use This Guide
This guide provides step-by-step instructions for using Oregon’s Patient Safety Reporting Program (PSRP)
online account management system. Instructions may vary depending on the user rights granted to you
by your facility.
The Oregon Patient Safety Commission (OPSC)’s Patient Safety Consultants are available to assist you at

User Rights
The PSRP online system allows users to request one of four different levels of user rights. Each level
allows users to complete specific functions. A facility may authorize as many users as desired; however,
each facility must designate one PSRP Manager to grant or deny new user requests and manage current
user accounts. Any user can manage his or her profile and generate recognition target dashboards. The
specific functions granted for each level of rights are:
Read Only
User can view any submitted or in-progress report for the facility
Read and Write
User can view and edit any report in-progress or start a new report for the facility
Read, Write, Submit
User can view, edit, and submit any report in-progress or start a new report for the facility
PSRP Manager
User can view, edit, and submit any report in-progress or start a new report for the facility. PSRP
Managers have the highest level of rights at a facility and can also:
Grant or deny user account requests
Create an account for a new user
Close a user account
Reactivate an expired user account

Request a New User Account
1. Go to https://psrp.oregonpatientsafety.org/reports/account/request
2. Choose your reporting segment: ambulatory surgery center, hospital, nursing facility or pharmacy.
a. If you would like to report for facilities in more than one reporting segment, you must initially
choose only one reporting segment. Once your account has been created, you may add facilities
in additional segments using the internal request process (see page 10).
3. Choose your healthcare system from the dropdown menu.
a. If your facility is a non-system hospital, choose “non-system.”
b. If your facility is an independent nursing facility or pharmacy, choose “Independent.”

4. Complete the user information fields. You must provide your full name, email address, and
password to complete this form. Completing information in all fields will help your PSRP Manager
recognize who you are and may speed up the approval process.
5. Click on Add Facility to add a row to the table.
6. Select the facility for which you would like user rights. Click on the drop down menu to display the
list of facilities in your healthcare system.
7. Select the user rights you would like from the drop down menu.
a. To request rights for more than one facility, click Add Facility to add another row to the table
and follow steps 4-6 for each additional facility.
b. To remove a facility from your form before submitting, click on the red at the end of the row.
8. Click Submit at the bottom of the form when you have completed the required fields and added the
necessary facilities with your desired user rights. You will receive a confirmation email from
[email protected] letting you know that your request is pending.

Change My Password
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org/ and click the Log in button.
2. Click on My Profile.
3. Click on Change My Password.
4. Enter a new password and click Save.

Update My User Account Information
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on My Profile.
3. Click Edit.
4. Make changes to your information.
5. Click Save.

View My Current Facility Access
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on My Profile.
3. Click on the My Facilities tab. Your current facility access will display next to each facility.

Request Additional Facility Rights
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on My Profile.
3. Click on the Requests for Access tab.
4. Click Edit.
5. Click Request Additional Facility.
6. Choose a facility from the facility drop down menu. Your facility options are based on your
healthcare system. To request access to a facility that you do not see on the list, please contact the
Oregon Patient Safety Commission at 503-928-6158.
7. Choose the appropriate user rights from the Account Type drop down menu.
8. Click Save.
Note: Your request must be approved before you can access the PSRP online system. If you work at
a healthcare facility with a PSRP Manager, your PSRP Manager will grant or deny your request. Only
a Commission Patient Safety Consultant can grant or deny your PSRP Manager request after
obtaining approval from your facility's leadership. You will receive an email when your request has
been approved or denied.

Remove My Facility Rights
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on My Profile.
3. Click on the My Facilities tab then click on Edit.
4. Click on the red X next to the facility you want to remove.
5. Click Yes when the pop up box asks, “Are you sure you want remove these rights?”

Close My Account
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on My Profile.
3. Click on the My Facilities tab then click on Edit.
4. Click on each red X in the Action column to remove all listed facilities.
5. Click Yes when the pop up box asks, “Are you sure you want remove these rights?”
6. Click Yes when the pop up box asks, “This account will now be closed. Would you like to continue?”
This question will appear after you have removed all listed facilities.
7. Click Save. Your account will be closed.

Grant or Deny a User Account Request
PSRP Managers must use their Account Manager to grant or deny user account requests for their
facility/facilities.
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on Manage Users.
3. Click on the Requests tab. New user requests will be highlighted in yellow.
4. Click on the user’s name to open their Account Profile.
5. Click on the Requests for Access tab.
6. Click the Edit button.
7. Click on Grant or Deny in the Approval column.
8. If you are granting the request, skip to step 9.
If you are denying the request…
a. Choose a reason. The reason you choose WILL be shared with the user in the email they
receive.

b. Click Submit in the pop up box.
9. Click Save.
Note: The user will receive a welcome email or a denial email based on your action. If you deny the
request, the email the user receives will specify the reason chosen.

Create a New User Account
PSRP Managers can create other user accounts in their Account Manager. 1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on Manage Users.
3. Click on the Add User button.
4. Complete the User Information on the User Information tab.
5. Click on the Granted Facility Access tab.
6. Click Add to give the user facility access.
7. Choose the user’s facility from the Facility drop down menu.
8. Choose the appropriate user rights from the Account Type drop down menu.
9. Click Save.
Note: The new user will receive a welcome email and a separate email with their password.

Grant Additional Facility Access
PSRP Managers can grant active users access to additional facilities. 1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on Manage Users.
3. Click on the user’s name in the View, Edit, Remove Users tab.
4. Click on the Granted Facility Access tab.
5. Click Edit.
6. Click Add to give the user additional facility access.
7. Choose the facility that you want to add the user to from the Facility drop down menu.

8. Choose the appropriate user rights from the Account Type drop down menu.
9. Click Save.
Note: The user will receive an email alerting them to the change in their account.

Remove a User’s Facility Rights
PSRP Managers can remove a user’s facility rights in their Account Manager. 1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on Manage Users.
3. Click on the View, Edit, Remove Users tab.
4. Click on the user’s name in the View, Edit, Remove Users tab.
5. Click on the Granted Facility Access tab then click on Edit.
6. Click on the red X next to the facility you want to remove.
7. Click Yes when the pop up box asks, “Are you sure you want remove these rights?”
8. Click Save.
Note: The user will receive an email alerting them to the change in their account.

Close a User Account
PSRP Managers cannot close their own account in Account Manager, but they can close other user
accounts (for instructions on how to close your own account, see page 12). The Close User Account
button is only available for PSRP Managers with multiple users at their facility. 1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on Manage Users.
3. Click on the Close User Account button.
4. Select the user account you would like to close from the dropdown menu and click on the
Continue button.
5. Select the reason for closing the account and click on the Close Account button.
6. Click Yes in the confirmation pop up box. The user will receive an email confirming that their
account has been closed.

Reactivate a User Account
PSRP Managers can reactivate other user accounts in their Account Manager. The Reactivate Expired
Account button is only available for PSRP Managers with multiple users at their facility. When a user at your facility tries to login to an expired account, all PSRP Managers for facilities to which
the user has rights will receive an email letting them know that the user’s account needs to be
reactivated. To reactivate a user’s account:
1. Log in to PSRP. Go to https://psrp.oregonpatientsafety.org and click the Log in button.
2. Click on Manage Users.
3. Click on the Reactivate Expired Account button.
4. Select the user account you would like to reactivate from the dropdown menu and click on the
Reactivate button.
5. Click OK in the confirmation pop up box. The user will receive an email confirming the reactivation
of their account.
